Pulsed Doppler flow characteristics of ductus arteriosus in infants with associated congenital anomalies of the heart or great arteries.

Pulsed Doppler echocardiography (PDE) from the suprasternal approach was used to assess flow characteristics of ductus arteriosus (DA) in 145 infants (aged 1 day to 6 months) with major congenital heart disease. Direct ductal Doppler interrogation was possible in 138 patients and serial studies before and after medical treatment were performed in 28 infants. According to pulmonary artery pressure and associated heart lesions, 3 ductal shunting patterns were identified. An isolated left-to-right shunt, observed in isolated DA or in right ventricular outflow tract obstruction, was characterized

Blood pressure in young adults.

INTRODUCTION: Hypertension (HT) is a major cardiovascular risk factor. Elevated blood pressure (BP) in childhood predisposes to HT in adulthood. Epidemiological studies in young age-groups are scarce. The aim of this study was to determine the prevalence of HT in a population of university students aged between 18 and 25 years; to analyze the profile of HT in the presence of other risk factors; and to alert patients and physicians to the need to assess and control cardiovascular risk factors in young adults. METHODS: We performed an observational, descriptive and cross-sectional study of a co

Treatment of the elderly hypertensive patient.

Changes due to aging of the cardiovascular system play an important role in the development of hypertension and its complications in the eld … e, captopril, and minoxidil may have value, but such potent agents should generally be avoided with isolated systolic hypertension. In elderly hypertensive patients with other complicating diseases, sufficient alternative treatment options are now available to allow tailoring of therapy to the special needs of each patient, thereby minimizing adverse reactions to therapy. New controversies in hypertension: questions answered, answers questioned.

Despite multiple, interdisciplinary group recommendations, we are still on uncertain ground when it comes to treatment of most aspects of hypertension. Seven major areas of controversy include mild hypertension, the relevance of hypertension and lipids, hypertensive agents and electrolyte imbalance, treatment and regression of left ventricular hypertrophy, isolated systolic hypertension, ambulatory blood pressure monitoring and overtreatment of hypertension--the "j shaped curve." Although our knowledge of these aspects has advanced tremendously, significant doubts exist as to our present appro…

Rising cases of hypertension among indigenous populations of Northeast India: an overview.

A sizable number of people with hypertension live in low- and middle-income countries that lack adequate health resources and where awareness of hypertension is limited, resulting in poor blood pressure control. This review paper aims to investigate the association between ethnicity and the prevalence of cardiovascular disease (CVD) risk factors, such as hypertension, among the indigenous populations of northeast India. Thirty-one articles were retrieved from PubMed, NCBI, and Google Scholar. These articles focus on hypertension cases across twenty-four different indigenous populations in all

Fixed low-dose combination in first-line treatment of hypertension.

Hypertension is poorly controlled in most patients. The control rate, defined as a systolic blood pressure (SBP) < 140 mmHg and diastolic blood pressure (DBP) < 90 mmHg, is 27% in the USA, despite data documenting the reduction of cardiovascular events by treating diastolic hypertension and isolated systolic hypertension. Control rate is even lower for patients with renal insufficiency and type 2 diabetes mellitus. Thus, monotherapy is unlikely to achieve blood pressure control. Supported by the 6th Joint National Committee Report (JNC VI), interest has been sparked in the use of fixed low-dos…

Longitudinal left ventricular function in normotensive prediabetics: a tissue Doppler and strain/strain rate echocardiography study.

BACKGROUND: Although diabetes mellitus is well known to result in systolic and diastolic left ventricular (LV) dysfunction at the subclinical level, even when it is not accompanied by hypertension and coronary artery disease, this situation has not been sufficiently investigated in prediabetes, which is the precursor of diabetes. The aims of the present study were to investigate LV systolic and diastolic function in normotensive and low-risk prediabetic and diabetic subjects for coronary disease using sensitive tissue Doppler echocardiographic parameters, to investigate early possible negative

Comparison Of Cardiovascular Characteristics In Normotensive And Hypertensive Rat Strains.

BACKGROUND: Hypertensive rats serve as valuable tools for studies of dysregulations in cardiovascular functions before and during pathological elevation of blood pressure. They exhibit many defects in structure and function of heart and vessels which are often related to severity of hypertension. OBJECTIVE: The relationship of blood pressure level and manifestation of aberrations in selected cardiovascular and metabolic parameters were determined in 20-week-old normotensive Wistar-Kyoto (WKY) rats, spontaneously hypertensive rats (SHR) and in their F1 offspring borderline hypertensive rats (B

Recommendations for the treatment of hypertension in elderly people.

High blood pressure is a major cardiovascular risk factor. The prevalence of hypertension increases with aging. As a consequence of changes in arterial wall that leads to arterial stiffness, the majority of elderly patients suffer isolated systolic hypertension. The evidence strongly supports that hypertension in the elderly is associated with an increase in stroke risk and cardiovascular mortality and morbidity. Several trials have shown the benefits of treating hypertension in elderly patients. Even in the very old patients, the use of antihypertensive agents such as calcium channel blockers…

[Antihypertensive drugs in diabetic's hypertension].

Hypertension is defined by the Joint National Committee (JNC 7) as blood pressure (BP) > or =Y130/80 mmHg. It affects up to 70% of patients with type 2 diabetes and is twice as prevalent in diabetics as in non-diabetics. Due to its contribution to renal and cardiovascular disease, hypertension increases diabetic mortality four to five-fold. Therefore, a BP goal of 130/80 mmHg or less (125/75 mmHg in patients with proteinuria >1 g/die and increased creatinemia), is recommended. For isolated systolic hypertension, defined as a systolic BP > or =Y180 mmHg with normal diastolic BP, the treatment g…

Chronic Right Heart Failure: Pathogenesis, Haemodynamic Foundations, and a Pragmatic Diagnostic Algorithm.

Chronic right heart failure (RHF) is a complex, progressive syndrome that remains underrecognized and inadequately defined in current clinical guidelines, where it is often relegated to a secondary complication of left-sided heart disease. Because the thin-walled right ventricle (RV) is well adapted to maintain pressures within the highly distensible venous system well below plasma oncotic pressure but poorly equipped to sustain pressure overload, when myocardial failure supervenes, conventional RV systolic indices frequently fail to capture the very essence of the syndrome. This review clarif

Chronic Hypoxia Decreases Endothelial Connexin 40, Attenuates Endothelium-Dependent Hyperpolarization-Mediated Relaxation in Small Distal Pulmonary Arteries, and Leads to Pulmonary Hypertension.

Background Abnormal endothelial function in the lungs is implicated in the development of pulmonary hypertension; however, there is little information about the difference of endothelial function between small distal pulmonary artery (PA) and large proximal PA and their contribution to the development of pulmonary hypertension. Herein, we investigate endothelium-dependent relaxation in different orders of PAs and examine the molecular mechanisms by which chronic hypoxia attenuates endothelium-dependent pulmonary vasodilation, leading to pulmonary hypertension. Methods and Results Endothelium-d
