Directly observed isoniazid preventive therapy for released jail inmates.

This study was designed to determine whether an integrated screening program could be implemented that identified persons with latent tuberculosis infection among jail inmates and produced a high rate of completion of isoniazid preventive therapy (IPT) in those persons after their discharge from short-term detention, by means of community-based directly observed preventive therapy (DOPT). From June 1, 1992 through December 31, 1994 inmates in the King County Jail who were from populations at high risk of tuberculosis were screened by means of the tuberculin skin test and those with latent tube

Treatment of substance-abusing jail inmates. Examination of gender differences.

Females incarcerated for drug-related offenses represent one of the fastest growing populations within jails and prisons. The few studies of female offenders with substance abuse disorders depict a population with multiple psychosocial problems and treatment needs, and one that is characterized by frequent exposure to sexual abuse and other violence. The current study examined intake assessment results from a sample of 1,655 substance-involved jail inmates referred to a jail treatment program in Tampa, Florida, including 26% female and 74% male inmates. The study was designed to identify gende

Identifying homeless mentally ill veterans in jail: a preliminary report.

Increasing evidence exists that suggests associations between mental illness, homelessness, and criminal activity and arrest. This article describes a program for identifying and providing treatment and housing for homeless mentally ill veterans detained at the Cook County Jail in Chicago. Preliminary data are provided describing characteristics of the veterans assessed in the jail, as well as those veterans who follow up with services upon release. The usefulness of an urban jail as a site for outreach efforts targeting homeless mentally ill veterans is discussed.

Death by incarceration: Detention duration, overdose, and COVID-19 in Los Angeles County Jails, 2008-2023.

IMPORTANCE: Jail mortality in the United States has risen sharply in recent years, yet deaths in custody-especially those related to substance use-remain underexamined. The COVID-19 pandemic and the opioid crisis compounded pre-existing carceral health issues, necessitating a closer examination of their combined impact and aftermath. Previous reports suggest that incarcerated people-particularly those who overdose on drugs-die early in their confinement. This article indicates that the dual health threats of the COVID-19 pandemic and the opioid crisis have changed this relationship, substantia

IGRA-Based Screening for Latent Tuberculosis Infection in Persons Newly Incarcerated in New York City Jails.

In the United States, latent tuberculosis infection (LTBI) detection in correctional settings is a public health priority. Interferon gamma release assay (IGRA)-based LTBI screening was introduced in New York City jails in 2011 to 2012, replacing historically used tuberculin skin testing (TST), which was associated with substantial incomplete screening rates. This retrospective, cross-sectional study evaluated LTBI screening outcomes and correlates of positivity in 40,986 persons newly incarcerated in 2011 to 2013. Of 35,090 eligible patients tested (96.4%), final results were 6.3% positive, 9

Association between jail-based methadone or buprenorphine treatment for opioid use disorder and overdose mortality after release from New York City jails 2011-17.

BACKGROUND AND AIMS: Opioid overdose is a leading cause of death during the immediate time after release from jail or prison. Most jails in the United States do not provide methadone and buprenorphine treatment for opioid use disorder (MOUD), and research in estimating its impact in jail settings is limited. We aimed to test the hypothesis that in-jail MOUD is associated with lower overdose mortality risk post-release. DESIGN, SETTING AND PARTICIPANTS: Retrospective, observational cohort study of 15 797 adults with opioid use disorder who were released from New York City jails to the communit

Jail, an unappreciated medical home: Assessing the feasibility of a strengths-based case management intervention to improve the care retention of HIV-infected persons once released from jail.

BACKGROUND: Linkage to and retention in care for US persons living with HIV (PLWH) after release from jail usually declines. We know of no rigorously evaluated behavioral interventions that can improve this. We hypothesized that a strengths-based case management intervention that we developed for PLWH leaving jail would increase linkage/retention in care (indicated by receipt of laboratory draws) and a suppressed HIV viral load (VL) in the year following release. METHODS AND FINDINGS: We conducted a quasi-experimental feasibility study of our intervention for PLWH jailed in Atlanta. We recrui

Perinatal and Substance Use Care in North Carolina Jails: A Qualitative Organizational Context Ideal-Type Analysis.

INTRODUCTION: Around 55,000 pregnant individuals enter US jails annually, with over one-third having a substance use disorder (SUD), putting them at risk of perinatal morbidity and mortality. Yet, uptake of perinatal/SUD care best-practices into jails is slow, impacted by many organizational factors. This study explores the organizational contexts that shape decision-making structures and policy development regarding perinatal/SUD care in North Carolina (NC) jails. METHOD: We conducted semi-structured interviews with employees at 26 NC jails in 2023. Interview transcripts were de-identified a

State-Mandated Opioid Use Disorder Treatment In Maryland Jails Helped Patients Recover Despite Gaps In Care.

Providing medications for opioid use disorder (MOUD) reduces overdose risk after release from a carceral facility. In 2019, Maryland enacted a comprehensive law mandating that all jails provide all three forms of MOUD during incarceration and reentry support. The experiences of patients who received this care in jail have not been explored to date. We examined the perspectives of twenty-four people recently released from Maryland jails during the period January 2023-March 2024. Most participants noted an improvement in opioid addiction care in jail when compared with their previous jail stays.

Estimated Use of Prescription Medications Among Individuals Incarcerated in Jails and State Prisons in the US.

IMPORTANCE: Although incarcerated individuals experience higher rates of chronic conditions, little is known regarding the use of prescription medications in jails and prisons in the US. OBJECTIVE: To characterize treatment with prescription medications in jails and state prisons relative to noncorrectional settings in the US. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional analysis using 2018 to 2020 data from the National Survey on Drug Use and Health (NSDUH) estimated the prevalence of disease among recently incarcerated and nonincarcerated adults in the US. The study used 2018 to

Community reentry: perceptions of people with substance use problems returning home from New York City jails.

Each year about 100,000 people return to New York City communities from municipal jails. Although about four-fifths report drug or alcohol problems, few have received any formal drug treatment while in jail. Researchers and practitioners have identified a number of policies related to corrections, income, housing, and drug treatment that may be harmful to the successful reintegration of people leaving jail. In order to explore the challenges to successful community reentry, six focus groups and one in-depth interview were conducted with 37 men and women who had been released from jail or priso

Lessons Learned From Implementation of a Jail-Based Naloxone Vending Machine.

To address postrelease overdose risk, a large urban county jail implemented an automated naloxone vending machine to provide no-cost, low-barrier naloxone at discharge. Through carceral and public health collaboration, staff installed a custom vending machine in the jail's discharge lobby to provide individuals 24/7 self-directed access to naloxone. In addition to the existing practice of providing naloxone units only to individuals clinically flagged for acute withdrawal at intake, machine-based distribution averaged 75 units monthly during the initial 4-month active phase and 100 units month

Suicide methods in the year following release from pretrial jail detention.

PURPOSE: Despite an increasing body of knowledge on suicide methods, no previous studies have examined methods of past suicide attempts among individuals in pretrial jail detention and a follow-up after release from jail in the USA or in any other country. This study aims to describe suicide attempt methods considered and used in one's lifetime prior to pretrial jail detention and suicide attempt methods considered and used in the year following release from pretrial jail detention. PrEP attitudes, willingness, and preferences among men incarcerated in jail in Massachusetts.

BACKGROUND: People who inject drugs (PWID) are both disproportionately incarcerated and affected by HIV infection. Systemic inequities perpetuate the cyclic nature of injection drug use (IDU) and incarceration, and both IDU and incarceration are linked to higher rates of HIV infection. Pre-exposure prophylaxis (PrEP) is highly effective in HIV prevention and is currently available as a daily oral pill. Longer-acting PrEP options, such as injectables and implants, are also in development to improve accessibility and adherence. Despite these advancements, PrEP uptake remains low among PWID and i

"Jail isn't the answer for these inmates": how to respond to methamphetamine use in prisons.

PURPOSE: The correctional system continues to face challenges with responding to and managing methamphetamine use among incarcerated individuals. This study aims to uncover what resources and policies could better help correctional workers deal with these challenges. The authors also examined methamphetamine's impact on correctional work and staff well-being. DESIGN/METHODOLOGY/APPROACH: An online survey was distributed to correctional workers (n = 269) in Manitoba, Canada, featuring questions about their experiences related to methamphetamine use in populations under their care, what support

Patient Perceptions of Hepatitis C Screening and Diagnosis While in a County Jail.

Prior research in the Santa Clara County (SCC) Jail showed that knowledge of hepatitis C virus (HCV), perception of risk, and perceived coercion impacted participation in routine HCV screening; however, the experiences of patients undergoing screening were not assessed. The purpose of the study was to understand patient perceptions of undergoing routine viral hepatitis screening and receiving a diagnosis of HCV while incarcerated. A qualitative design was employed using semi-structured interviews of individuals newly diagnosed with HCV via routine screening in the SCC Jail between March and No

COVID-19 impact on opioid overdose after jail release in Massachusetts.

INTRODUCTION: Release from incarceration is a high-risk period for opioid overdose. Concern about COVID-19 spread in jails led to early releases; it is unknown whether pandemic era releases of persons with opioid use disorder (OUD) contributed to increases in community overdose rates. METHODS: Observational data compared overdose rates three months after release among jailed persons with OUD released before (9/1/2019-3/9/2020) and during the pandemic (3/10/2020-8/10/2020) from seven jails in Massachusetts. Data on overdoses come from the Massachusetts Ambulance Trip Record Information System

Correlates and Patterns in Use of Medications to Treat Opioid Use Disorder in Jail.

OBJECTIVES: Little is known about the correlates of use of medications to treat opioid use disorder (MOUD, ie, buprenorphine, methadone, naltrexone) offered in jails. We evaluated the implementation and outcomes of a MOUD program offered by 2 of the first jails nationwide to provide access to such care. METHODS: We examined use of MOUD among adults with opioid use disorder (n = 347) incarcerated by 2 rural jails in Massachusetts (2018-2021). We examined MOUD transitions from intake to during incarceration. Using logistic regression, we examined factors associated with in-jail use of MOUD. RE
