Pathomechanics of glenohumeral capsule and scapula in idiopathic frozen shoulder: a study using a three-dimensional finite element model.

BACKGROUND: The pathomechanics of primary frozen shoulders are not yet fully understood. There is ongoing uncertainty regarding the optimal extent of surgical release, with concerns about both over- and under-release of the joint capsule. In the frozen shoulder, different ranges of motion of the shoulder joint experience varying areas of stress, contributing to stiffness and limited movement. The ultimate goal of treatment is to restore the full range of motion while carefully addressing the specific areas of pathology. The purpose of this study was to analyze the kinematics of the glenohumera

Diffusion tensor magnetic resonance imaging of white matter integrity in patients with HIV-associated neurocognitive disorders.

BACKGROUND: This study investigated the efficacy and neurotoxicity of highly active antiretroviral therapy (HAART) by evaluating white matter (WM) injury using diffusion tensor magnetic resonance imaging (DTI) in patients with human immunodeficiency virus (HIV)-associated neurocognitive disorders (HAND). METHODS: Forty-six patients with HAND underwent DTI before and every six months during HAART treatment. DTI data, including fractional anisotropy (FA) and mean diffusivity (MD) values of structural WM before and after HAART, were compared. The relationship between DTI values and plasma viral

Obsessive compulsive disorder and the right hemisphere: topographic analysis of lesions after anterior capsulotomy performed with thermocoagulation.

Considerable but uncontrolled evidence suggests that stereotactic capsulotomy by means of thermolesions may provide symptomatic relief for patients with otherwise therapy refractory "malignant" obsessive compulsive disorder (OCD). Unlike in other functional stereotactic interventions, target localization for capsulotomy is based upon anatomical definition only. Few systematic attempts have been made to correlate the site and size of the capsular lesions with postoperative clinical outcome. Between 1976 and 1989 bilateral thermo-capsulotomy (TC) was performed in 22 OCD patients. In 19 patients

Long insular artery damage might be a key sign for predicting functional prognosis of putaminal hemorrhage.

OBJECTIVE: Although the putamen is the most common area of spontaneous intracerebral hemorrhage, previous reports about the effects of surgery are limited. We sometimes experience a poor prognosis in patients in whom there is no damage to the internal capsule, but with injury in the long insular artery (LIA) region. The purpose of this study was to confirm the relationship between LIA damage and patient prognosis following surgery for putaminal hemorrhage. METHODS: We retrospectively collected data of 287 surgical cases who presented with putaminal hemorrhage between January 2004 and March 20

Meta-analysis of apparent diffusion coefficients in the newborn brain.

Diffusion-weighted imaging and its quantitative apparent diffusion coefficient can assess severity in newborn hypoxic-ischemic injuries. A meta-analysis established normative values in term newborns, in comparison to those values in hypoxic-ischemic newborns with good versus poor outcomes. Measurements from 14 reports were stratified into three levels of increasing specificity: tissue type (gray matter, white matter, or cerebellum), tissue distribution (e.g., cortex or white-matter tracts), and anatomic structures (e.g., frontal white matter or posterior limb of the internal capsule). Normativ

Dementia and capsular genu ischemia in patients with severe bacterial meningitis.

Infarction in the genu of the internal capsule causes dementia that is characterized by abulia, lethargy and memory loss without obvious motor palsy (capsular genu syndrome). We found infarction or decreased cerebral blood flow in the genu of the internal capsule in 6 of 13 patients with severe bacterial meningitis. Four of these six patients developed post-meningitis dementia, characterized by abulia, lethargy, and memory loss. Of 24 patients with viral meningitis, none developed capsular genu ischemia or post-meningitis dementia. In patients with severe bacterial meningitis, capsular genu is

Reanalysis of multislice (1)H MRSI in amyotrophic lateral sclerosis.

The goal of this work was to reexamine previously published (1) brain spectroscopy data of abnormal metabolite ratios in amyotrophic lateral sclerosis (ALS). Toward this goal, (1)H MR spectroscopic imaging data from 10 ALS and nine control subjects were reanalyzed using improved data analysis techniques, including automated curve fitting and tissue-volume correction. In the motor cortex of ALS, N-acetyl aspartate (NAA) was 23% (P = 0.004) lower than in controls, and in the posterior internal capsule of ALS choline compounds (Cho) were 20% (P = 0.02) higher. This demonstrates that the metabolit

Hip mechanics after posterior structure repair in total hip arthroplasty.

This study investigated the rotational response of the hip with different repairs of posterior structures after using the posterior approach in total hip replacement. Five groups were tested: (1) the normal hip without a replacement, (2) the normal hip with a vented capsule, (3) no repair of the capsule and external rotators after total hip replacement, (4) repair of only the piriformis tendon, and (5) repair of the capsule and external rotators as a flap of tissue (capsule, piriformis, obturator internus, gemellae, and quadratus) to the posterior aspect of the greater trochanter. Electric double-sleeved vacuuming microtrephine for lens refilling.

An automated electric double-sleeved vacuuming microtrephine has been developed that consistently creates a 0.8 to 1.0 mm tear-free hole in the anterior lens capsule. This hole is required for lens refilling to retain accommodation after cataract removal. The trephine is double-sleeved. The stainless-steel outer sleeve vacuums the internal anterior capsule; the stainless steel rotating inner sleeve, which has a blade on the head, then slides forward and cuts the taut anterior capsule. All manipulations are controlled automatically by depressing a foot pedal. Using this instrument, a 0. 8 to 1.

Stereotactic radiosurgery for treatment-refractory obsessive-compulsive disorder: a systematic review of targets, targeting methods, lesion geometry, and clinical outcomes.

BACKGROUND: Stereotactic radiosurgery (SRS) is a noninvasive option for carefully selected patients with treatment-refractory obsessive-compulsive disorder (OCD), but the literature is heterogeneous in targets, targeting methods, lesion geometry, and outcomes. METHODS: PRISMA-guided systematic review of PubMed, Scopus, and Web of Science. Full-text human studies of radiosurgical capsulotomy for treatment-refractory OCD were included. Narrative synthesis was performed across technical and clinical domains. When ≥3 comparable studies reported the same endpoint, exploratory random-effects single

Effect of soft-tissue impingement on range of motion during posterior approach Total Hip Arthroplasty: an in vivo measurement study.

PURPOSE: While implant impingement and bony impingement have been recognized as causes of poor outcomes in total hip arthroplasty (THA), reports of soft-tissue impingement are rare. To clarify the issue, the effect of anterior capsule resection on hip range of motion (ROM) was quantitatively measured in vivo during posterior approach THA using a CT-based hip navigation system. MATERIALS AND METHODS: For 47 patients (51 hips), hip ROM was measured intraoperatively before and after resection of the anterior hip capsule, and the difference was compared. RESULTS: Resection of the anterior hip ca

White matter involvement in chronic musculoskeletal pain.

UNLABELLED: There is emerging evidence that chronic musculoskeletal pain is associated with anatomic and functional abnormalities in gray matter. However, little research has investigated the relationship between chronic musculoskeletal pain and white matter. In this study, we used whole-brain tract-based spatial statistics and region-of-interest analyses of diffusion tensor imaging data to demonstrate that patients with chronic musculoskeletal pain exhibit several abnormal metrics of white matter integrity compared with healthy controls. Chronic musculoskeletal pain was associated with lower

Benefits of stereotactic radiosurgical anterior capsulotomy for obsessive-compulsive disorder: a meta-analysis.

OBJECTIVE: Anterior capsulotomy (AC) is a therapeutic option for patients with severe, treatment-resistant obsessive-compulsive disorder (OCD). The procedure can be performed via multiple techniques, with stereotactic radiosurgery (SRS) gaining popularity because of its minimally invasive nature. The risk-benefit profile of AC performed specifically with SRS has not been well characterized. Therefore, the primary objective of this study was to characterize outcomes following stereotactic radiosurgical AC in OCD patients. METHODS: Studies assessing mean Yale-Brown Obsessive Compulsive Scale (Y

Deep brain stimulation and ablation for obsessive compulsive disorder: evolution of contemporary indications, targets and techniques.

Surgical therapy for treatment-resistant obsessive compulsive disorder (OCD) remains an effective option for well-selected patients managed within a multidisciplinary setting. Historically, lesions within the limbic system have been used to control both obsessive thoughts and repetitive compulsions associated with this disease. We discuss classical targets as well as contemporary neuromodulatory approaches that have been shown to provide symptomatic relief. Recently, deep brain stimulation (DBS) of the anterior limb of the internal capsule/ventral striatum received Conformité Européene (CE) ma

Theoretical prediction of the hemodynamic performance of slow resorbing polyester protein impregnated arterial prostheses after implantation: a plea for fast resorption of the coating.

The clinical literature cites cases where slow, incomplete, or nonuniform protein resorption from protein impregnated arterial prostheses produces undesirable localized internal capsule proliferation leading to a significant reduction of the internal diameter of the prosthesis. In an attempt to describe the hemodynamic response to this phenomenon, the blood flow in such stenotic regions was simulated and characterized numerically using FIDAP computational fluid dynamics software to determine the Navier-Stokes and continuity equations for simple channel flow. To simulate various stages of inter

Cerebral white matter and cognition in hydrocephalic children.

Although children with hydrocephalus frequently show poor development of nonverbal cognitive skills relative to verbal skills, little is known about the neuropathologic correlates of these discrepancies. In this study, cerebral white-matter structures and lateral ventricles were measured from the magnetic resonance images of age-matched children with meningomyelocele, meningocele, and aqueductal stenosis and normal subjects. The volume of each lateral ventricle and the cross-sectional area of the corpus callosum and internal capsules were correlated with concurrent measures of verbal and nonve

Lesion topography and outcome after thermocapsulotomy or gamma knife capsulotomy for obsessive-compulsive disorder: relevance of the right hemisphere.

OBJECTIVES: Obsessive-compulsive disorder is a common mental disorder, notorious for its chronicity and intractability. Stereotactic lesions within the anterior limb of the internal capsule have been shown to provide symptomatic relief in such refractory cases, but only few systematic evaluations have correlated anatomic lesion location with individual postoperative outcome. PATIENTS AND METHODS: Between 1976 and 1989, extremely disabled and otherwise intractable patients with a chronic deteriorating clinical course of obsessive-compulsive disorder underwent bilateral thermocapsulotomy (n = 2

Brain lesions affecting gait recovery in stroke patients.

OBJECTIVES: Gait recovery is an important goal in stroke patients. Several studies have sought to uncover relationships between specific brain lesions and the recovery of gait, but the effects of specific brain lesions on gait remain unclear. Thus, we investigated the effects of stroke lesions on gait recovery in stroke patients. MATERIALS AND METHODS: In total, 30 subjects with stroke were assessed in a retrograde longitudinal observational study. To assess gait function, the functional ambulation category (FAC) was tested four times: initially (within 2 weeks) and 1, 3, and 6 months after t

Prenatal and adolescent exposure to tobacco smoke modulates the development of white matter microstructure.

Prenatal exposure to maternal smoking has been linked to cognitive and auditory processing deficits in offspring. Preclinical studies have demonstrated that exposure to nicotine disrupts neurodevelopment during gestation and adolescence, possibly by disrupting the trophic effects of acetylcholine. Given recent clinical and preclinical work suggesting that neurocircuits that support auditory processing may be particularly vulnerable to developmental disruption by nicotine, we examined white matter microstructure in 67 adolescent smokers and nonsmokers with and without prenatal exposure to mater
