A Novel Clinical Test for Setting Intermittent Pneumatic Compression Parameters Based on Edema Fluid Hydromechanics in the Lymphedematous Calf.

BACKGROUND: Long-term observations confirm lasting effects and lack of complications of intermittent pneumatic compression (IPC) therapy. So far, no test has been designed that would provide data necessary for setting pressure and time parameters of the IPC device to obtain optimum decrease in limb volume. AIM: To design a test providing data on decrease of circumference under the inflated chamber in time depending on the applied compression pressure. METHODS: One chamber was placed above the ankle joint and inflated to 120 mmHg in order to occlude tissue fluid backflow during inflation of t

Compression therapy for treating post-thrombotic syndrome.

BACKGROUND: Post-thrombotic syndrome (PTS) is a long-term complication of deep vein thrombosis (DVT) characterised by chronic complaints such as oedema and skin changes including; venous ectasia, varicose veins, redness, eczema, hyperpigmentation, and in severe cases fibrosis of the subcutaneous adipose in the affected limb. These chronic complaints are the effects of venous outflow restriction that can cause symptoms such as heaviness, itching, pain, cramps, and paraesthesia. Twenty to fifty percent of people with DVT develop post-thrombotic complications. Several non-pharmaceutical measures

Home-Based Intermittent Pneumatic Compression Therapy: The Impact in Chronic Leg Lymphedema in Patients Treated for Gynecologic Cancer.

We conducted a prospective study of cancer patients to investigate the efficacy, quality of life, satisfaction, and safety of a home-based intermittent pneumatic compression (IPC) device during the maintenance phase of lower extremity lymphedema. This device has a unique mode designed to mimic the manual lymphatic drainage (MLD) technique and thereby gently facilitate lymphatic draining of proximal extremities. Thirty patients with stage 3 chronic secondary unilateral leg lymphedema in the maintenance phase underwent IPC and conventional compression therapy for 4 weeks at home. The participant

Evaluation of patient-reported outcomes in over 10,000 patients using pneumatic compression therapy for lower-extremity lymphedema.

OBJECTIVE: Lymphedema treatment varies from compression and manual lymphatic drainage to surgical measures. Although compression demonstrates clinical benefit, poor adherence and dissatisfaction can contribute to treatment failure. This study aimed to assess patient-reported satisfaction, device usage, and symptom improvement with pneumatic compression device therapy. METHODS: This online survey-based study was conducted from January 1, 2024, to December 31, 2024, with 35,522 invitations delivered without incentive to respond. Patients with lower-extremity edema who had been prescribed a medi

The use of foot pumps compression devices in the perioperative management of ankle fractures: Systematic review of the current literature.

BACKGROUND: Ankle fractures account for 9% of all fractures seen in the United Kingdom. 15,000 of these fractures undergo operative fixation each year. Soft tissue swelling impacts on timing of fixation due to fears of infection and wound dehiscence. The use of arterio-venous foot pumps (AVFP) is increasing in this population although the evidence for their efficacy is unclear. In order to address this, we present an overview of the evidence for AVFP device use following ankle fracture. METHODS: In September 2015 an electronic literature search was undertaken of studies comparing two or more

Effectiveness of 'in-cast' pneumatic intermittent pedal compression for the pre-operative management of closed ankle fractures: a clinical audit.

BACKGROUND: Timing of surgery for ankle fractures is largely dependent on the condition of the surrounding soft-tissues. This study aimed to determine the clinical effectiveness of a pre-operative in-cast artero-venous (AV) impulse device in the management of closed ankle fractures requiring surgery. METHODS: A consecutive series of 64 closed ankle fractures were managed using the AV impulse system prior to surgery. Patients were compared to 73 consecutive closed ankle fractures managed surgically in the same unit immediately prior to the implementation of the AV impulse device study. Outcome

Influence of intermittent compression cuff design on interface pressure and calf deformation: experimental results.

Intermittent pneumatic compression (IPC) is widely used for deep vein thrombosis (DVT) prophylaxis. The technique involves periodic inflation of a compression cuff around a limb, which acts to simulate the muscle pump mechanism, encouraging venous blood flow. However, there is uncertainty regarding the relationship between compression, vascular effects and clinical outcomes. This study investigates calf compression provided by four IPC cuffs with different air bladder configurations. Interface pressure between the cuff and the skin surface is measured and magnetic resonance (MR) images are obt
