Traumatic Superficial Femoral Arteriovenous Fistula with Pulsatile Mass and Leg Pain 60 Years after Stabbing Injury.

Post-traumatic arteriovenous fistula (AVF) is a vascular injury complication and can present with vessel dilation, forming pulsatile varices, venous hypertension, distal ischemia, and congestive heart failure. We present a case of only pulsatile mass and leg pain caused by a 60-year-old post-traumatic AVF. Computed tomography angiography showed an AVF between the superficial femoral artery and superficial femoral vein. Surgical repair with AVF ligation was successfully performed. Traumatic AVF caused vascular and heart failure in the future; therefore, post-traumatic AVF is better eliminated a

An intracortical neuroprosthesis immediately alleviates walking deficits and improves recovery of leg control after spinal cord injury.

Most rehabilitation interventions after spinal cord injury (SCI) only target the sublesional spinal networks, peripheral nerves, and muscles. However, mammalian locomotion is not a mere act of rhythmic pattern generation. Recovery of cortical control is essential for voluntary movement and modulation of gait. We developed an intracortical neuroprosthetic intervention to SCI, with the goal to condition cortical locomotor control. Neurostimulation delivered in phase coherence with ongoing locomotion immediately alleviated primary SCI deficits, such as leg dragging, in rats with incomplete SCI. C

A Rare Case of Dynamic Popliteal Artery Occlusion After Gunshot Injury with Reconstitution of Flow in the Frog-leg Position.

A 16-year-old male was transferred to our institution shortly after a gunshot injury to the right lower extremity. Physical examination was remarkable for two bullet entry points in the right posterior leg. A right lower extremity computed tomography angiogram (CTA) demonstrated a retained bullet fragment in the right popliteal fossa and a 10 centimeter (cm) in length occlusion of the proximal peroneal artery with reconstitution of flow distally. A diagnostic angiogram of the right lower extremity with the patient's leg extended demonstrated lack of popliteal arterial flow immediately distal t

Optimized lower leg injury probability curves from postmortem human subject tests under axial impacts.

OBJECTIVE: Derive optimum injury probability curves to describe human tolerance of the lower leg using parametric survival analysis. METHODS: The study reexamined lower leg postmortem human subjects (PMHS) data from a large group of specimens. Briefly, axial loading experiments were conducted by impacting the plantar surface of the foot. Both injury and noninjury tests were included in the testing process. They were identified by pre- and posttest radiographic images and detailed dissection following the impact test. Fractures included injuries to the calcaneus and distal tibia-fibula complex

Arm and leg coordination during treadmill walking in individuals with motor incomplete spinal cord injury: a preliminary study.

Arm and leg coordination naturally emerges during walking, but can be affected by stroke or Parkinson's disease. The purpose of this preliminary study was to characterize arm and leg coordination during treadmill walking at self-selected comfortable walking speeds (CWSs) in individuals using arm swing with motor incomplete spinal cord injury (iSCI). Hip and shoulder angle cycle durations and amplitudes, strength of peak correlations between contralateral hip and shoulder joint angle time series, the time shifts at which these peak correlations occur, and associated variability were quantified.

Leg sympathetic response to noxious skin stimuli is similar in high and low level human spinal cord injury.

OBJECTIVE: To determine if sympathetically mediated vasoconstriction in the lower extremities is injury level dependent. Although sympathetic responses have been measured in the limbs of people with high and low level SCI using blood flow measurements, including Doppler ultrasound and venous plethysmography, a direct comparison between injury levels has not been made. METHODS: Volunteers with chronic SCI were grouped according to injury level. Above T6: high level (HL, n=7), and T6 and below: low level (LL, n=6). All subjects had complete motor and sensory loss. Leg arterial flows were record

Development of a strength test battery for evaluating leg muscle power after anterior cruciate ligament injury and reconstruction.

A more sports-specific and detailed strength assessment has been advocated for patients after anterior cruciate ligament (ACL) injury and reconstruction. The purpose of this study was to develop a test battery of lower extremity strength tests with high ability to discriminate between leg power development on the injured and uninjured sides in patients after ACL injury and in patients who have undergone ACL reconstruction. Twenty-three patients were tested 6 months after ACL injury and 44 patients were tested 6 months after ACL reconstruction. Twenty-four of the 44 patients were operated on us

The Use of a Single Trunk-Mounted Accelerometer to Detect Changes in Center of Mass Motion Linked to Lower-Leg Overuse Injuries: A Prospective Study.

Movement dynamics during running was previously characterized using a trunk-mounted accelerometer, and were associated with a history of overuse injuries. However, it remains unknown if these measures are also linked to the development of overuse injuries. The aim of this study was therefore to determine how movement dynamics alter in response to fatigue, and the possible link with developing lower-leg overuse injuries during a six-month follow-up period. Two hundred and eight movement science university students completed a 12-min all-out run while wearing a trunk-mounted accelerometer. Dynam

Limb salvage using reverse sural cross-leg flap in complex lower limb injury: A case series.

BACKGROUND: Complex lower limb injuries involving soft tissue defects, fractures, and vascular compromise present significant reconstructive challenges. Free tissue transfer is often the gold standard for reconstruction; however, reverse sural cross-leg flaps provide a reliable alternative in resource-limited settings or when the recipient's vessels are compromised. This case series highlights the utility of this technique in challenging scenarios, including comminuted fractures, vascular injuries, and extensive soft tissue defects. METHODS: Six male patients with complex lower limb injuries

Increased movement variability in one-leg hops about 20 years after treatment of anterior cruciate ligament injury.

BACKGROUND: Recent studies highlight the need for understanding movement control of adjacent joints when evaluating knee function following anterior cruciate ligament (ACL) injury. However, while short-term adaptations to lower-extremity joint coupling have been studied, little is known about any potential long-term adaptations in neuromuscular control displayed by ACL-injured individuals. The aim of our study was to determine whether coordination variability of the hip-knee joint couplings during the one-leg hop is altered about 20 years after injury in two ACL-injured groups compared to heal

Anterior cruciate ligament injury about 20 years post-treatment: A kinematic analysis of one-leg hop.

Reduced dynamic knee stability, often evaluated with one-leg hops (OLHs), is reported after anterior cruciate ligament (ACL) injury. This may lead to long-standing altered movement patterns, which are less investigated. 3D kinematics during OLH were explored in 70 persons 23 ± 2 years after ACL injury; 33 were treated with physiotherapy in combination with ACL reconstruction (ACL(R)) and 37 with physiotherapy alone (ACL(PT)). Comparisons were made to 33 matched controls. We analyzed (a) maximal knee joint angles and range of motion (flexion, abduction, rotation); (b) medio-lateral position of

A systematic review of postural control during single-leg stance in patients with untreated anterior cruciate ligament injury.

PURPOSE: The aim of this review was to determine whether postural control is impaired in patients with anterior cruciate ligament (ACL) injury as compared to healthy controls. METHODS: The relevant papers were retrieved through electronic databases including PubMed, EMBASE, Web of Science, and Sport Discus followed by hand search and contact with the authors. Studies that evaluated static postural control during single-leg stance without applying external perturbations were included. Also, the patients should not have undergone ACL reconstruction or any surgical repair on the injured knee. R

[Exertional compartment syndrome of the lower leg and common peroneal nerve palsy as combined injury after weight lifting].

While the compartment syndrome after traumatic injury is a well-known entity and well recognised, an exercise-induced acute compartment syndrome is seldom and often missed. Delayed diagnosis and treatment can result in a poor outcome such as loss of function or limb. Early recognition and prompt fasciotomy are necessary. We would like to present the case of an exertional compartment syndrome of the tibialis anterior space after weight-lifting which was complicated by an acute pressure-induced palsy of the common peroneal nerve due to a self applied knee-support-bandage. A thorough case history
