Cervical intervertebral disc prolapse associated with traumatic facet dislocations.

Closed reduction of dislocated facet joints is considered safe by most authors. We report the occurrence of traumatic cervical disc prolapse associated with these injuries in four patients. Without adequate radiological exclusion of this pathology, reduction may have catastrophic consequences as illustrated by two of our cases. Anterior discectomy and open reduction and fixation provide safe and successful avenues of treatment.

Reasons for failure of L5-S1 intervertebral disc excisions.

This study reviews 247 patients who still had preoperative complaints six months to two years after disc excision. In over half, spondylosis of the L5-S1 facets was present, concurrently associated with a pelvic obliquity. A number of other organic causative factors were identified in 18% of the patients. Nonorganic reasons accounted for failure in only 13%. Over 70% of patients with associated spondylosis were improved by treatment directed at this condition.

Intervertebral disc degeneration in adult mice with hereditary kyphoscoliosis.

Breeding experiments confirmed that a hereditary form of kyphoscoliosis in the BDL strain mouse was due to an autosomal recessive gene (ky). Sagittal sections of whole vertebral columns from adult homozygous recessive mice (ky/ky) were examined histologically. All mice showed varying degrees of degenerative change in one or more intervertebral discs between the fifth cervical and the second thoracic vertebrae. The changes comprised loss of cells, loss of distinction between nucleus pulposus and annulus fibrosus, loss of characteristic ring-like structure in the annulus, and development of wedg

An epidemiological study of acute prolapsed cervical intervertebral disc.

In this epidemiological study of acute prolapsed cervical intervertebral disc, we found that people in the fourth decade of life were affected somewhat more frequently than individuals in other age groups, and men with a prolapsed cervical disc outnumbered women by a ratio of 1.4 to one. Factors that were associated relatively strongly with this diagnosis were frequent lifting of heavy objects on the job that was held around the time of the onset of symptoms, cigarette-smoking, and frequent diving from a board. Positive associations that were of borderline statistical significance or were not

Prevalence, MRI findings, and clinical features of lumbosacral intervertebral disc protrusion in French Bulldogs diagnosed with acute thoracic or lumbar intervertebral disc extrusion.

INTRODUCTION: Intervertebral disc protrusion (IVDP) is a neurological disorder commonly observed at the lumbosacral junction of old, medium-to-large breeds, non-chondrodystrophic dogs. Although uncommon, lumbosacral IVDP can also be seen in chondrodystrophic dogs, among them French Bulldogs (FBs) and could be associated with congenital vertebral malformations in this breed. This study aims to evaluate the prevalence, clinical features, and MRI characteristics of lumbosacral IVDP and congenital vertebral malformations in FBs diagnosed with thoracic or lumbar intervertebral disc extrusion (IVDE)

Smaller Intervertebral Disc Volume and More Disc Degeneration after Spinal Distraction in Scoliotic Children.

In recent decades, magnetically controlled growing rods (MCGR) were established to treat progressive early-onset scoliosis. The aim of this investigation was to assess the effect of long-term MCGR with continuous distraction on intervertebral discs in scoliotic children. Magnetic resonance imaging (MRI) of 33 children with spinal muscular atrophy was analyzed by grading intervertebral disc degeneration (IDD) and measuring intervertebral disc volume. Cohort I (n = 17) were children who had continuous spinal distraction with MCGRs for 5.1 years and MRI before (av. age 8.1) and after (av. age 13.

Prevalence of radiographic detectable intervertebral disc calcifications in Dachshunds surgically treated for disc extrusion.

BACKGROUND: An association between the occurrence of calcified discs, visible on radiographic examination (CDVR), and disc extrusions has been suggested in published literature over the past 10-20 years, mainly from Nordic countries. It has also been postulated that dogs without CDVR would not develop disc extrusions. Furthermore, inheritance of CDVR has been calculated and it has been postulated that, by selecting dogs for breeding with few, or no CDVR, the prevalence of disc extrusions in the Dachshund population may be reduced. METHODS: The prevalence of radiographic detectable interverteb

Intervertebral disc progenitor cells: roles in regeneration and disease.

Intervertebral disc (IVD) degenerative disease is a prevalent and debilitating spinal disease. Current treatments only focus on symptomatic relief but fail to halt disease progression or restore the native biomechanical function of the spine. Regenerative medicine strategies, particularly those harnessing endogenous progenitor cells, offer a promising avenue for achieving biological repair and functional homeostasis. The identification of intervertebral disc progenitor cells (IVD-PCs) has unveiled a potential cellular reservoir for self-repair, given their demonstrated stemness attributes, inc

Pyroptosis: mechanism and therapeutic strategies with intervertebral disc degeneration.

Intervertebral disc degeneration (IDD) is a major cause of low back pain, characterized by a complex interplay of inflammation, extracellular matrix degradation and various modes of cell death. Among these, pyroptosis, a highly inflammatory form of programmed cell death, has recently emerged as a critical pathogenetic mechanism. This review systematically elaborates on the role of pyroptosis in IDD, detailing its activation via the canonical and noncanonical inflammasome pathways in response to oxidative stress, mechanical load and metabolic disturbances. Here we highlight how pyroptosis syner

Cell and Hydrogel-Integrated Therapies for Intervertebral Disc Regeneration.

Intervertebral disc degeneration (IDD) is a major cause of low back pain (LBP), significantly affecting on global disability and healthcare costs. Traditional treatments primarily focus on symptom management rather than addressing the underlying causes, such as the decline in nucleus pulposus (NP) cells and reduced extracellular matrix (ECM) synthesis. Cell therapy shows promise by replenishing NP cells, activating resident cells, and enhancing ECM deposition. However, the harsh microenvironment of IDD can hinder the survival of transplanted cells. Biocompatible hydrogels, with their ECM-like

Annulus fibrosus micro-damage in Mild intervertebral disc degeneration.

BACKGROUND CONTEXT: Degeneration of the intervertebral discs (IVDs) causes significant biomechanical changes and structural defects, often leading to pain or disability. These defects impair both the biomechanics and physiology of the IVD, contributing to further structural failure of the IVD. Although the function and structure of the annulus fibrosus (AF) affect overall disc function, its structural change during degeneration is not fully characterized. PURPOSE: The objective of this study was to investigate AF microdamage and mechanical function in mildly degenerated discs. STUDY DESIGN:

Diurnal Asymmetric Loading Modulates Cell Phenotype in Intervertebral Disc.

PURPOSE: This study tested the hypothesis that asymmetric dynamic loading alone or in combination with static loading influences the morphological and biological characteristics of the intervertebral disc (IVD) cells in an ex vivo model. METHODS: Bovine caudal IVDs were assigned to four groups: (1) Parallel dynamic load (1 h) + free swelling (23 h); (2) Parallel dynamic load (1 h) + static load (23 h); (3) Wedge dynamic load (1 h) + free swelling (23 h); (4) Wedge dynamic load (1 h) + static load (23 h). IVD structure was assessed with measurements of height loss and histological staining. IV

Cervical Intradural/Extramedullary Intervertebral Disc Extrusion in a Dog.

A 6-year-old male neutered Jack Russell Terrier presented via the emergency department for subacute progressive asymmetrical tetraparesis and cervical hyperesthesia. MRI showed T2-weighted (T2W) and T1-weighted (T1W) hypointense material, positioned dorsocaudally to the C3-4 intervertebral disc on the right ventrolateral aspect of the spinal cord, causing moderate-to-severe compression of the spinal cord. The material was sharply marginated with a pointed/sharp beak-like appearance (beak sign). On parasagittal images, two hyperintense lines appearing as a "Y" were appreciated (Y-sign). Surgica

Towards biomimetic evolution of artificial intervertebral disc: a review.

In lumbar total disc replacement, artificial disc implants are utilized to cure degenerative disc disease and restore natural motion. Human intervertebral discs (IVD) are part of the spine and contribute to delivering six degrees of freedom, elastic deformation, and shock absorption and act differently under different load conditions. Despite advancements in spinal fixation systems and IVD replacement techniques, achieving long-term segmental stability while preserving physiological motion remains a significant challenge. To overcome this issue, the proposed work aims to identify the biomechan

Natural products can modulate inflammation in intervertebral disc degeneration.

Intervertebral discs (IVDs) play a crucial role in maintaining normal vertebral anatomy as well as mobile function. Intervertebral disc degeneration (IDD) is a common clinical symptom and is an important cause of low back pain (LBP). IDD is initially considered to be associated with aging and abnormal mechanical loads. However, over recent years, researchers have discovered that IDD is caused by a variety of mechanisms, including persistent inflammation, functional cell loss, accelerated extracellular matrix decomposition, the imbalance of functional components, and genetic metabolic disorders

Mitochondrial Dysfunction in Oxidative Stress-Mediated Intervertebral Disc Degeneration.

Intervertebral disc degeneration (IVDD) is the most common contributor to low back pain (LBP). Recent studies have found that oxidative stress and reactive oxygen species (ROS) play an important role in IVDD. As a by-product of aerobic respiration, ROS is mainly produced in the mitochondria by the electron transport chain and other mitochondrial located proteins. With the excessive accumulation of ROS, mitochondria are also the primary target of ROS attack in disc cells. A disrupted balance between intracellular ROS production and antioxidant capacity will lead to oxidative stress, which is th

Pediatric idiopathic intervertebral disc calcification (IIDC): a challenging case.

Idiopathic intervertebral disc calcification is a rare condition in children with a very good prognosis. As there are no biological markers, imaging is invaluable for diagnosing this "do not touch" lesion. While the characteristic feature is nucleus pulposus calcification at one or more cervical or thoracic levels, it is important that practitioners be able to recognize atypical patterns so that biopsy can be avoided. Here we report a case of pediatric idiopathic intervertebral disc calcification with contiguous vertebral involvement and anterior longitudinal ligament ossification.
