Opsoclonus myoclonus with ataxia (OMA) is a rare neurological disorder. The syndrome is characterized by involuntary, conjugate, multidirectional eye movements accompanied by involuntary movements of limb or face, and sometimes ataxia, dysarthria, irritability, dementia, altered level of consciousness and even death. OMA is associated with various etiologies including infectious, toxic, drug-related, vascular and paraneoplastic conditions. Paraneoplastic opsoclonus myoclonus with ataxia (POMA) is more common in patients over 40 years of age and is usually associated with lung (especially small

Tardive dyskinesia (TD) is a hyperkinetic movement disorder that may result from treatment with antipsychotics or other dopamine receptor blocking agents. Underlying pathophysiology is incompletely understood but since the 1970s dopamine depleting agents have been used to reduce involuntary movements. The search for safe, effective treatments for TD is ongoing. Valbenazine, a novel VMAT2 inhibitor, has recently been FDA approved for treatment of TD. Areas covered: An overview of TD, unmet medical needs and current treatment guidelines are presented. The background, chemistry and clinical devel

Phenotypic overlap between autoimmune encephalitis and movement disorders may rarely occur. We present a patient with progressive gait disturbance, vertical supranuclear gaze palsy, cognitive decline, and daytime somnolence, as well as extremity chorea. He was initially suspected of having autoimmune encephalitis. Antibody testing was negative in serum and cerebrospinal fluid. A fall leading to subdural hemorrhage and bilateral post-traumatic frontal lobe changes likely led to his bilateral upper and lower extremity involuntary movements. His mental status improved after lumbar puncture. His c

Bilateral limb-shaking transient ischemic attack (LS-TIA) is a rare disease involving carotid artery stenosis, characterized by ballism-like involuntary movements of the arms and legs. We describe the case report of a male patient in his 80s presented with continuous bilateral ballism in the arms and legs and tongue dyskinesia. Magnetic resonance imaging showed no ischemic lesions, while cerebral angiography revealed right internal carotid artery (ICA) occlusion and 80% stenosis of the left ICA. 99mTc-ethyl cysteinate dimer single-photon emission computed tomography demonstrated hypoperfusion

The opsoclonus-myoclonus syndrome (OMS) was diagnosed in nine patients, two men and seven women, varying in age from 34 to 65 years. In two cases the findings indicated a post-infectious form, in three cases there was a malignancy, in one case there was a connection with the use of medication and in three cases the cause was unknown. OMS is a rare neurological disorder with symptoms of rapid, involuntary saccadic eye movements (opsoclonus) and sudden involuntary muscle contractions (myoclonus). It has been associated with infections, malignancies, intoxications and metabolic disorders. In 50%

We evaluated the outcome of open antero-inferior capsular shift in 17 patients with multidirectional instability of the shoulder who failed to respond to conservative treatment. Six shoulders presented with secondary impingement syndrome and 11 with involuntary instability. The mean duration of follow-up was 39 months (range 7-89 months). Based on the Rowe and Constant scores and the 12-item questionnaire of Dawson, the results were rated excellent in 14 patients, fair in 2 and poor in 1. All but 2 patients were satisfied with the results, although 6 patients experienced some residual pain, an

Oromandibular dystonia (OMD) is a focal neurological movement disorder characterized by involuntary sustained and often painful muscle contraction, usually producing repetitive movements or abnormal positions of the mouth, jaw and/or tongue. We report on a 30-year-old woman affected with OMD with a 12-year follow-up. Focal dystonia involved an involuntary activity of the lateral pterygoid muscles causing forceful jaw displacement in the maximal protrusive position. These episodes initially occurred during jaw function and increased up to an open-lock with bilateral pre-auricular pain. Dystonic

INTRODUCTION: Conservative management is the first option for patients with stress urinary incontinence (SUI). However, successful management of women diagnosed with SUI is dependent on a proper assessment and a tailored treatment plan. This case report aims to show the effectiveness of physiotherapy management in a 42-year-old patient diagnosed with SUI.
PATIENT PRESENTATION: The patient's main complaints were involuntary loss of urine on coughing, sneezing and lifting of heavy objects, which started following the birth of her third child.
MANAGEMENT AND OUTCOME: The patient was taught the

Tourette's syndrome is a disorder of central nervous system functioning in which the most easily recognized symptoms of involuntary movements and vocalizations have been found to be responsive, in many cases, to pharmacotherapy with haloperidol. We have recently evaluated five patients with Tourette's syndrome and have been impressed with the learning difficulties, speech and language problems, and behavioral problems which accompanied the involuntary neuromotor manifestations. These lesser known manifestations were not improved with haloperidol, and in fact, the sleepiness associated with the

Nonketotic hyperglycemia hemichorea-hemiballismus syndrome (NHH) is an uncommon neurological condition linked to poorly managed diabetes mellitus (DM). It presents with spontaneous, erratic movements that impact just one side of the body. Our case of NHH was of a 76-year-old female with uncontrolled type 2 DM, ischemic heart disease, and dilated cardiomyopathy. Despite previous treatment for similar symptoms, the patient developed left-sided choreo-ballistic movements. Despite difficulties obtaining clear magnetic resonance imaging (MRI) due to involuntary movements, the image revealed T1 hype

Buprenorphine induction can lead to precipitated opioid withdrawal, even when using novel techniques such as transdermal buprenorphine. Involuntary limb movements are a distressing symptom of precipitated withdrawal that can be difficult to treat. We report a case of a military veteran transitioning from methadone to buprenorphine for the treatment of opioid use disorder (OUD) using small doses of transdermal buprenorphine. Herein, we review the literature associated with opioid withdrawal-related restlessness. Despite the known risk of concurrent benzodiazepine and buprenorphine administratio

Ataxia-telangiectasia is a cerebellar neurodegenerative disorder presenting with ataxia, chorea, myoclonus, and bradykinesia. Literature on treatment of movement disorders is scarce. We treated 17 children (aged 11.2 ± 3.9 years) for 8 weeks with the dopaminergic and anti-N-methyl-d-aspartate (NMDA) agent amantadine sulfate 6.3 ± 0.87 mg/kg/d. Ataxia was assessed by using the International Cooperative Ataxia Scale, parkinsonism by the Unified Parkinson Disease Rating Scale, and chorea/myoclonus by the Abnormal Involuntary Movement Scale. Responders were considered those patients who had at lea

INTRODUCTION: Pallidal DBS is an established treatment for severe isolated dystonia. However, its use in disabling and treatment-refractory tardive syndromes (TS) including tardive dyskinesia and tardive dystonia (TD) is less well investigated and long-term data remain sparse. This observational study evaluates long-term effects of deep brain stimulation (DBS) of the globus pallidus internus (GPi) in patients with medically refractory TS.
METHODS: We retrospectively analyzed a cohort of seven TD patients with bilateral GPi-DBS. Involuntary movements, dystonia and disability were rated at long

This article describes an 8-month-old boy with the full clinical spectrum anti-N-methyl-d-aspartate receptor encephalitis. He was admitted to the hospital with involuntary orofacial head movements, behavioral changes, and fluctuation in consciousness. His examination showed tongue thrusting, decreased responsiveness, and hypotonia without fever. Analysis of the cerebrospinal fluid revealed increased protein levels (62 mg/dL). The next day he developed oral dyskinesia and choreoathetosis. Video-electroencephalogram polygraphy showed coreo-dystonic movements without electrographic correlation. A

We studied 101 inmates of a county jail for women who were randomly selected from those referred for psychiatric evaluation. Eighty-six percent had a history of psychiatric hospitalization; 94% had prior arrest records, 50% of these for felonies. More than half met the criteria for involuntary hospitalization. When arrested more than 40% were transients, and only 7% were employed. Seventy percent had histories of serious physical violence, more than 40% had engaged in prostitution, and 84% of those with children had demonstrated an inability to take care of them. Far fewer mentally ill women t

BACKGROUND: In involuntary psychiatric admission, used globally, professionals or caretakers decide upon hospitalization regardless of what the person with psychosocial disabilities decides. This raises clinical, ethical, legal, and human rights concerns, and it goes against Convention on the Rights of Persons with Disabilities (CRPD). CRPD mandates that member states respect the autonomy of people with disabilities. Through Article 12, it recognizes full enjoyment of legal capacity for persons with disabilities. Implementation of Article 12 is challenging in every country, and exploring all t

Jaw-opening oromandibular dystonia (O-OMD) is a clinical subtype of OMD, commonly resistant to treatment. Here, we report a distinct case of tardive O-OMD with a characteristic sensory trick, successfully treated with high-dose botulinum toxin (BTX) injection. A 34-year-old male patient presented with involuntary jaw opening, tongue protrusion, dysarthria, and mild cervical dystonia. The patient reported improved abilities to talk and close his mouth after putting something, like a cigarette, between his teeth. After an unsuccessful treatment with anticholinergic medications, the patient recei
