Severe laryngeal fracture treated by supracricoid laryngectomy.

OBJECTIVE: To report a rare case of severe laryngeal fracture treated by supracricoid laryngectomy. Previously, a few cases of major laryngeal trauma treated by reconstructive laryngectomy have been briefly described. This paper aims to comprehensively document a rare case of severe laryngeal fracture for which this difficult treatment choice represented an acceptable option. METHODS: A 33-year-old woman sustained very serious blunt laryngeal trauma. The complexity of the laryngeal injuries led us to opt for supracricoid laryngectomy, rather than to attempt laryngeal repair. RESULTS: The pos

Videolaryngostroboscopy following frontolateral laryngectomy with sternohyoid flap.

We conducted a retrospective study to analyze the videostroboscopic findings in a group of 21 patients who underwent vertical frontolateral laryngectomy with reconstruction by use of the sternohyoid muscle flap. They had T1b and T2 glottic tumors and a follow-up of more than 1 year. Three observers analyzed the recorded videostroboscopic data. The cases in which the stroboscopic evaluation of the vocal folds was incomplete were studied in regard to the clinical staging, the supraglottic hyperfunction, and the vibratory site; the Fisher exact test was applied. The glottic closure was complete i

[Phonation assessment of voice restoration after laryngectomy].

OBJECTIVE: To take an objective phonation assessment for the patients who have taken the tracheo-esophageal (TE) shunt procedure after total laryngectomy. METHOD: Twenty-four patients after TE shunt procedure were selected as TE group. Esophageal speakers after total laryngectomy and normal subjects were considered as controls. All the subjects of three groups took part in the voice acoustic assessment and MPT measurement. Twenty TE speakers took part in intra-tracheal pressure measurement. TE speakers and esophageal speakers participated in the electronic laryngoscope examination. RESULT: T

Pulmonary function pre- and post-total laryngectomy.

Pulmonary function pre- and post-total laryngectomy was assessed in 16 patients without overt inspiratory stridor preoperatively. It could be shown that total laryngectomy is associated with a significant decrease in vital capacity and in an increase in inspiratory flow-volume values 9 days postoperatively. The inspiratory values tended to increase slightly over the next half year. The measured expiratory flow-volume values were preoperatively already significantly lower than the predicted values, and no significant changes could be observed immediately after the operation nor after 6 months.

Partial vertical laryngectomy for recurrent glottic carcinoma.

Partial vertical laryngectomy for recurrent glottic carcinoma was performed in 61 patients according to stringent criteria. The great majority of the recurrent tumours appeared within 2 years of radiotherapy (80%). The mean follow-up after surgery was 79 months. At 5 years 85% of the patients were free of local recurrence. Nine patients (15%) developed a local recurrence; eight of them underwent total laryngectomy; one patient refused the operation and died. Seven patients died of other causes. The actuarial overall survival rate was 88% at 5 years. Post-operative complications were seen in 12

Prognosis of recurrent laryngeal carcinoma after laryngectomy.

BACKGROUND: Recurrence is common after total laryngectomy for advanced laryngeal carcinoma. The aim of the present study was to review the prognosis of recurrent laryngeal carcinoma after total laryngectomy. METHODS: The records of 165 patients who developed recurrences after total laryngectomy for laryngeal squamous cell carcinoma between January 1971 and December 1990 were reviewed. RESULTS: Of the 165 patients who developed recurrences, 34 (21%) patients had surgical salvage. The sites of recurrence of these 34 operable patients included 11 pharyngeal, 3 tracheostomal, 15 nodal, 2 pharyng

Functional restoration after subtotal glossectomy and laryngectomy.

Extensive resection of carcinoma that involves the tongue base and supraglottic larynx is accompanied by significant potential morbidity and mortality. This is often indicated by poor rates of cure and the limited palliation afforded by radiotherapy alone. Removal of a significant portion of the posterior tongue frequently results in intractable aspiration. Techniques in reconstruction of the oropharyngeal defect and tongue base have included primary closure, random flaps, and myocutaneous flaps. Each of these techniques has been successful, to some degree, in resurfacing pharyngeal defects. H

The neck after total laryngectomy: CT study.

Computed tomographic scans in 23 patients who had undergone total laryngectomy were analyzed retrospectively to determine normal postoperative appearance and to evaluate the role of CT in assessing recurrent neoplasm. Nine patients without clinical evidence of recurrence illustrated the normal postoperative changes: a round or ovoid neopharynx connecting the base of the tongue with the cervical esophagus and intact fat planes surrounding the neopharynx, neurovascular bundles, and sternocleidomastoid muscles. In the 12 patients with recurrent neoplasm, the CT manifestations included masses invo

[Treatment results following supraglottic laryngectomy in carcinoma of the oropharynx, base of the tongue and hypopharynx. 1. Indication limits of supraglottic laryngectomy].

The indications are exactly fixed and critically screened in 110 cases of supraglottic laryngectomy. Comparison with more radical surgical methods yields the same results with functional advantages of supraglottic laryngectomy. The limitations of resection are: the foramen coecum at the base of tongue, the arytenoids and the vocal cord at the entrance of the larynx, the piriform recessus in cases of extension to the hypopharynx.

Silicone adhesives and skin health in laryngectomy care.

Peristomal skin complications are common in patients following a laryngectomy because of the repeated use of devices requiring medical adhesive. Medical adhesive-related skin injury (MARSI) is increasingly recognised as a preventable source of pain, infection risk and reduced quality of life. Laryngectomy patients are particularly vulnerable owing to age-related skin changes, radiotherapy, malnutrition and prolonged moisture exposure. Historically, MARSI has been accepted as an unwelcome side effect of care in this patient group. This article explores the pathophysiology of MARSI, risk factors

Tracheoesophageal prostheses in voice rehabilitation after total laryngectomy.

Loss of voice in patients after total laryngectomy is a significant issue that medicine has been grappling with since the 19th century. Scientific literature reports unequivocally confirm the substantial impact of total laryngectomy on patients' quality of life, which directly translates into their psychological well-being. The purpose of this study is to compile and organize the latest scientific findings on tracheoesophageal prostheses and subsequently integrate the description of this treatment modality with its rehabilitative aspects. In light of the growing number of patients using TEP, t

Total laryngectomy and readmission: causes, rates and predictors.

BACKGROUND: Total laryngectomy (TL) is a complex procedure, and patients undergoing TL are at high risk for readmission, which exposes them to hospital-acquired complications. Readmission rate is a metric for quality of care. We aimed to identify the rate, causes, and predictors of hospital readmission within 60 days after discharge following TL. METHODS: This is a 12-year retrospective study where we included all patients undergoing TL in a single tertiary care center between 2008 and 2022. Patient charts were reviewed for demographics, comorbidities, and causes for readmission. RESULTS: Of
