A 53-year-old woman presented with hirsutism of long duration and virilization of recent onset. She was gravida 12 para 9. The 17-ketosteroids were normal, gonadotropins were in the post-menopausal range, and the serum testosterone was in the male range. After bilateral oophorectomy, the testosterone became normal. Ovarian pathology revealed stromal hyperthecosis.

A case of maternal virilization associated with bilateral luteomas of pregnancy is described. Urinary 17-ketosteroids and plasma testosterone fell from markedly abnormal values to normal limits within 2 weeks of delivery. However, symptoms of virilization persisted in the postpartum state. A possible relation between polycystic ovarian syndrome in the nonpregnant state and luteoma of pregnancy is proposed.

Long-term results of treatment of colorectal cancer versus levels of 17-ketosteroids, 17-ketogenic steroids, adrenaline and norepinephrine in diurnal urine were evaluated in 116 patients. Normal or slightly elevated urine-corticosteroid and catecholamine levels were shown to correlate with good prognosis, particularly, in cases with normal excretion of all (or almost all) hormones studied and their normal ratios.

A new dansylation reaction, where trifluoromethanesulfonic acid is used as catalyst, has been characterized for six ketosteroids by employing experimental design followed by multivariate data analysis. On the basis of clinical and hormonal investigations of 58 girls with hirsutism aged between 15-19 years there were confirmed congenital adrenal hyperplasia in 3.4%, late onset adrenal hyperplasia in 24.1% and polycystic ovary syndrome in 72.4%. The proper diagnosis were established above all on endocrinological investigations especially on determinations of 17- ketosteroids, of DHEA, of 17 OH-corticoids in urine and RIA of total and free testosterone in blood.

The experience with treatment of 34 children with gynecomastia is summarized. The examination included roentgenography of the Turkish saddle, definition of bone age, investigation of buccal sex chromatin, excretion of 17-ketosteroids, excretory urography against the background of retropneumoperitoneum, ultrasound investigation of the mammary glands. In juvenile gynecomastia, the mammary glands were removed, the expectant tactic being considered unwarranted. The long-term results of the surgical treatment are good.

Two patients with metastatic adrenal cortical carcinoma were treated with cisplatin and etoposide. Both achieved a partial remission after only one cycle. One patient had a "nonfunctioning" tumor that had previously regressed on mitotane whereas the second patient presented with classical Cushing's syndrome. Although the latter patient's urinary 17-ketosteroids normalized while receiving mitotane, the tumor itself failed to regress objectively. Cisplatin plus etoposide appears to be an active combination for the treatment of metastatic adrenal cortical carcinoma.

A left ovarian gynandroblastoma in a 24-year-old woman was clinically manifest by prolonged (7 years) amenorrheas interrupted by metrorrhagies, with moderate hypertrichosis is presented. An important urinary elimination of estrogens (1.500 gamma/24 h) and of 17-ketosteroids (25 gamma/24 h) with normal 17-OH and gonadotrophins were evidenced. Elevated concentrations of peptide hormones have been described previously in human breast fluid. In the current study, the levels of cortisol, progesterone, testosterone, dihydrotestosterone, androsterone, androsterone sulfate, dehydroisoandrosterone, dehydroisoandrosterone sulfate, estradiol, estrone, estradiol sulfate, and estrone sulfate were measured. The levels of the four 17-ketosteroids and the two estrogen sulfates were markedly elevated over the plasma level, while that of the other compounds was the same or only slightly higher than the plasma levels of the same compounds.

The authors present the case of a young hirsute woman of 28 years old. Urinary 17 KS were very increased without DHA, but with androsterone, etiocholanolone and 11 oxy 17 Ketosteroids. Androstenedione and testosterone plasmatic levels were very high and per operative selective ovarian vein catheterization showed a direct production of these potent androgens. In vitro study of the tumor allowed to obtain same results. Light and electron microscopic study showed a Leydig cell tumor of left ovary.

BACKGROUND: Hirsutism in women is usually caused by benign adrenal or ovarian disorders, but it can also be caused by adrenal carcinoma. The most effective way to identify such carcinomas is not known.
METHODS: We measured serum and urinary steroids before and after the administration of 3 mg of dexamethasone per day for five days in 14 hirsute women with histologically proved adrenal tumors (12 adrenal carcinomas and 2 adrenal adenomas) and in 73 women with hirsutism of non-neoplastic origin.

A secretion was collected from the nipple and was examined. Hirsutism and abnormal pigmentation were lacking, and the aspect of the external genitalia was normal. The cromatin sex was male. The sella turcica had a normal roentgenologic aspect. The urinary 17-ketosteroid and oestrogen excretion was normal. The swelling was removed surgically. A histologic diagnosis of gynaecomasty was made. The differential diagnosis, classification and physiopathology of gynaecomasty are discussed.
