Vascular changes in the chest wall after unilateral resection of the intercostal nerves in the growing rabbit.

In young, growing white New Zealand rabbits the third, fourth, and fifth intercostal nerves were resected anteriorly on the right side. Six months later the animals developed structural left convex scoliosis, with a Cobb angle ranging from 15 to 31 degrees. The vascular structure changes of the anterior chest wall were evaluated by measuring surface temperature and fluorescein intensity of the pectoral muscles, and the capillary density of the pectoral and intercostal muscle and periosteal parts of the ribs after angiography. In five normal control rabbits there was neither scoliotic deformity

Surgical anatomy of the 10th and 11th intercostal, and subcostal nerves: prevention of damage during lumbotomy.

PURPOSE: In a descriptive, inventorial anatomical study we mapped the course of the 10th and 11th intercostal nerves, and the subcostal nerve in the abdominal wall to determine a safe zone for lumbotomy. MATERIALS AND METHODS: We dissected 11 embalmed cadavers, of which 10 were analyzed. The 10th and 11th intercostal nerves, and the subcostal nerve were dissected from the intercostal space to the rectus sheath. Analysis was done using computer assisted surgical anatomy mapping. A safe zone and an incision line with a minimum of nerve crossings were determined. RESULTS: The 10th and 11th inte

Primary bacterial intercostal pyomyositis diagnosis: A case report.

RATIONALE: Pyomyositis is a microbial infection of the muscles and contributes to local abscess formation. Staphylococcus aureus frequently causes pyomyositis; however, transient bacteremia hinders positive blood cultures and needle aspiration does not yield pus, especially at the early disease stage. Therefore, identifying the pathogen is challenging, even if bacterial pyomyositis is suspected. Herein, we report a case of primary pyomyositis in an immunocompetent individual, with the identification of S aureus by repeated blood cultures. PATIENT CONCERNS: A 21-year-old healthy man presented

[Oblique-transverse approach to the extrahepatic bile ducts].

A new access to extrahepatic bile ducts is proposed which keeps the lateral abdominal muscles and intercostal vascular-nervous plexuses intact. The spatial conditions of the access were studied experimentally in 108 corpses. The clinical assessment was tested in 102 operations in comparison with the Kocher access. The oblique-transversal access was found to have advantages from the clinico-physiological viewpoint as well (the development of the postoperative period, restoration of the external respiration function and the function of the abdominal wall muscles).

Anatomic spread of india ink in the human intercostal space with radiographic correlation.

In a study designed to determine the spread of anesthetic solutions in the intercostal space, the spread of india ink was studied in fresh cadavers. They were turned prone and had epidural catheters placed at ribs 4, 6, 8 and 10, 7-8 cm lateral to the midline, by two different techniques. Group 1 had epidural catheters directed laterally into the subcostal groove. Group 2 had catheters directed medially into the intercostal space. A window was dissected in the anterior chest wall to visualize the location of the catheters and the spread of india ink. Catheters in Group 1 were observed to be la

Modified anterolateral approach provides improved accessibility to the lung hilum vascular region for giant mass lung cancer pneumonectomy.

Surgical management of patients with a huge pulmonary mass remains a challenge. The surgical risk is amplified by potential hemorrhage, problematic surgical exposure, difficult handling of the tumor, and poor vascular control. We describe a technique that uses a modified anterolateral thoracotomy to accomplish pneumonectomy. An anterolateral thoracotomy was entered through the 4th or 5th intercostal space. The intercostal muscles of the involved interspace were dissected off long enough but without dividing the overlying chest wall muscles. This crucial maneuver allowed a wide rage of rib spre

The effects of oesophageal distension on diaphragm and laryngeal muscle activity in the anaesthetized cat.

The electromyographic (EMG) activities of diaphragm and laryngeal muscles were recorded during oesophageal distension in anesthetized cats. The responses to distension of the thoracic oesophagus differed from those evoked by distension of the cervical oesophagus. The crural component of the diaphragm (CD) was inhibited by distension of the thoracic oesophagus; distension of the cervical oesophagus did not affect CD EMG. Thyroarytenoid (TA) muscle EMG increased markedly and consistently in response to distension of the cervical oesophagus. Distension of the thoracic oesophagus only produced sta

Cardiac modulation of alpha motoneuron discharges.

Recordings of alpha motoneuron discharges from branches of the intercostal and abdominal nerves in anesthetized cats were analyzed for modulation during the cardiac cycle. Cardiac modulation was assessed by the construction of cross-correlation histograms between the R-wave of the ECG and the largest amplitude efferent spikes. In all but two recordings (which were believed to have either no or few alpha spikes), the histograms showed relatively short duration peaks and/or troughs (widths at half amplitude 4-50 ms) at lags of 10-150 ms. These observations were deduced to result from activity in

Intercostal nerve injury after rib fractures: A case series correlating electrodiagnostic testing with imaging, ultrasound, and surgical findings.

BACKGROUND: Intercostal nerve injury can occur after rib fractures, resulting in denervation of the abdominal musculature. Loss of innervation to the rectus abdominis and intercostal muscles can cause atrophy and subsequent eventration, pain, and cosmetic issues. Intercostal electrodiagnostic testing can diagnose and localize intercostal nerve damage after rib fractures at levels T7 and below. OBJECTIVE: To characterize rib fracture patterns associated with intercostal nerve injury and evaluate the diagnostic and surgical utility of preoperative nerve conduction study/electromyography (NCS/EM

[Intercostal abdominal hernias. Apropos of 2 cases with right hernia].

Two patients presented with abdominal wall hernia in the 9th intercostal space on the right. Such localizations are extremely rare, occurring after closed or open chest or abdominal trauma producing two contiguous orifices in the diaphragm and the intercostal chest wall. The diaphragmatic orifice is due to a low lateral disinsertion and the intercostal orifice is situated in the 7th to 11th space, occurring immediately or later. Intercostal abdominal hernias are sometimes discovered due to pain or digestive or respiratory disorders. Clinical diagnosis is simple. CT scan confirms the clinical d

Respiration-related laryngeal electromyography in children with bilateral vocal fold paralysis.

We present 2 case reports to demonstrate the relationship between laryngeal muscle activity and respiration in children with bilateral vocal fold paralysis (BVFP) by simultaneous laryngeal electromyography (EMG) with recording of chest wall movement and intercostal muscle EMG. Laryngeal EMG was performed together with recording of chest wall movement in a 55-day-old girl who was undergoing tracheostomy for idiopathic congenital BVFP. Normal phasic activity was observed, i.e., the thyroarytenoid (TA) muscle was active during expiration and the posterior cricoarytenoid (PCA) muscle during inspir

Does ¹⁸F-FDG uptake by respiratory muscles on PET/CT correlate with chronic obstructive pulmonary disease?

UNLABELLED: (18)F-FDG muscle uptake is evident in some benign physiologic processes as seen in the respiratory muscles of patients with chronic obstructive pulmonary disease (COPD) and labored breathing. The purpose of this study was to correlate the presence of COPD with the patterns of (18)F-FDG uptake by muscles as demonstrated by PET/CT scans. METHODS: (18)F-FDG PET/CT scans and pulmonary function tests (PFTs) were performed for 63 consecutive patients with newly diagnosed or highly suspected lung cancer. Presurgical pulmonary function tests by way of spirometry examinations were performe

An anatomic study of structure and innervation of the serratus anterior muscle.

BACKGROUND: The structure and function of the serratus anterior muscle are partitioned into three parts. If the morphological characteristics in each part can be demonstrated in more detail, the cause of dysfunction will probably be identifiable more accurately. The purpose of this study was to demonstrate the details of the structure and innervation in each part of the serratus anterior muscle. MATERIALS AND METHODS: This macroscopic anatomic study was conducted using ten sides from five cadavers. The structure and innervation in each part of this muscle were examined. RESULTS: In the super

Rectus Abdominis Motor Nerves as Donor Option for Free Functional Muscle Transfer: A Cadaver Study and Case Series.

BACKGROUND: Current management of brachial plexus injuries includes nerve grafts and nerve transfers. However, in cases of late presentation or pan plexus injuries, free functional muscle transfers are an option to restore function. The purpose of our study was to describe and evaluate the rectus abdominis motor nerves histomorphologically and functionally as a donor nerve option for free functional muscle transfer for the reconstruction of brachial plexus injuries. METHODS: High intercostal, rectus abdominis, thoracodorsal, and medial pectoral nerves were harvested for histomorphometric anal

Incidence of inadvertent intercostal or epidural spread during thoracic sympathetic ganglion block.

BACKGROUND: Sympathetic blocks (SBs) have been used widely to relieve the symptoms of sympathetically maintained pain (SMP). The thoracic sympathetic ganglion is not separated from somatic nerves by muscles and connective tissue. The upper thoracic ganglion runs along the posterior surface of the vertebral column in close proximity to the adjacent epidural region. This anatomical difference leads to frequent epidural and intercostal spread in cases of thoracic SBs. The purpose of this study was to investigate the incidence of inadvertent intercostal and epidural injections during thoracic SBs.

Morgagni hernia associated with ipsilateral lateral chest wall defect: a case report of transdiaphragmatic intercostal hernia in an adult.

INTRODUCTION AND IMPORTANCE: Transdiaphragmatic intercostal hernias (TDIHs) are rare and typically posttraumatic. The coexistence of a TDIH with congenital anomalies, such as a Morgagni hernia (MH) and chest wall defect, is exceptionally uncommon. This report details a unique case of MH associated with an ipsilateral intercostal hernia in an adult, underscoring key diagnostic and surgical challenges. CASE PRESENTATION: A 71-year-old man with multiple comorbidities presented with acute abdominal pain and a right-sided chest wall bulge. Computed tomography revealed a large right lateral diaphra
