Bilateral sternoclavicular joint dislocation due to sternal fracture: Is it a dislocation or a separation?

Traumatic bilateral sternoclavicular joint dislocation is very rare injury. In shoulder girdle injuries, anterior dislocation of the sternoclavicular joint accounts for 3 % and posterior sternoclavicular joint dislocation is lesser. Previous reported cases about bilateral sternoclavicular joint dislocation were result from proximal clavicle fracture with intact connection between sternum and ribs. But, the sternoclavicular joint dislocation secondary to fracture and angulation of the sternum with intact relationship between ribs and clavicle has not been reported. Authors experienced patient w

The floating metatarsal: first metatarsophalangeal joint dislocation with associated Lisfranc dislocation.

The authors report a case of an irreducible dorsal dislocation of the first metatarsophalangeal joint, with concomitant Lisfranc dislocation and fractures of the second, third, and fourth metatarsals. This combination has been reported only once in the literature. This extremely rare combined injury results in a floating metatarsal. Open reduction of the metatarsophalangeal joint dislocation and fixation of Lisfranc joint and metatarsal fractures with Kirschner wires was performed. One year after surgery, the patient is active and the first metatarsophalangeal joint is asymptomatic, but there

Two surgical stabilization strategies for irreducible proximal tibiofibular joint dislocations: case report.

Proximal tibiofibular joint (PTFJ) dislocation is a rare and frequently underdiagnosed injury that may occur following both sports-related trauma and high-energy mechanisms. Irreducible dislocations represent a particular challenge because they often reflect complex injury patterns requiring surgical management. We report two cases of irreducible PTFJ dislocation with distinct displacement patterns: an anterolateral dislocation following a sports injury and a posteromedial dislocation associated with high-energy trauma and common fibular nerve palsy. In both cases, closed reduction was unsucce

Surgical management of chronic high-grade acromioclavicular joint dislocations: a systematic review.

BACKGROUND: To date, no gold-standard technique exists for the treatment of chronic acromioclavicular joint (ACJ) instability. We systematically reviewed the clinical results of 3 main categories of ACJ reconstruction for high-grade chronic instability. METHODS: A literature search was performed according to Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines. The inclusion criteria were clinical studies involving patients with ACJ instability (Rockwood grades III-VI) for at least 6 weeks, managed with ACJ stabilization, with a minimum 1-year follow-up. A case study of atypical Larsen syndrome with absent hallmark joint dislocations.

BACKGROUND: A family with skeletal and craniofacial anomalies is presented. Whole-exome sequencing (WES) analysis indicated a diagnosis of Larsen syndrome, although their clinical presentation does not include the hallmark joint dislocations typically observed in Larsen syndrome. METHODS: Patient consent for the sharing of de-identified clinical and genetic information, along with use of photographs for publication, was obtained. WES and variant segregation analysis by WES were performed by commercial laboratory, GeneDx (Gaithersburg, MD), on peripheral blood samples from the proband, her bro

Transarticular plating for acute posterior sternoclavicular joint dislocations: a valid treatment option?

BACKGROUND: Restoration of the sternoclavicular joint after posterior dislocation is important for upper limb function. The proximity of neuro-vascular mediastinal structures makes open reduction and internal fixation (ORIF) a high-risk procedure. The ideal treatment strategy is still debated. Our aim is to review the treatment of acute posterior sternoclavicular joint dislocation with a locking compression plate. MATERIALS AND METHODS: We present our experience of transarticular plating using a locking compression plate for the treatment of three consecutive patients with posterior sternocla

Unstable dorsal proximal interphalangeal joint fracture-dislocations treated with extension-block pinning.

Unstable proximal interphalangeal (PIP) joint fracture-dislocations, which can cause significant disability, can be treated with multiple techniques. Extension-block pinning (EBP) allows for early motion and is less technically demanding than alternative surgical treatments. In the study reported here, 12 patients with unstable dorsal PIP fracture-dislocations were treated with closed reduction of the PIP joint followed by percutaneous insertion of a Kirschner wire (K-wire) into the distal aspect of the proximal phalanx. For these patients, extent of articular surface involvement averaged 43%

Arthroscopic Stabilization of Chronic Acromioclavicular Joint Dislocations: Triple- Versus Single-Bundle Reconstruction.

BACKGROUND: Arthroscopically assisted single-bundle (SB) or double-bundle coracoclavicular (CC) ligament reconstruction using autologous tendon grafts has been reported to provide acromioclavicular (AC) joint (ACJ) stability in chronic instability cases. Recently, additional AC ligament reconstruction to provide triple-bundle (TB) stabilization has been introduced but lacks a comparison of clinical and radiological outcomes. HYPOTHESIS: Arthroscopically assisted anatomic TB CC and AC reconstruction yields superior clinical and radiological results when compared with nonanatomic SB CC reconstr

A novel radiographic index for the diagnosis of posterior acromioclavicular joint dislocations.

BACKGROUND: Posterior acromioclavicular (AC) joint dislocations are frequently misclassified because posterior translation of the clavicle is difficult to evaluate in Zanca radiograph views. A novel radiographic index was used in this study to accurately diagnose posterior dislocations of the AC joint. HYPOTHESIS: This novel index has a high degree of accuracy for the diagnosis of posterior AC joint dislocations. STUDY DESIGN: Cohort study (diagnosis); Level of evidence, 2. METHODS: This was an analytic, descriptive study of 150 patients with different grades of AC injuries according to the

[Lower extremity nerves injuries in bone fractures and joint dislocations--etiopathogenesis, treatment].

Lower extremity nerve injuries accompanying bone fractures and joint dislocations lead to serious disturbances at the lower limbs and caused significant prolongation of the treatment time. Clinical material consisted of 73 patients treated surgically with lower extremity nerve injuries due to bone fractures and joint dislocation. Based on this material we analysed the most frequent localisations of the nerves lesions and the mechanism of injury. The indications to surgical treatment were also described.

Fracture dislocations of Lisfranc's joint treated with closed reduction and percutaneous fixation.

We reviewed 42 patients (mean age 37.7+/-14.2 years) with closed fracture dislocations of Lisfranc's joint treated with percutaneous screw fixation. Mean follow-up was 58.4+/-17.3 months. The aim was to compare dislocations in which a perfect anatomical reduction had been reached with dislocations in which reduction was only near anatomical. The mean American Orthopaedic Foot and Ankle Society score for all patients was 81.0+/-13.5. There were no significant differences in outcome scores between patients with perfect anatomical reduction and patients with near anatomical reduction. However, pa

[Dislocations of the carpometacarpal joint of the thumb--results of follow-up].

This report deals with twenty-nine dislocations of the carpometacarpal joint of the thumb--twenty-five closed and four compound dislocations, three of which were part of complex injuries of the hand--treated during the period 1957 to 1983. The closed dislocations comprised eighteen dorsal, three dorsal-ulnar, two dorsal-radial, one palmar, and one palmar-ulnar. The compound dislocations comprised one radial (isolated), one palmar-radial, one dorsal-radial, and one palmar-ulnar; the last three occurring together with complex injuries. Eleven patients were followed up after an average period of

Irreducible compound dorsal dislocations of the proximal interphalangeal joint of the finger.

Three cases of compound irreducible dorsal dislocation of the proximal interphalangeal joint of the finger are reported. The probable mechanism is discussed. The findings warrant the description "volar capsular boutonnière" as the condyles of the proximal phalanx buttonhole, through the volar structures. Open reduction combined with debridement, is the treatment for such compound irreducible dorsal dislocations. Dislocation of the proximal interphalangeal joints of the fingers are common and can usually be reduced by simple traction. Occasionally reduction by closed methods is unsuccessful bec
