PURPOSE: Anatomical variants in the posterior compartment of forearm and hand are not uncommon. Physicians should be aware of variations in this region for correct diagnosis and treatment of diseased hands.
METHODS: During a routine dissection at our department, an extremely rare case of deep hand extensor muscle was discovered.
RESULTS: A complete extensor digitorum profundus complex was found in the fourth extensor compartment in addition to the extensor indicis proprius. The complex consisted of two muscle bellies originating from the lateral aspect of distal ulna and the adjacent interos

Osteogenesis imperfecta (OI) is a heterogenous group of genetic disorders of bone fragility. OI type V is an autosomal-dominant disease characterized by calcification of the forearm interosseous membrane, radial head dislocation, a subphyseal metaphyseal radiodense line, and hyperplastic callus formation; the causative mutation involved in this disease has not been discovered yet. Using linkage analysis in a four-generation family and whole-exome sequencing, we identified a heterozygous mutation of c.-14C>T in the 5'-untranslated region of a gene encoding interferon-induced transmembrane prote

Osteogenesis imperfecta (OI) is a large group of genetically heterogeneous diseases resulting from decreased bone density and an abnormal microarchitecture, which are clinically manifested by abnormal bone fractures. A distinctive clinical feature of this group of diseases is the presence of spontaneous fractures and skeletal deformities. However, the clinical manifestations of different types of OI are characterized by marked polymorphism with variable severity of skeletal and extra-skeletal features. Previous studies have shown that a mutation (c.-14C>T) in the IFITM5 gene is responsible for

Background Treatment algorithm for disruption of the triangular fibrocartilage complex (TFCC) from the ulnar fovea includes direct TFCC repair, tendon reconstruction of the radioulnar ligaments, or a salvage procedure in cases with painful distal radioulnar joint (DRUJ) degeneration. Case Description We describe our surgical technique for reconstruction of the distal oblique bundle (DOB), to attain DRUJ stability in a young man, after failed attempts of direct TFCC reinsertion and radioulnar ligament reconstruction with the Adams procedure. Literature Review Reconstruction of the central ba

INTRODUCTION: Skeletal fluorosis is a rare toxic osteopathy characterized by massive bone fluoride fixation. The disease is an endemic problem in some parts of the world and is the result of the prolonged ingestion or inhalation of large amounts of fluoride. The radiographic presentation is mainly characterized by bone changes, with osteocondensation and ossification of many ligaments and interosseous membranes.
DIAGNOSES: A 59-year-old woman with skeletal fluorosis underwent staged bilateral total knee arthroplasty. Preoperative radiographs showed increased bone density, thickened and fused

In the forearm, posttraumatic heterotopic ossification usually forms as a proximal radioulnar synostosis. It can occur after soft tissue injury involving the interosseous membrane or after surgery involving the radio and ulna, such as distal biceps tendon repair. It can also be induced by radial head dislocation or fracture. Screening radiography can be used to select the appropriate time for excision. The synostosis can be resected when the ectopic bone margin and trabeculation appear mature on radiographs. An interval of 6-12 months from the injury is generally recommended based on ectopic b

INTRODUCTION: A new mechanism of injury of the forearm bones, crisscross injury, is described. It is more common than the Essex-Lopresti fracture dislocation. The old concept of isolated injury of one side of the radioulnar joint may be challenged. It often occurs in Mason type II fracture dislocation of the radial head or dislocation of radioulnar joints.
MATERIALS AND METHODS: The first part was a cadaveric study of the crisscross injury of forearms. The second part was a clinical study of the crisscross injury in some cases of Mason type II fracture radial head and double dislocation of th

The fibular free flap is seen as one of the foremost technical options in mandibular reconstruction, especially in those defects where long bone is required. Cases with squamous-cell carcinoma of the floor of the mouth with mandibular spread and subsequent segmentary mandibular removal are the cornerstone examples. A case of squamous-cell carcinoma of the whole floor of the mouth with mandibular invasion is reported. Radical resection of the floor of the mouth and bilateral mandibular horizontal ramus was performed, with a bony defect extending from angle to angle. The patient revealed an acho

Five fresh cadaver upper extremities were studied with use of a static positioning frame, pressure-sensitive film and a microcomputer-based videodigitizing system to assess the effect of increasing radioulnar instability on the load distribution within the proximal carpal joint. Three stages of radioulnar instability were studied: (1) an avulsion fracture at the base of the ulna styloid; (2) an avulsion fracture at the base of the ulna styloid plus disruption of the dorsal portion of the distal radioulnar joint capsule; and (3) an avulsion fracture at the base of the ulna styloid, disruption o

Forearm supination contractures can occur as a result of neurological derangement of the upper extremity. Primarily, this is observed in patients with neonatal brachial plexus birth palsy. The contractures develop slowly over time and become problematic in childhood as the patients begin requiring forearm pronation for activities of daily living including typing on a keyboard and writing. Although supination contracture is a well-described sequelae of neonatal brachial plexus birth palsy, there is a paucity of literature describing techniques and outcomes for reconstruction and restoration of

Radius and ulna fractures from airbag deployment onto the forearm have been reported in the literature. Based on laboratory experiments with eight cadaveric upper extremities, this paper presents a method for using strain gages to evaluate upper extremity loading during airbag deployment. The technique provides strain rates, bending moments, and time of fracture for the radius and ulna. Planar rosettes (350 omega, 5% strain) were selected as the best choice given the application to bone with a rosette being placed mid-shaft on both the anterior and posterior surfaces of the radius and ulna. Extensive forearm bone loss, whatever its etiology, presents a difficult reconstruction problem. This is mainly the case in the presence of lesions of the interosseous membrane associated with the radio-ulnar joint. When preservation of forearm rotation is not possible, cubitalization of the radius and reconstruction of the forearm by creation of a "one bone forearm" seems to be an excellent salvage technique both functionally and cosmetically. Our experience concerns six clinical cases; two of these cases are original and give the authors the opportunity to describe a new reconstructive techn

The scapholunate interosseous ligament (SLIL) plays a fundamental role in stabilizing the wrist bones, and its disruption is a frequent cause of wrist arthrosis and disfunction. Traditionally, this structure is considered to be a variety of fibrocartilaginous tissue and consists of three regions: dorsal, membranous and palmar. Despite its functional relevance, the exact composition of the human SLIL is not well understood. In the present work, we have analyzed the human SLIL and control tissues from the human hand using an array of histological, histochemical and immunohistochemical methods to

BACKGROUND: Report on surgical management of ulnar impaction syndrome secondary to chronic Monteggia fractures are limited, and optimal techniques and outcomes remain unclear. Herein, we report a case in which preoperative stress evaluation followed by diaphyseal-level ulnar-shortening osteotomy led to favorable clinical outcomes.
CASE DESCRIPTION: A 54-year-old woman with a childhood forearm injury presented with ulnar-sided wrist pain. Imaging revealed ulnar impaction syndrome with foveal triangular fibrocartilage complex (TFCC) injury. An asymptomatic anterior radial head dislocation, cons

Osteogenesis imperfecta (OI) is typically caused by mutations in type 1 collagen genes, but in recent years new recessive and dominant forms caused by mutations in a plethora of different genes have been characterized. OI type V is a dominant form caused by the recurrent (c.-14C > T) mutation in the 5'UTR of the IFITM5 gene. The mutation adds five residues to the N-terminus of the IFITM5, but the pathophysiology of the disease remains to be elucidated. Typical clinical features present in the majority of OI type V patients include interosseous membrane calcification between the radius and ulna

INTRODUCTION: Acute avulsion of the distal biceps brachii tendon requires urgent surgical reattachment. The usual open surgical techniques carry significant risks of local complications, mostly neurovascular injuries but also interosseous membrane synostosis.
MATERIAL AND METHODS: We have developed a mini-invasive method with endoscopic control, that is simple and reproducible, allowing anatomical repair of acute lesions using a suture anchor. This new technique was used in 23 consecutive patients (among them, 11 professional sportsmen), with a median age of 46 years, and a median postoperati

Forty-seven of 661 head-injured adults sustained 50 forearm fractures. Eight extremities exhibited elbow flexion contractures greater than 55 degrees. Traumatic heterotopic ossification at the elbow developed in ten (20%) extremities: four with Monteggia fractures, two with olecranon fractures, and four with no known trauma. Only two of 18 (11%) extremities treated by plaster immobilization achieved good or excellent results, while 17 of 32 (53%) extremities treated by open reduction and internal fixation achieved good or excellent results. Union of fractures of one or both bones occurred at t
