Improving sexual health education for young people with learning disabilities.

Young people with special needs have the same rights as other young people to receive information and education about sexual health issues and to access services. Despite national guidance stating that schools have a duty to ensure that all young people are properly included in sex and relationship education, the provision of such health education to this client group remains inadequate. Comprehensive personal, social and sexual education programmes are needed to enable young people to clarify their values and attitudes, develop self-esteem, self-awareness, recognise and resist peer pressure,

Evidence-based assessment of learning disabilities in children and adolescents.

The reliability and validity of 4 approaches to the assessment of children and adolescents with learning disabilities (LD) are reviewed, including models based on (a) aptitude-achievement discrepancies, (b) low achievement, (c) intra-individual differences, and (d) response to intervention (RTI). We identify serious psychometric problems that affect the reliability of models based on aptitude-achievement discrepancies and low achievement. There are also significant validity problems for models based on aptitude-achievement discrepancies and intra-individual differences. Models that incorporate

Alternative models of learning disabilities identification: considerations and initial conclusions.

The final session of the National Research Center on Learning Disabilities (NRCLD) Responsiveness-to-Intervention (RTI) Symposium, "What are alternative models to LD identification other than RTI?" included four papers that discussed concerns over the exclusive reliance on an RTI approach to learning disability identification, considerations for analyzing proposed LD identification models, and various alternatives to LD identification. The work of the participating panelists is summarized in this discussant paper, and next steps for the NRCLD in light of these presentations are suggested.

Learning disabilities identification: primary intervention, secondary intervention, and then what?

A broad consensus has been achieved regarding the importance of early primary and secondary interventions for children in academic domains for the purposes of improving overall academic competencies and preventing low achievement that often leads to a diagnosis of specific learning disability (SLD) and long-term special education placement. The characteristics of effective prevention programs generally are well established. The degree to which these programs prevent SLD is uncertain, and the subsequent procedures for determining SLD eligibility are very much at issue. Issues are discussed rega

Developmental Gerstmann's syndrome: a distinct clinical entity of learning disabilities.

The symptom complex of finger anomia, right-left disorientation, dysgraphia, and dyscalculia constitutes Gerstmann's syndrome. It is mostly described in adults and is caused by acquired lesions of the dominant parietal lobe. It is infrequently described in children with learning disabilities and has been designated developmental Gerstmann's syndrome. Developmental Gerstmann's syndrome goes unnoticed if not specifically sought by clinicians. A detailed evaluation will reveal subtle neurologic deficits, behavioral problems, and neuropsychologic and specific speech and language abnormalities. Ten

Preventing rape and sexual assault of people with learning disabilities.

This article will explore the increased prevalence of rape and sexual assault among people with a learning disability and identify factors that exist which increase this client group's vulnerability to becoming a victim of sexual abuse. There is significant under-reporting of such incidents and very few cases end up in prosecution. Within the nursing profession, there is much more that can be done to help decrease the level of vulnerability of people with a learning disability and to work with other agencies to advocate for legal justice and therapeutic redress for individuals.

Epilepsy and learning disabilities--a challenge for the next millennium?

People with learning disabilities often have seizures in addition to other disorders. Precise diagnosis may be difficult, but accuracy can be improved using electroencephalographic and video investigations. Following the establishment of a diagnosis of epilepsy, individually tailored care is necessary taking into account other health, behavioural and therapeutic issues. Neuroimaging may indicate a need for surgery which should not be automatically excluded as a treatment option. Rational antiepileptic drug use is advised, with emphasis upon the newer agents due to their better tolerance and ea

Collaborative speech and language services for students with learning disabilities.

Fueled by educational reforms such as the Regular Education Initiative, the inclusion movement, and Goals 2000, speech and language pathologists (SLPs) have explored the use of collaborative consultation in providing integrated service delivery. The implications of classroom-based services are discussed, along with models that have been adopted by SLPs, learning disabilities specialists (LDSs), and classroom teachers. The characteristics of students served, and the areas of speech and language (i.e., language, articulation, fluency, voice) targeted in the classroom, are reviewed. Ways in which

The prevalence and costs of psychiatric disorders and learning disabilities.

BACKGROUND: The economic burden of psychiatric disorders and learning disability is assessed in order to aid decisions on priorities for research funding. METHOD: A wide variety of data sources both on prevalence and on the usage and costs of relevant services were used to measure the economic burden of each condition. RESULTS: Despite methodological problems and problems with the data, an attempt is made to estimate the relative economic burden imposed by each condition. No attempt is made to sum up the costs for each condition across the agencies and individuals involved. CONCLUSIONS: Our

Defining and classifying learning disabilities and attention-deficit/hyperactivity disorder.

This paper provides an overview of current conceptualizations of learning disabilities and ADHD, a conceptual framework critical for defining and classifying each disorder and for distinguishing each disorder from the other and from other, less common problems of childhood. At a most basic level, reading disability or dyslexia (the most common and best defined of the learning disabilities) and ADHD represent two distinct disorders that may frequently cooccur in the same unfortunate child but that can be clearly distinguished from one another. Reading disability represents a disorder of cogniti

Epistemology and learning disabilities: invited commentary on Hastings and Remington.

The effect on service provision of describing a variety of actions as challenging behaviour is discussed: it is suggested that studying stereotyped, aggressive and self-injurious acts in their own right has yielded more useful psychological debate, and ignoring such conceptual thinking leads to implicit theorizing where assumptions go unquestioned. Evidence for the relevance of staff actions on different topographies of client responses is critically reviewed, alongside discussion of the authors' rhetoric. The importance of adjusting language and frameworks to make room for alternative concept

Traditional IQ is irrelevant to learning disabilities--intelligence is not.

This investigation examined the relationship between intelligence and phonological coding when ability was redefined according to the Planning, Attention, Simultaneous, Successive (PASS) cognitive processing model. This study directly tested the hypothesis that traditional IQ tests may be irrelevant to the definition of learning disabilities, but a different theory of intelligence may be more sensitive to deficiencies related to reading failure. Normally achieving students (n = 30) and students with reading disabilities (n = 30) ranging in age from 7.7 years to 15.3 years (mean = 11.9 years, S

Vigabatrin in adults with poorly-controlled epilepsy and learning disabilities.

Twenty-two adult patients with uncontrolled epilepsy and severe learning difficulties were included in an open study of vigabatrin. Patients were all in residential care and had experienced at least 12 seizures during the previous 12 months despite all attempts to optimize antiepileptic drug (AED) treatment. Following a 4 month baseline period, vigabatrin 500 mg twice daily was added to the current AED treatment and the dose increased according to response, up to a maximum of 4 g/day. Ten patients achieved a reduction in seizure frequency of more than 50% during this 4 month dose titration pha

Instructional and curricular requisites of mainstreamed students with learning disabilities.

The successful integration of students with learning disabilities into mainstream reading classes may require teachers to adapt instruction and curricula in major ways. In this article, we discuss the relation between the instruction prescribed by standardized commercial curricula and the reading achievement of mainstreamed students with learning disabilities and other low-performing nonhandicapped students. An instructional template designed to complement commercially published reading lessons is described and its effects on the reading achievement of mainstreamed students with learning disab

Tinted lenses and related therapies for learning disabilities--a review.

Research relating to the use of Irlen tinted lenses and coloured overlays for underachievers is reviewed. Many of the studies were not published in refereed journals and were methodologically poor. The weaknesses of the Irlen argument are discussed, including the absence of evidence to support the claims that these tints need to be uniquely prescribed and manufactured. Syntonics is another form of visual colour therapy that has been applied to those with a learning disability. Research on this is reviewed, and is also shown to have procedural irregularities which preclude firm conclusions. Owi

Genetic influences on learning disabilities and speech and language disorders.

This paper is a comprehensive review of known examples of genetically influenced learning disabilities (LDs) and speech and language disorders (SLDs). The review is divided between 2 broad classes of studies: (a) those which begin with an LD or SLD phenotype that appears to be familial and attempt to learn more about the specifics of genetic transmission, if any; and (b) those which begin with a group of individuals, all of whom share a given documented genetic risk factor, to see if it leads to a specific LD or SLD. Included in the first category are familial dyslexia, stuttering, and other s
