Direct end-to-end repair of flexor pollicis longus tendon lacerations.

Between 1976 and 1986, 38 consecutive acute isolated flexor pollicis longus lacerations were repaired. This study excluded all replanted or mutilated digits and all lacerations with associated fracture. Average follow-up was 26 months. Tendon rehabilitation was standardized. Range of motion and pinch strength were measured postoperatively. Seventy-four percent (28/38) of the flexor pollicis longus injuries occurred in zone II. Neurovascular injury occurred in 82% of the lacerations, and this correlated with the zone of tendon injury. In 21% of the patients (8/38) both digital nerves and arteri

Splenic lacerations: a retrospective analysis of management strategies and clinical outcomes.

BACKGROUND: Splenic injuries are among the most frequently encountered conditions in abdominal trauma. In such cases, the treatment approach must be carefully determined based on the patient's hemodynamic stability, the severity of the injury, and the presence of associated intra-abdominal pathologies, typically requiring a choice between non-operative management (NOM) and surgical intervention. The decision-making process is primarily guided by hemodynamic status, injury grade, and imaging findings. This study evaluates clinical outcomes and the factors influencing treatment decisions in pati

Cerebral white matter lacerations in children caused by repetitive head trauma.

It has been known that infants less than 1 year develop cerebral white matter (WM) lacerations associated with head trauma, however, there has been no report of similar WM lesions over 1 year. We report three teenage boys (11, 12, and 18 years at final MRI studies) with acquired WM lacerations associated with recurrent head trauma who developed neurologic symptoms such as spastic paralysis, afebrile convulsions, and cognitive impairment. Two of them (patients 1 and 2) were given a diagnosis of autism spectrum disorder and had a history of repeated severe self-inflicted head trauma from prescho

Severe perineal lacerations after vaginal delivery: are they an anesthesiologist's problem?

PURPOSE OF REVIEW: Perineal tears or lacerations are common occurrences after vaginal delivery. Understanding the degree of severity of these tears and the immediate and long-term complications of severe perineal lacerations can assist anesthesiologists with the management of these patients in the immediate postpartum period. RECENT FINDINGS: Severe perineal lacerations have a high degree of association with postpartum depression. The presence of neuraxial labor analgesia decreases the odds of severe perineal lacerations. SUMMARY: Neuraxial labor analgesia does not directly predispose partur

[Post-intubation tracheal lacerations during cervical spine surgery: about 3 cases].

We report 3 rare cases of post-intubation tracheal lacerations detected during cervical spine surgery. Our study involved 3 patients aged 27, 30 and 42 years admitted to the operating room for total thyroidectomy for heterogeneous multinodular goitre under general anesthesia. Orotracheal intubation using 7.5 rigid guide probe and balloon cuff was performed. During surgery we discovered a protrusion of the balloon cuff at the surgical site through posterolateral tracheal rupture. The treatment was based on closure of the tracheal rupture in 2 cases and on surgical abstention in the third case,

Perineal body length and perineal lacerations during delivery in primigravid patients.

This study assessed the relation between perineal body length and the risk of perineal laceration extending into the anal sphincter during vaginal delivery in primigravid patients at an institution with a low utilization of episiotomy. This was a prospective study of primigravid patients in active labor. Primigravid women with singleton pregnancies who were in the first stage of labor at 37 weeks gestation or greater were recruited, and the admitting physician measured the length of the perineal body. The degree of perineal laceration and other delivery characteristics were recorded. Data were

Lacerations and Embedded Needles Caused by Epinephrine Autoinjector Use in Children.

STUDY OBJECTIVE: Epinephrine autoinjector use for anaphylaxis is increasing. There are reports of digit injections because of incorrect autoinjector use, but no previous reports of lacerations, to our knowledge. We report complications of epinephrine autoinjector use in children and discuss features of these devices, and their instructions for use, and how these may contribute to injuries. METHODS: We queried emergency medicine e-mail discussion lists and social media allergy groups to identify epinephrine autoinjector injuries involving children. RESULTS: Twenty-two cases of epinephrine aut

Cervical lacerations in planned versus labor cerclage removal: a systematic review.

OBJECTIVE: The aim of this study was to evaluate the incidence of cervical lacerations with cerclage removal planned before labor compared to after the onset of labor by a systematic review of published studies. STUDY DESIGN: Searches were performed in electronic databases from inception of each database to November 2014. We identified all studies reporting the rate of cervical lacerations and the timing of cerclage removal (either before or after the onset of labor). The primary outcome was the incidence of spontaneous and clinically significant intrapartum cervical lacerations (i.e. lacerat

Anatomic guide and sonography for surgical repair of leg muscle lacerations.

BACKGROUND: There were over 110,000 leg laceration cases reported in the United States in 2011. Currently, muscle laceration is repaired by suturing epimysium to epimysium. Tendon-to-tendon repair is stronger, restores the muscle's resting length, and leads to a better functional recovery. Tendons retract into the muscle belly following laceration and surgeons have a difficult time finding them. Many surgeons are unfamiliar with leg muscle anatomy and the fact that the leg muscles have long intramuscular tendons that are not visible in situ. A surgical anatomic guide exists to help surgeons lo

Effect of rotation on perineal lacerations in forceps-assisted vaginal deliveries.

OBJECTIVE: To determine the difference in the rates of severe perineal lacerations between forceps-assisted vaginal deliveries in the occiput-posterior position compared with forceps-assisted vaginal deliveries in which the fetal head was rotated to occiput-anterior before delivery. METHODS: We studied a retrospective cohort of 148 women who had a forceps-assisted vaginal delivery from 2008 to 2011 at the University of Pittsburgh. Mild perineal lacerations were defined as first or second degree, and severe lacerations were defined as third or fourth degree. χ and t tests were used for bivaria

[Pontomedullary lacerations and concomitant injuries: a review of possible underlying mechanisms].

Anatomically, brainstem is constituted of medulla oblongata, pons and mesencephalon. Traumatic lesions of brainstem most commonly occur on pontomedullary junction. There are several possible mechanisms of pontomedullary lacerations. The first mechanism includes impact to the chin, with or without a skull base fracture, and most often leads to this fatal injury, due to impact force transmission through the jawbone and temporomandibular joint. The second mechanism includes lateral and posterior head impacts with subsequent hinge fractures, where occurrence of pontomedullary lacerations in these

Periorbital infections after Dermabond closure of traumatic lacerations in three children.

PURPOSE: To report the occurrence of periorbital infections in 3 children treated with the tissue adhesive 2-octyl cyanoacrylate (Dermabond) after traumatic periorbital laceration. METHODS: We retrospectively reviewed the records of consecutive patients referred to Vanderbilt Children's Hospital for the treatment of periorbital infections to identify cases associated with the use of Dermabond. The clinical features and outcomes of each case were reviewed. We performed a meta-analysis of published cases to identify any association of tissue adhesive with wound infection rate. RESULTS: The rev

Third- and fourth-degree perineal lacerations: defining high-risk clinical clusters.

OBJECTIVE: Statistical methods that measure the independent contribution of individual factors for third-/fourth-degree perineal laceration (TFPL) fall short when the clinician is faced with a combination of factors. Our objective was to demonstrate how a statistical technique, classification and regression trees (CART), can identify high-risk clinical clusters. STUDY DESIGN: We performed multivariable logistic regression, and CART analysis on data from 25,150 term vaginal births. RESULTS: Multivariable analyses found strong associations with the use of episiotomy, forceps, vacuum, nullipari

The diagnostic value of oral lacerations and incontinence during convulsive "seizures".

PURPOSE: Oral lacerations and urinary incontinence have long been considered useful clinical features for the diagnosis of epileptic seizures; however, both are also reported in patients with psychogenic nonepileptic seizures (PNES). The aims of the study were (1) to investigate whether the presence and nature of oral lacerations or incontinence during convulsive seizures of patients with epilepsy differed from those with PNES, and (2) whether the side of the oral laceration has any correlation with the epilepsy syndrome or lateralization. METHODS: Eighty-four consecutive patients who experie

Case report: labial fusion postpartum and clinical management of labial lacerations.

Lacerations of the external genitalia of various degrees are a common occurrence in the childbirth process. Evidence-based management of minor lacerations of the vulva has yet to emerge in the scientific literature. Spontaneous approximation of minor lacerations of the labia may uncommonly result in distorted anatomical healing, with resultant dyspareunia, among other distressing symptoms. Management of this occurrence, including pharmacologic and surgical strategies, is presented in this case study discussion.
