Rectal foreign body of a cosmetic bottle treated successfully by transanal retrieval: A case report.

RATIONALE: Retained rectal foreign bodies (RFBs) are unusual clinical presentations whose management is challenging for emergency physicians owing to variations in the object types, anorectal anatomy, sacral curvature, insertion times, and local contamination. Here, we present the diagnosis and treatment in 1 case of retained rectal foreign body. PATIENT CONCERNS: A 62-year-old male presented to the emergency department with a cosmetic bottle inserted into the rectum while bathing. He had difficulty defecating and denied any underlying psychiatric illness. Before coming to the hospital, the p

The Effect of Sphincter Pressure and Anal Length on Surgical Decision in Chronic Anal Fissure.

Background/Objectives: Chronic anal fissure, an anorectal condition, is characterized by internal anal sphincter fibers at the base of the fissure. The lifetime prevalence is 7-8%. Medical management and lifestyle modifications are the first-line treatments, whereas chemical or surgical internal anal sphincterectomy is an option for refractory cases. The aim of this study was to evaluate the impact of surgical anal canal length and sphincter pressure on postoperative symptom relief and continence outcomes. Methods: A total of 26 patients diagnosed with chronic anal fissures were selected for s

Anal manometry in patients with fissure-in-ano before and after internal sphincterotomy.

The motility pattern of the internal anal sphincter was estimated manometrically in 42 patients with fissure-in-ano before and after left lateral internal sphincterotomy (LAS). Resting anal pressure and anal pressure during straining were significantly higher in patients than in controls (132 +/- 21 SD cmH2O vs 81 +/- 14 SD cmH2O P < 0.0002 and 46 +/- 16 SD cmH2O vs 13 +/- 4 SD cmH2O P < 0.0005), but were normal after LAS. Slow waves were more common in fissure patients (86 +/- 6 SD% of total recording time vs 68 +/- 11 SD% of total recording time, P < 0.0002), but also became normal after suc

[Chronic anal fissure-Cremes or surgery?].

An anal fissure is a painful lesion of the anal canal. The acute form often heals spontaneously and should be treated conservatively. The treatment of chronic anal fissures is the subject of many studies but the treatment recommendations of international guidelines are contradictory. Conservative treatment consists of chemical sphincterotomy using sphincter-relaxing drugs, such as calcium channel antagonists, nitrates or botulinum toxin and leads to healing in only about 50% of patients with a prolonged healing time and a high rate of recurrence. In contrast surgical treatment is much more eff

Simple in-office sphincterotomy with partial fissurectomy for chronic anal fissure.

Chronic anal fissure is an extremely common problem of the anorectum. It is often confused with symptomatic hemorrhoids and is frequently missed by the examining physician. Symptoms are most commonly pain and bleeding after defecation. A simple office procedure involving partial lateral internal sphincterotomy with or without excision of a sentinel tag has been used over the past five years with extremely satisfactory results. This procedure was used upon 86 patients. Anatomic and symptomatic relief was obtained in 96.4 per cent of the patients. Due to diagnostic related groups and attempts at

Idiopathic hypertensive anal canal: a place of internal sphincterotomy.

BACKGROUND: Hypertensive anal canal is frequently known to be associated with the presence of anal fissure. Based on clinical experience, we hypothesized that idiopathic anal sphincter hypertonia was a condition equivalent to anal fissure, and therefore, it could be treated the same way. PATIENT AND METHODS: Sixty-three patients complaining of anal pain without any anal pathology and ten healthy volunteers were examined. All patients underwent clinical evaluation, neurological examination, anorectal manometry, and measurement of pudendal nerve terminal motor latency. All patients with hyperte

[Surgical therapy of chronic anal fissure--do additional proctologic operations impair continence?].

78 patients with chronic anal fissures have been mainly operated on by lateral internal sphincterotomy (LATS). Continence have been evaluated by questionnaire at least 9 months postoperatively. Patient without any additional proctological operation had minor disturbances of continence in 17%. Patient with additional operations had disturbances of continence in 30%. Especially the subgroup of patients with LATS and haemorrhoidectomy had bad results. In this group only 45% were fully continent.

Anal Fissure.

Anal fissure (fissure-in-ano) is a very common anorectal condition. The exact etiology of this condition is debated; however, there is a clear association with elevated internal anal sphincter pressures. Though hard bowel movements are implicated in fissure etiology, they are not universally present in patients with anal fissures. Half of all patients with fissures heal with nonoperative management such as high fiber diet, sitz baths, and pharmacological agents. When nonoperative management fails, surgical treatment with lateral internal sphincterotomy has a high success rate. In this chapter,

Sequelae of internal sphincterotomy for chronic fissure in ano.

A total of 1355 patients underwent internal sphincterotomy for chronic fissure in ano between 1980 and 1985. Surgical data were obtained for 1102 patients, and 829 patients responded to a questionnaire. Of the 1057 for whom the time of healing was recorded, 1033 (97.7 per cent) healed by a mean time of 5.6 weeks. No significant differences in satisfaction with the outcome or in deficits in continence were noted between groups undergoing lateral, bilateral or posterior midline sphincterotomy. Excision of the fissure was found to be unnecessary. According to responses on the questionnaires, defi

Long-term results of open and closed sphincterotomy for anal fissure.

Three hundred fifty patients who underwent open or closed lateral internal anal sphincterotomy for acute or chronic anal fissure between January 1981 and June 1985 were reviewed. Minimum follow-up was 14 months (mean, 37 months). No patient underwent an additional procedure at the time of sphincterotomy. Twenty-one failed to heal or developed a recurrence in the interval (6 percent). Five of these individuals were found subsequently to have Crohn's disease. Excluding these patients, the incidence of nonhealing was 4.6 percent. Eight patients (2.3 percent) developed postoperative infections req

Decoding the Risks of Fecal Incontinence: A Comprehensive Analysis of Closed Internal Sphincterotomy in Chronic Anal Fissure.

Introduction An anal fissure is marked by a longitudinal tear in the mucosal lining of the lower anal canal, causing painful defecation and mild anal bleeding. The classical triad includes an anal ulcer, a sentinel tag, and a hypertrophic papilla. This study investigates the frequency of fecal incontinence in patients with anal fissure undergoing closed internal sphincterotomy, offering recent insights for treatment recommendations. Objective To determine the prevalence of fecal incontinence in individuals with chronic anal fissure undergoing closed internal sphincterotomy. Methodology The stu

[Fistulas and fissures. Part II: fissures].

Fissures in ano are linear ulcers situated in the anal canal and extending from the dentate line to the margin of the anus. They cause pain and spasms. Diagnosis is made by the history alone. Local medical treatment might consist of topical 0.4% glycerol trinitrate or 2% calcium blocker. In case of therapy resistance, botulinum toxin injection into the internal sphincter is an effective but expensive alternative with encouraging results. If medical treatment fails, then operation has to be recommended. As lateral internal sphincterotomy represents poses a clear danger to continence, fissurecto

Fissurectomy combined with botulinum toxin A injection for medically resistant chronic anal fissures.

INTRODUCTION: Chemical sphincterotomy, the use of pharmacological agents to reduce anal sphincter resting pressure, has become more and more popular in the treatment of chronic anal fissures (CAFs). It offers the possibility to avoid a lateral internal sphincterotomy and its associated risk of incontinence. In our hospital, patient with a chronic anal fissure are consecutively treated with isosorbide dinitrate 1% ointment, applied 6 times a day for 8 weeks, followed by diltiazem 2% ointment, applied 2 times a day for 8 weeks and Botulin Toxin A injections (Dysport; Ipsen, Hoofddorp, the Nether

Neuromyogenic properties of the internal anal sphincter: therapeutic rationale for anal fissures.

Lateral sphincterotomy diminishes internal anal sphincter hypertonia and thereby reduces anal canal pressure. This improves anal mucosal blood flow and promotes the healing of anal fissures. However, sphincterotomy can be associated with long term disturbances of sphincter function. The optimal treatment for an anal fissure is to induce a temporary reduction of anal canal resting pressure to allow healing of the fissure without permanently disrupting normal sphincter function. Broader understanding of the intrinsic mechanisms controlling smooth muscle contraction has allowed pharmacological ma

Anal fissures and fistulas.

Thorough understanding of the pathophysiology of anal fissures and fistulas is essential for the physician treating these conditions. An acute fissure often heals in a month with conservative management; long-standing disease is relieved by lateral internal sphincterotomy. An acute fistulous abscess must be drained by unroofing the abscess rather than by simple incision, which fosters recurrence. Chronic fistula in ano requires fistulotomy, which can be done with a local anesthetic. Fistulas with multiple external orifices should be treated by a specialist in rectal surgery.

Safety and effectiveness of saving sphincter procedure in the treatment of chronic anal fissure in female patients.

INTRODUCTION: Lateral internal sphincterotomy (LIS) is still the approach of choice for the treatment of chronic anal fissure (CAF) regardless to the internal anal sphincter tone but it is burdened by high risk post-operative faecal incontinence (FI). In female patient there are some anatomical and functional differences of the sphinteric system which make them more at risk of FI and vaginal birth could cause sphinteric lesions affecting the anal continence function. The aim of our study is to evaluate the results of saving sphincter procedure as treatment for female patients affected by CAF.

The internal sphincter and anal fissure.

The activity of the internal anal sphincter in patients with fissure has been studied by measuring anal pressure and motility with a small balloon probe. The mean maximum anal pressure in 12 patients with fissure (116-8 +/- 21-8, s.d., cm H2O) was significantly higher than that in 40 control subjects (85-0 +/- 20-5 cm H2O; P less than 0-01). Ultra-slow pressure waves were present in 80 per cent of the patients and in 5 per cent of the controls. Both the high pressure of ultra-slow waves were due to abnormal activity of the internal sphincter. This could be corrected equally well by either dila
