The effect on breathing of abruptly stopping carotid body discharge.

In bilaterally vagotomized, decerebrate or pentobarbitone anaesthetized cats, intense carotid body discharge (FET O2 ca. 0.075) was abruptly removed by injections centrally of 100% O2-equilibrated Ringer into both external carotid arteries. In an inspiration the injections usually shortened that inspiration, reduced its volume and prolonged the immediately following expiration. Early in expiration they prolonged that expiration, but later in expiration they shortened it. The inspiratory results can be reconciled with von Euler's model of the inspiratory off-switch if the off-switch acts early

Inspiratory muscle strength training improves lung function in patients with the hypermobile Ehlers-Danlos syndrome: A randomized controlled trial.

As exertional inspiratory dyspnea is a common disabling complaint in hypermobile Ehlers-Danlos syndrome (hEDS) often also known as joint hypermobility syndrome (JHS), we investigated inspiratory muscle (IM) strength in patients with hEDS, and we assessed the effects of IM training (IMT) on IM strength, lung function, and exercise capacity. A prospective evaluation of IM strength followed by a randomized controlled trial of IMT was performed in women with hEDS. Sniff nasal inspiratory pressure (SNIP) was used to routinely measure IM strength and IMT was carried out using a pressure threshold de

Inspiratory Muscle Training plus Pulmonary Rehabilitation versus Rehabilitation alone in COPD A Systematic Review of Randomized Controlled Trials.

BACKGROUND: Pulmonary rehabilitation (PR) is an established intervention for COPD, but the added value of inspiratory muscle training (IMT) within PR remains uncertain. This systematic review examined whether IMT plus PR provides benefits beyond PR alone in adults with moderate to severe COPD. METHODS: PubMed, ScienceDirect, Cochrane Library, and Web of Science were searched from inception to January 2023. Randomized controlled trials (RCTs) comparing IMT+PR with PR alone in adults with moderate to severe COPD were included. Primary outcomes were inspiratory muscle strength (PImax), dyspnea,

Inspiratory and skeletal muscle strength and endurance and diaphragmatic activation in patients with chronic airflow limitation.

To determine whether patients with chronic airflow limitation have a specific alteration in skeletal muscle performance, the strength and endurance of inspiratory and limb muscles were compared in 11 patients with chronic airflow limitation and 11 control subjects during maximal voluntary contractions. Peak inspiratory pressure at observed functional residual capacity (FRC) was significantly less in the patients than in the control subjects (mean 72 (SD 25) v 93 (21) cm H2O), though only two patients had low maximal pressures across a wide volume range. Maximal voluntary torque of the elbow fl

Respiratory muscle length and strength in patients with chronic abdominal distension.

Starling's law (the energy of muscle contraction is proportional to the initial fiber length) has been applied to contraction of inspiratory muscles. Its application to the expiratory muscles is difficult because both maximal length and maximal pressure development occur at total lung capacity (TLC). We hypothesize that decrease of both inspiratory (Pimax) and expiratory (Pemax) muscle strength in chronic ascites (CA) will reflect generalized muscle weakness and stretching of both the diaphragm and abdominal wall as well. To test this hypothesis, we evaluated Pimax and diaphragm length (at fun

Non-organic acute upper airway obstruction: description and a diagnostic approach.

We describe 3 cases of upper airway obstruction in which no organic cause could be identified. All 3 patients presented with severe dyspnea and stridor. Direct laryngoscopy and bronchoscopy failed to demonstrate any lesion. Flow-volume loops showed a pattern of severe variable extrathoracic airway obstruction with a decreased peak inspiratory flow and an increased ratio of the forced expiratory flow at 50% vital capacity to the forced inspiratory flow at the same lung volume. Plethysmographically measured airway resistance was normal in the 2 patients for whom it was obtained. We believe a fun

Respiratory muscle strength in radiographic axial spondyloarthritis: clinical significance and predictors.

INTRODUCTION/OBJECTIVE: Respiratory muscle function may be impaired in axial spondyloarthritis (axSpA) due to mechanical limitations of the thoracic spine and chest wall. However, the extent of respiratory muscle weakness and its clinical associations remain largely underexplored. METHODS: This comparative cross-sectional study included 51 patients with radiographic axSpA (r-axSpA) and 52 healthy controls. Maximal inspiratory (PImax) and expiratory (PEmax) pressures were measured using standardized protocols. Patients with r-axSpA also underwent assessments of pulmonary functions (forced vita

Short-Term Inspiratory Muscle Training Enhances Functional and Metabolic Health in Older Adults.

Background: Age-related declines in respiratory muscle strength and ventilatory efficiency can impair functional capacity and metabolic health in older adults. Inspiratory muscle training (IMT) has been proposed as a practical intervention to counteract these changes, yet its systemic effects remain unclear. This study aimed to examine the effects of short-term IMT on functional capacity, diaphragm thickness, and liver tissue characteristics in healthy elderly men. Methods: Thirty community-dwelling men aged 60-80 years were randomly assigned to an IMT or control group. The IMT group performed

Longitudinal Changes in Maximal Forced Inspiratory Flow and Clinical Outcomes in Patients With COPD.

BACKGROUND: COPD primarily impairs expiratory flow due to progressive airflow obstruction and reduced lung elasticity. Increasing evidence underlines the importance of inspiratory flow as a biomarker for selecting inhaler devices and providing ancillary aerodynamic information. RESEARCH QUESTION: Do the longitudinal changes in maximum forced inspiratory flow (FIFmax) influence acute exacerbations and lung function decline in patients with COPD? STUDY DESIGN AND METHODS: This longitudinal study evaluated FIFmax in patients with COPD over a 7-year period from 2004 to 2020. Eligible patients we

[Inspiratory muscle resistance to fatigue during exercise and simulated airway obstruction].

Respiratory muscle fatigue can develop during simulated airway obstruction. The aim of this study was to characterize the pattern of inspiratory muscle fatigue and to assess the resistance to fatigue of diaphragm (D), parasternal (PS), sternocleidomastoid (SM) and scalene (SC). 8 healthy untrained subjects participated in this study. To identify signs of inspiratory muscles fatigue development electromyographic activity of D, PS, SCM and SC was recorded during 5-min exercise with loaded breathing (40 cm H2O/L · s(-1)). The before-to-after exercise measurements of maximal inspiratory pressure (

Performance of maximum inspiratory pressure tests and maximum inspiratory pressure reference equations for 4 race/ethnic groups.

BACKGROUND: Maximum inspiratory pressure (MIP) is an important and noninvasive index of diaphragm strength and an independent predictor of all-cause mortality. The ability of adults over a wide age range and multiple race/ethnicities to perform MIP tests has previously not been evaluated. METHODS: The Multi-Ethnic Study of Atherosclerosis recruited white, African American, Hispanic, and Chinese American participants, ages 45-84 years, and free of clinical cardiovascular disease in 6 United States cities. MIP was measured using standard techniques among 3,849 Multi-Ethnic Study of Atherosclero

Prevalence of sleepiness and its relation to autonomic evidence of arousals and increased inspiratory effort in a community based population of men and women.

Degrees of sleep apnoea and daytime sleepiness are quite common in community populations. However the relationship between the two is poor, although sleepiness does correlate better with a history of snoring. It has been suggested that sleep can be fragmented by upper airways obstructive events, short of full apnoeas or hypopnoeas, and that these events may not provoke full cortical arousal, but be detectable through activation of the autonomic system. Failure to detect both these could mask a relationship between 'sleep apnoea' and daytime sleepiness. We have therefore measured sleepiness (Ep

The inspiratory work of breathing during assisted mechanical ventilation.

We quantified the mechanical work of breathing in six normal subjects during assisted mechanical ventilation. Using two volume-cycled ventilators of different design, we investigated the influence of minute ventilation (VE) and machine settings of trigger sensitivity and flow during CO2-driven hyperventilation to moderate and high levels (12-24 L/min). Work estimates were derived from plots of esophageal and airway pressure against inflation volume. Peak flow and trigger sensitivity were important determinants of the energy expended, and for each combination of machine settings the work done b

Transcranial direct current stimulation's effect on exercise capacity, respiratory muscle strength, and physical performance in patients with chronic obstructive pulmonary disease: Randomized controlled trial.

BackgroundChronic obstructive pulmonary disease (COPD) is associated with alterations in brain function that contribute to reduced respiratory and skeletal muscle strength, ultimately limiting exercise capacity. Transcranial direct current stimulation (tDCS), a non-invasive neuromodulation technique, has demonstrated efficacy in enhancing lower extremity function in healthy older adults.ObjectiveTo evaluate the effects of a single session of anodal tDCS on exercise capacity, respiratory muscle strength, and physical performance in individuals with COPD.MethodsIn a randomized, double-blind, sha

Cardiopulmonary Rehabilitation Improves Respiratory Muscle Function and Functional Capacity in Children with Congenital Heart Disease. A Prospective Cohort Study.

Critical surgical and medical advances have shifted the focus of congenital heart disease (CHD) patients from survival to achievement of a greater health-related quality of life (HRQoL). HRQoL is influenced, amongst other factors, by aerobic capacity and respiratory muscle strength, both of which are reduced in CHD patients. This study evaluates the influence of a cardiopulmonary rehabilitation program (CPRP) on respiratory muscle strength and functional capacity. Fifteen CHD patients, ages 12 to 16, with reduced aerobic capacity in cardiopulmonary exercise testing (CPET) were enrolled in a CP

Does inspiratory muscle training improve lung function and quality of life in people with inclusion body myositis? A pilot study.

Inclusion body myositis is the most common acquired myositis in adults, predominantly weakening forearm flexor and knee extensor muscles. Subclinical respiratory muscle weakness has recently been recognised in people with inclusion body myositis, increasing their risk of respiratory complications. Inspiratory muscle training, a technique which demonstrates efficacy and safety in improving respiratory function in people with neuromuscular disorders, has never been explored in those with inclusion body myositis. In this pilot study, six adults with inclusion body myositis (age range 53 to 81 yea
