Intelligence without intuition: a mixed-methods pilot study on reasoning models in musculoskeletal physiotherapy for low-back pain.

Musculoskeletal pain, especially low-back pain, is highly prevalent and often challenging to manage due to its multifactorial nature. Effective diagnosis and therapy require clinicians to integrate biopsychosocial information within an evidence-based clinical reasoning framework. Large language models that "think" before responding, so-called reasoning models, show promise to support such complex decision-making, yet their validity and reliability in this setting remain unclear. In our work, we present a comprehensive human evaluation of reasoning models for clinical reasoning. Our results ind

Molecular Descriptors Property Prediction Using Transformer-Based Approach.

In this study, we introduce semi-supervised machine learning models designed to predict molecular properties. Our model employs a two-stage approach, involving pre-training and fine-tuning. Particularly, our model leverages a substantial amount of labeled and unlabeled data consisting of SMILES strings, a text representation system for molecules. During the pre-training stage, our model capitalizes on the Masked Language Model, which is widely used in natural language processing, for learning molecular chemical space representations. During the fine-tuning stage, our model is trained on a smal

Effect of embryo selection based morphokinetics on IVF/ICSI outcomes: evidence from a systematic review and meta-analysis of randomized controlled trials.

PURPOSE: Debate exists for the optimal tool to select embryos for transfer in assisted reproductive technology (ART). Time-lapse monitoring (TLM) is a noninvasive tool suggested where each embryo can be captured every 5-20 min. Given the inconsistency in the existing studies, we conducted this meta-analysis of RCTs to summarize the evidence available concerning the predictive ability of morphokinetics compared with the routine assessment of embryo development in ART. METHODS: The primary databases MEDLINE, EMBASE, Cochrane, NHS, WHO, and Other Non-Indexed Citations were consulted for RCTs tha

Temporal proximity to counseling is associated with longitudinal bedaquiline and antiretroviral therapy adherence in multidrug-resistant tuberculosis and HIV treatment.

BACKGROUND: Medication adherence is a persistent challenge in the treatment of multidrug-resistant tuberculosis (MDR-TB) and HIV. Routine counseling notes are an underutilized source of information on patient adherence context. METHODS: We applied natural language processing (NLP) to routine counseling notes from a randomized controlled trial of MDR-TB/HIV treatment in South Africa. Topics were derived using topic modeling; notes were semantically matched to topics and clustered using K-means. We fitted linear mixed-effects (LME) regression models with participant-level random intercepts and

The Oxford-Durham study: a randomized, controlled trial of dietary supplementation with fatty acids in children with developmental coordination disorder.

BACKGROUND: Developmental coordination disorder (DCD) affects approximately 5% of school-aged children. In addition to the core deficits in motor function, this condition is associated commonly with difficulties in learning, behavior, and psychosocial adjustment that persist into adulthood. Mounting evidence suggests that a relative lack of certain polyunsaturated fatty acids may contribute to related neurodevelopmental and psychiatric disorders such as dyslexia and attention-deficit/hyperactivity disorder. Given the current lack of effective, evidence-based treatment options for DCD, the use

Autodelineation of Treatment Target Volume for Radiation Therapy Using Large Language Model-Aided Multimodal Learning.

PURPOSE: Artificial intelligence-aided methods have made significant progress in the auto-delineation of normal tissues. However, these approaches struggle with the auto-contouring of radiation therapy target volume. Our goal was to model the delineation of target volume as a clinical decision-making problem, resolved by leveraging large language model-aided multimodal learning approaches. METHODS AND MATERIALS: A vision-language model, termed Medformer, has been developed, employing the hierarchical vision transformer as its backbone and incorporating large language models to extract text-ri

The Effects of High-Dose Probiotic Supplementation on Immune Activation and Neurocognitive Disorders in People Living with HIV Undergoing Successful Antiretroviral Treatment: The Procog Study.

BACKGROUND: The prevalence of HIV-associated neurocognitive disorders (HAND) remains high despite antiretroviral treatment (ART). Changes in gut microbiota and persistent immune activation have been suggested as possible causes, while the role of probiotic supplementation remains controversial. METHODS: We included subjects with mild HAND and successful ART. They were randomized to receive either 6 months of high-dose probiotic supplementation or to continue with only ART. Immune activation markers and neuropsychological testing were performed at baseline and the end of follow-up. Neuropsycho

Neurodevelopment in early treated HIV-infected infants participating in a developmental stimulation programme compared with controls.

BACKGROUND: Neurodevelopmental stimulation programmes can improve developmental outcomes. Antiretroviral therapy (ART) started soon after birth potentially limits the invasion of HIV into the central nervous system. A combination of developmental stimulation and early ART initiation may reduce developmental delays in children with perinatally acquired HIV infection. METHODS: At a single site in Johannesburg, South Africa, we enrolled 36 HIV-infected neonates on ART into an intervention group (IG) participating in a yearlong home-based, neurodevelopmental stimulation programme. Efavirenz or nevirapine in three-drug combination therapy with two nucleoside-reverse transcriptase inhibitors for initial treatment of HIV infection in antiretroviral-naïve individuals.

BACKGROUND: The advent of highly active antiretroviral therapy (HAART) has reduced the morbidity and mortality due to HIV. The World Health Organisation (WHO) antiretroviral treatment (ART) guidelines focus on three classes of antiretroviral drugs, namely: nucleoside/nucleotide reverse transcriptase inhibitors (NRTI), non-nucleoside reverse transcriptase inhibitors (NNRTI) and protease inhibitors (PI). Two of the most common medications given in first-line treatment are the NNRTIs, efavirenz (EFV) and nevirapine (NVP). It is unclear which NNRTI is more efficacious for initial therapy. OBJECTI

Structural brain differences in school-aged children who are HIV-exposed uninfected.

BACKGROUND: Antiretroviral therapy (ART) has dramatically reduced perinatal HIV transmission, leading to a growing population of children who are HIV-exposed but uninfected (CHEU). While the neuroanatomic developmental impacts of in utero HIV and ART exposure have been studied in young children, long-term effects on school-aged children are poorly understood, prompting this investigation. METHODS: Fifty-eight CHEU and 38 children who are HIV-unexposed, uninfected (CHUU), 6-12 years old, were recruited through hospitals and community groups in Ontario, Canada. From T1-weighted magnetic resonan

VIGIL: Vision-Language Guided Multiple Instance Learning Framework for Ulcerative Colitis Histological Healing Prediction.

OBJECTIVE: Ulcerative colitis (UC) is characterized by chronic inflammation with alternating remission-relapse cycles, necessitating precise histological healing (HH) evaluation to improve clinical outcomes. To address the limitations of annotation-intensive deep learning methods and suboptimal multi-instance learning (MIL) in HH prediction, we propose VIGIL, the first vision-language-guided MIL framework that integrates white-light endoscopy (WLE) and endocytoscopy (EC). METHODS: VIGIL introduces a dual-branch MIL module, KS-MIL, based on top-K typical frame selection and similarity-weighted

Multiple perspectives and dialogue in understanding experiences of living with eating disorders: Two narratives-four unpackings.

BACKGROUND: This is a response to Conti et al.'s article, "Listening in the dark: why we need stories of people living with severe and enduring anorexia nervosa" (published in JED, 2016), and its call for relational metaphors and a relational approach to supplement the traditional medical/psychological diagnostic language used to describe the life experiences and complex emotions of people affected by an eating disorder. METHODS: Four authors with different backgrounds unpack two narratives, 'The Prima Donna with the Green Dress' and 'Breaking down the Wall', both narrated during fieldwork in

Think Before You Move: Latent Motion Reasoning for Text-to-Motion Generation.

Current state-of-the-art paradigms predominantly treat Text-to-Motion (T2M) generation as a direct translation problem, mapping symbolic language directly to continuous poses. While effective for simple actions, this "System 1" approach faces a fundamental theoretical bottleneck we identify as the Semantic-Kinematic Impedance Mismatch: the inherent difficulty of grounding semantically dense, discrete linguistic intent into kinematically dense, high-frequency motion data in a single shot. In this paper, we argue that the solution lies in an architectural shift towards Latent System 2 Reasoning.

Quantifying improvement of psychotic symptoms in clozapine-treated schizophrenia: clinical note analysis with large language models.

Symptoms of schizophrenia are often reflected in patients' speech. Natural language processing (NLP) approaches enable quantitative assessment of language-related symptoms in schizophrenia. Previous applications have primarily focused on acute psychopathology or predicting the onset or relapse of psychosis rather than treatment-related improvements. Although electronic health records (EHRs) contain rich longitudinal data, unstructured notes hinder structured quantifications. We applied recent large language models (LLMs) to evaluate symptoms based on speech content recorded in EHRs. We analyze

LaVIDE: Language-Prompted Satellite Change Detection via Map-Image Alignment.

Remote sensing change detection based on a map reference and an up-to-date image boosts timely observation of the Earth's surface when earlier images are lacking for comparison. However, the semantic gap between high-level map categories and low-level image details hinders the extraction of homogeneous features for robust temporal association in change detection. Unlike conventional approaches that either compare pixel-level visual similarity or propagate segmentation errors, we propose a novel framework, Language-VIsion Discriminator for dEtecting changes, LaVIDE, which bridges the semantic g
