Our aim was to identify factors predisposing athletes to multiple stress fractures, with the emphasis on biomechanical factors. Our hypothesis was that certain anatomic factors of the ankle are associated with risk of multiple stress fractures of the lower extremities in athletes. Limb salvage was previously described for patients with osteogenic sarcoma who had resectable lesions. In the very young child the usual methods of reconstruction are not applicable due to the resulting major leg-length inequality. One solution is to resect the lesion in accordance with good cancer surgery guidelines and then perform a rotationplasty of the Van Nes type. The ankle then functions as a knee joint, and the final result is a below-knee amputation rather than a high above-knee amputation. Functional results in two patients were remarkably good.

Low back pain is a common complaint of athletes, and accurate diagnosis is essential, since many causes of back problems can lead to disability. If the obvious mechanical causes, such as muscle strain, "kissing spines," and leg-length inequality, have been ruled out, the more serious sources of low back pain, such as disk herniation, Scheuermann's disease (swimmer's back), compression fractures of the vertebral body, and stress fractures of the posterior elements, should be considered. Treatment goals for all these disorders include pain relief and prevention of further injury.

BACKGROUND: We present a patient with a subtotal traumatic supramalleolar amputation of the leg, which was initially treated by a vascular reconstruction with deliberate bone and soft-tissue shortening.
METHODS: To correct the ensuing complex deformity, which consisted of a varus hindfoot, leg length discrepancy and equinus, a staged reconstruction was planned. Initially, the hindfoot varus, in presence of a stiff ankle, was corrected by a supramalleolar osteotomy, followed by a Wagner distraction and finally a correction of the equinus.

A case of aesthetic calf augmentation by a microsurgical free deepithelialized VRAM-flap is reported. As a result of poliomyelitis infection in childhood and atrophy of the m. gastrocnemius medialis sinister, the contour of the left calf was deformed. The reconstruction by autologous material was performed by use of the free VRAM-flap. The indications, advantages, and disadvantages of this method compared to calf augmentation by silicon prostheses are discussed.

A one-stage release of circumferential congenital constriction bands was performed in four extremities (three patients). No wound problems occurred, even when there had been marked swelling of the extremity distal to the band. The one-stage release facilitated postoperative care, and there was no need for additional periods of anesthesia or for additional operations, which are necessary when this problem is treated with a release performed in two or three stages.

This paper reports fifteen cases of ball and socket articulation at the ankle followed up for an average of twelve years. All patients showed inequality of leg length. Ten patients showed coalitions of the bones of the hindfoot and nine patients had a reduction in the number of bony elements of the forefoot. Other associated anomalies are described. The abnormality seems to be part of a congenital short-limb malformation, perhaps modified by adaptive change associated with rigidity of the hindfoot. Surgical intervention was not required in any patient in the series.

Congenital diffuse hemangiomatous involvement of the entire left thigh and knee joint in a 12-year-old girl was complicated by a Kasabach-Merrit type of consumptive coagulopathy. The triad of varicose veins, hypertrophy of soft tissue and bone, and cutaneous hemangiomas (The Klippel-Trenaunay syndrome) often results in major malformations. A review of eleven children treated for this condition showed that the overgrowth of the limb was a major problem in management. In three patients the altered hemodynamics resulted in cardiac complications. Venous stripping, excision of the arteriovenous fistulae, and radiation therapy should be avoided.

PURPOSE OF THE STUDY: Physeal distraction or chondrodiastasis uses the growth plate as a zone of least resistance. Application of force thus provokes distraction and opening of the plate (Salter and Harris type I epiphyseal detachment).
