Markers of Kidney Tubule Dysfunction and Major Adverse Kidney Events.

BACKGROUND: Serum creatinine and albuminuria are primary markers of glomerular function and injury, respectively. Tubular secretion, acid-base homeostasis, protein reabsorption, among other tubular functions, are largely ignored. This mini-review aimed to discuss how two tubular functions, secretion, and acid-base homeostasis are associated with major adverse kidney events (MAKEs). SUMMARY: Proximal tubular secretion is an essential function that allows the elimination of endogenous substances and drugs. Recently discovered endogenous markers in urine and plasma allow a noninvasive way of ass

Impact of Deceased-donor Acute Kidney Injury on Kidney Transplantation.

Even as record numbers of deceased donors are undergoing organ recovery, the global transplant community continues to struggle with a shortage of donor organs and a high organ discard rate. Acute kidney injury (AKI) occurs in many hospitalized patients, including up to 25% of patients in critical condition. Registry studies have shown a significant increase in nonrecovery or organ discard rates in AKI donors, despite most studies reporting similar clinical outcomes compared with non-AKI donors. This review aims to capture the salient information learned from these studies and to summarize the

Longitudinal biomarkers and kidney disease progression after acute kidney injury.

BACKGROUNDLongitudinal investigations of murine acute kidney injury (AKI) suggest that injury and inflammation may persist long after the initial insult. However, the evolution of these processes and their prognostic values are unknown in patients with AKI.METHODSIn a prospective cohort of 656 participants hospitalized with AKI, we measured 7 urine and 2 plasma biomarkers of kidney injury, inflammation, and tubular health at multiple time points from the diagnosis to 12 months after AKI. We used linear mixed-effect models to estimate biomarker changes over time, and we used Cox proportional ha

Leptospirosis kidney disease: Evolution from acute to chronic kidney disease.

Leptospirosis is a neglected bacterial disease caused by leptospiral infection that carries a substantial mortality risk in severe cases. Research has shown that acute, chronic, and asymptomatic leptospiral infections are closely linked to acute and chronic kidney disease (CKD) and renal fibrosis. Leptospires affect renal function by infiltrating kidney cells via the renal tubules and interstitium and surviving in the kidney by circumventing the immune system. The most well-known pathogenic molecular mechanism of renal tubular damage caused by leptospiral infection is the direct binding of the

Bilateral Pelvic Kidney Masquerading as Horseshoe Kidney in Fanconi Anemia.

Fanconi anemia (FA) is a genetic disease associated with a risk of congenital malformations, bone marrow failure, and a predisposition to cancer. Congenital abnormalities of the kidney and urinary tract are not infrequent in FA, with renal ectopia being among them. The incidence of bilateral pelvic kidney is restricted to only a few reported cases; however, its association with FA has never, to our knowledge, been reported in the literature. We present a case of FA in a girl whose 99mTc-dimercaptosuccinic acid planar scan showed apparently fused kidneys, which were confirmed to be bilateral pe

Timing of kidney replacement therapy initiation in acute kidney injury.

PURPOSE OF REVIEW: Over the past 5 years, four major randomized controlled trials were published informing our practice on the optimal timing for kidney replacement therapy (KRT) initiation in critically ill patients with acute kidney injury (AKI). In this review, we summarize the main findings of these trails and discuss the knowledge gaps that still need to be addressed. RECENT FINDINGS: Four recent trials compared early versus delayed initiation of KRT in critically ill patients with acute kidney injury. Though each trial had unique design features, the three largest trials showed that ear

Renal Cell Carcinoma on the Native Kidney Following Kidney Transplantation.

A 48-year-old man with histories of IgA nephropathy for 33 years, hemodialysis for 29 years, and a kidney transplant from a deceased donor 5 years ago was admitted to our institute complaining of high fever and back pain. Although repeated follow-up of computed tomography failed to detect any de novo issues, he was eventually diagnosed as a renal cell carcinoma with multiple metastases, developing from his native-acquired cystic disease kidney with multiple cysts using a positron emission tomography. We should be cautious of de novo renal cell carcinoma in kidney transplantation recipients, an

Is Progressive Chronic Kidney Disease a Slow Acute Kidney Injury?

International Renal Interest Society chronic kidney disease Stage 1 and acute kidney injury Grade I categorizations of kidney disease are often confused or ignored because patients are nonazotemic and generally asymptomatic. Recent evidence suggests these seemingly disparate conditions may be mechanistically linked and interrelated. Active kidney injury biomarkers have the potential to establish a new understanding for traditional views of chronic kidney disease, including its early identification and possible mediators of its progression, which, if validated, would establish a new and sophist

Pemetrexed-induced acute kidney injury leading to chronic kidney disease.

INTRODUCTION: Methadone is an effective medication for treating pain and has unique characteristics that require specialized knowledge to prescribe safely. Palliative care providers use methadone for analgesia in patients with a wide range of prognoses, goals of care, and comorbid conditions. New consensus guidelines for methadone use released in 2019 by the American Academy of Hospice and Palliative Medicine provide guidance for safe use in patients who have potentially life-limiting illnesses. A needs assessment of palliative care fellows and faculty at our institution highlighted lack of kn

Kidney volume and function in autosomal dominant polycystic kidney disease.

BACKGROUND: The significance of total kidney volume (TKV) as a biomarker of kidney function in autosomal dominant polycystic kidney disease (ADPKD) is controversial and has been reappraised. METHODS: Between 2007 and 2012, 64 patients were followed with a mean 39.7-month observation period. TKV measurements by magnetic resonance imaging and estimation of renal function with estimated glomerular filtration rate (eGFR) using the Modification of Diet in Renal Disease equation and 24-h urine creatinine clearance were repeated annually. RESULTS: TKV and its adjusted parameters (height-adjusted, b

[Managing anaemia in kidney transplant patients with chronic kidney disease].

DEFINITION: The definition of anemia is established by the World Health Organization and was subsequently adopted by the American Society of Transplantation, which defines anemia as hemoglobin concentration <12 g/dl in women and <13 g/dl in men. PREVALENCE OF ANEMIA POSTTRANSPLANTATION: Varies throughout the posttransplantation period and is associated with the degree of renal graft function. The relationship between hemoglobin levels and glomerular filtration does not behave the same way as in the population with chronic kidney disease. The results of various studies show a high prevalence in

Improvement of kidney failure with fetal kidney precursor cell transplantation.

BACKGROUND: Current therapies for end-stage renal disease have severe limitations. Dialysis is only a temporary treatment and does not restore kidney function. Transplantation is limited by donor organ shortage and immune-related problems. Here, we show that the transplantation of fetal kidney precursor cells reconstitutes kidney tissues, reduces uremic symptoms, and provides life-saving metabolic support in kidney failure animal models. METHODS: Kidney failure was surgically induced by resecting kidneys, leaving approximately 1/6 of the total kidney mass (5/6 nephrectomy). Fetal kidney precu

Kidney and kidney/pancreas transplantation at Stanford University Medical Center.

The disparity between the supply of cadaveric donors and the demand for renal allografts continues to grow. We have taken a multifaceted approach to increase the allograft pool: 1. Spiral computed tomography to evaluate potential living kidney donors is safer, less invasive, less expensive and more time efficient and thus should encourage living organ donation. 2. Use of selected expanded criteria cadaveric donor kidneys (aged 60 or over, hypertensive) in size- and age-matched recipients have short-term function at 3 and 6 months comparable to standard cadaveric renal allografts. 3. Kidneys fr

[Renocortical oxygen supply after kidney transplantation. Renocortical oxygen supply and kidney circulation after autologous kidney transplantation in the dog; its value for human kidney transplantation].

Determination of the viability of a transplanted kidney is still an unsolved problem. Apart from immunological effects hemodynamic and microcirculatory changes in the kidney are important. Thus in 39 mongrel dogs renal floow flow (electromagnetic and local renocortical oxygen supply) and microcirculation (by meaning local tissue pO2) were determined before nephrectomy, immediately after revascularization and before sacrification in experiments with autologous transplantation. Whole kidney blood flow decreased by 30-40%. Measurements of tissue pO2 revealed distinct changes in renocortical oxyge

Knowledge of Kidney Health among Those with or at Risk for Kidney Disease: Implications for Chronic Kidney Disease Interventions.

The purpose of this study was to assess the knowledge of individuals with or at risk for chronic kidney disease (CKD) and whether that knowledge differed as a function of race, socio-economic status, education level, and rural/urban living status. Knowledge about kidney disease is important to prevent or manage CKD. Data for this descriptive research were collected from 519 adults living in Alabama, reflecting diversity in age, gender, and race. Knowledge deficits were identified. Nephrology nurses have a role in preventing and promoting early diagnosis of CKD through education of individuals

Impact of kidney function and kidney volume on intracranial aneurysms in patients with autosomal dominant polycystic kidney disease.

Presently, only personal or family history of intracranial aneurysm/subarachnoid hemorrhage (IA/SAH) has been established as a risk factor for IA in autosomal dominant polycystic kidney disease (ADPKD). This study aimed to verify the association between kidney function/volume and IAs in patients with ADPKD. This study included 519 patients with ADPKD. At baseline IA screening, the median age and estimated glomerular filtration rate were 44 years and 54.5 mL/min/1.73 m2, respectively. Family IA/SAH history was confirmed in 18.1% of the patients, and 54.3% of the patients had hypertension. The I

Potassium and the kidney.

In the past decade, understanding of the mechanisms by which the kidneys sense dietary potassium imbalance and affect the cardiorenal system has substantially advanced. These mechanisms explain why low-potassium diets promote salt-sensitive hypertension, whereas potassium-rich diets enhance natriuresis and lower blood pressure. Increasing dietary potassium may therefore be an effective strategy to reduce cardiovascular disease risk. Clinical trials have shown that replacing regular salt with potassium-enriched substitutes lowers blood pressure and reduces cardiovascular events and mortality. E
