Phototherapeutic Keratectomy in Salzmann Nodular Degeneration With "Optical Cornea Plana".

PURPOSE: Typically, we observe an unintended hyperopic shift after phototherapeutic keratectomy (PTK). Despite a clear optical axis, elevations of the mid-peripheral cornea often lead to remarkable visual impairment in Salzmann nodular degeneration (SND). The aim of this study was to analyze the effect of mechanical pannus removal and PTK on the corneal curvature in SND. METHODS: Fifteen eyes of 10 patients with visual impairment caused by SND were treated with combined pannus removal and PTK using a masking fluid. Mean age was 53 years. We analyzed best-corrected visual acuity, central corne

Biologic and environmental risk factors for regression after photorefractive keratectomy.

BACKGROUND/PURPOSE: After photorefractive keratectomy (PRK), the final refraction in the majority of patients is near the intended refraction. However, a significant proportion of patients show regression, which is commonly associated with corneal haze. This study aims to determine whether biologic and environmental factors influence the outcome of PRK. METHODS: One hundred patients prospectively underwent -3.00- or -6.00-diopter corrections. Three months after surgery, 100% of patients answered a questionnaire that recorded their biologic characteristics, medical and ocular history, and post

Night vision after excimer laser photorefractive keratectomy: haze and halos.

A series of 85 patients with myopia, up to -6.00D, was treated by photorefractive keratectomy (PRK), using a 5 mm diameter ablation zone. At six months, 38 patients (45%) reported slight disturbances of night vision, nine (11%) of whom had significant problems. Perturbations of night vision after PRK are seen as starbursts and halos around lights. Corneal haze produces the starbursts, which are usually transient. In contrast, halos are myopic blur circles and may be persistent. Using a computer program, halos after PRK were found to be significantly larger than those in emmetropes and myopes c

Excimer laser photorefractive keratectomy in high myopia. A multicenter study.

Excimer photorefractive keratectomy was performed at three centers on 16 highly myopic eyes (8 diopters [D] or more) and followed up for 6 months. Ablation depths ranged from 137 to 230 microns. The preoperative spherical equivalent of myopia ranged from -8.62 D to -14.50 D (mean +/- SD, -11.57 +/- 1.62 D). Six months after surgery, the mean refraction (spherical equivalent) was -0.90 +/- 2.13 D. Eleven of 16 eyes achieved refractions within 2 D of that attempted. All eight patients at one site were treated with a maximum-beam diameter of 6.0 mm and were corrected to within 2 D of that attempt

Adhesion complex formation after small keratectomy wounds in the cornea.

The adhesion complex of the corneal epithelium consists of the hemidesmosome and its associated structures, such as anchoring filaments, lamina densa of the basement membrane, and anchoring fibrils. It contributes to the adhesion of the corneal epithelium to Bowman's layer. To understand the adhesion complex better, an electron microscopic and immunofluorescence analysis was done of the reformation of the adhesion complex in small (1 mm) keratectomy wounds in the guinea pig cornea. In these wounds, the epithelium, hemidesmosomes, basal lamina, anchoring fibrils, and anterior stroma were remove
