A clinical decision rule in the evaluation of acute knee injuries.

We constructed a clinical decision rule to optimize the use of radiography in patients with acute knee injuries. A prospective observational study at a university hospital ED was conducted over 10 months. Patients 15 years of age and older with acute knee injuries who underwent radiography were included. Patients were excluded if they were intoxicated, had distracting injuries, previous knee surgery, or open wounds. A standardized closed question data collection instrument that recorded 12 historical and 26 physical examination criteria was used. Radiographs were interpreted by two radiologist

Disruption of the vastus medialis obliquus with medial knee ligament injuries.

We reviewed the clinical records of 189 consecutive surgically treated acute ligamentous injuries of the medial compartment of the knee to determine the prevalence of disruptions of the vastus medialis obliquus muscle and to document the results of simultaneous repair of the disruption. Forty knees (40 patients) demonstrated a vastus medialis muscle disruption at the time of surgical repair for the medial ligamentous disruption. All were surgically corrected and the sites of tearing were documented. The vastus medialis obliquus muscle was ruptured from the adductor tubercle in 31 (78%) knees.

Arthroscopy in the early diagnosis of injuries to the knee joint.

In a series of 192 patients with acute knee injuries the diagnosis at arthroscopy was compared with the clinical diagnosis with or without confirmation at arthrotomy. Arthroscopy was used in 117 cases, clinical examination in 72 cases, and arthrography in 3 cases. All patients presented for diagnosis and treatment within 4 weeks of injury, and most within 1 week. The anterior cruciate ligament (ACL) was torn in 100 cases, and in 92 this was the main injury. The incidence of complete, isolated ACL tear was 1.5 per cent. ACL tear occurred in a higher proportion of cases in the arthroscopy group

Holster accidents: self-inflicted gunshot injuries of the knee and leg.

A man accidentally shot himself in the knee while holstering his own revolver. Although the pain was minimal, the wound "clean", and the joint flexible, angiography revealed a false aneurysm of the popliteal artery, a short segment of minimal tears below it, and a popliteal arteriovenus fistula slightly distal to the false aneurysm. Any penetrating wound of the popliteal fossa whose trajectory points to the vascular structures should be investigated angiographically.

Injury History and Perceived Knee Function as Risk Factors for Knee Injury in Youth Team-Sports Athletes.

BACKGROUND: The identification of risk factors for sports injuries is essential before injury prevention strategies can be planned. HYPOTHESIS: Previous acute knee injury and lower perceived knee function measured by Knee injury and Osteoarthritis Outcome Score (KOOS) will increase the risk of acute knee injury in youth team-sports athletes. STUDY DESIGN: Prospective cohort study. LEVEL OF EVIDENCE: Level 3. METHODS: At baseline, youth (≤21 years old) male and female basketball and floorball athletes completed a questionnaire on previous acute knee injuries and perceived knee function (KOO

Floating Knee Severity Score (FKS): A Novel Multidimensional Prognostic Model for Predicting Functional Outcomes After Floating Knee Injuries.

Purpose: Floating knee (FK) injuries are complex high-energy traumas associated with poor functional outcomes. This study aimed to identify independent predictors of functional prognosis and to develop a novel, multidimensional scoring system to predict long-term functional outcomes. Methods: A retrospective analysis was performed on 182 adult patients with ipsilateral femur and tibia fractures treated between January 2010 and December 2023. Functional outcomes were assessed using the Karlström-Olerud criteria and dichotomized as excellent-good versus fair-poor. Variables significant in univar

Multiligament Reconstruction of the Knee in the Setting of Knee Dislocation With a Medial-Sided Injury.

Multiple ligament knee injuries are complex pathologies that often result from traumatic knee dislocations. Both a high level of suspicion and a thorough clinical and radiographic examination are mandatory to diagnose and identify all injured structures. Reconstruction of all injured ligaments is recommended to aid in early mobilization and to avoid joint stiffness or graft failure. For knee dislocations involving injury to the anterior cruciate ligament, posterior cruciate ligament, and medial-sided structures, a repair and augmentation of the medial collateral ligament, together with an anat

Fracture of the Fabella: An Uncommon Injury in Knee.

The fabella is a sesamoid bone that may contribute to the stabilization of the posterolateral knee corner and it can very occasionally act as a source of atypical and rare knee pain and functional impairment. Fracture of the fabella is a rare but important clinical entity which may be overlooked clinically and radiographically. However, it causes an intermittent mechanical pain of the knee and it can mistakenly harm another knee pathology like intra-articular loose body. We report a case of a 21-year-old man who was sustaining a fracture of fabella following vehicle accident.
