As a synthetic fiber maker, we Teijin Ltd. used polymer chemistry technology to develop an industrial oxygen-enriching membrane. With this membrane, Teijin had made first domestically manufactured membrane-type oxygen concentrator and entered home oxygen therapy market in 1982, which was three years prior to the start of medical insurance reimbursement for home oxygen therapy. Since then, the company has developed an adsorption-type oxygen concentrator, focused on power and noise reduction, and developed both the "Ultressa," a small, lightweight, portable tank used for outside activities and t

INTRODUCTION: Decisions regarding life-sustaining treatment and end-of-life care are among the most ethically complex and emotionally challenging decisions encountered in emergency departments and intensive care units. Such decisions frequently involve uncertainty regarding prognosis, limited time for deliberation and situations in which patients may lack decision-making capacity, requiring family members or surrogate decision-makers to participate in the decision-making process. Shared decision-making (SDM) has increasingly been recommended as an approach to align treatment decisions with pat

The paper analyzes the concept of "Life-Sustaining Treatments" (LST) in the context of Medically Assisted Suicide (MAS) in Italy. The authors highlight how the requirement of LST, established by Constitutional Court Ruling n. 242/2019 along with three other criteria (irreversible pathology, intolerable suffering, decision-making capacity), has often been used to reject requests for access to MAS. Subsequent Constitutional Court rulings (135/2024 and 66/2025) have progressively expanded the definition of LST, including care procedures such as bowel evacuation, urinary catheterization, and mucus

Despite the many advances in treatment methods for gastric cancer, it remains a leading cause of cancer-related mortality worldwide, with treatment outcomes intrinsically linked to the nutritional status of the patient. Malnutrition is a frequent and severe complication in patients with gastric cancer, arising from a confluence of factors including tumor-induced anorexia-cachexia syndrome, mechanical obstruction, and the metabolic stress of anti-cancer therapies including surgery and chemotherapy. Historically, nutritional support has often been a secondary consideration compared with surgical

BACKGROUND: Withdrawal of Life Sustaining Therapies (WOLST) is a critical procedure in up to 50% of inpatient pediatric deaths. No best practice guideline (BPG) exists. We aimed to develop and implement a BPG and standard note template for WOLST procedures.
MEASURES: The smart aim was to increase use of the tools by 20% within 6 months. The measures were the percentage of tools used (process), usefulness of the tool (outcome), and narrative responses to identify unintended consequences (balance).
INTERVENTION: PDSA cycles include engagement of stakeholders to create the tools, BPG distributi

Traumatic Brain Injury (TBI) remains a significant global health concern with significant impact on morbidity and mortality. This narrative review explores adjunctive pharmacologic agents to be employed by emergency medicine clinicians during Advanced Trauma Life Support (ATLS) in patients presenting with a TBI. Pharmacologic agents are commonly employed for the management of rapid sequence intubation and post-intubation analgosedation, hemodynamics, intracranial pressure, coagulopathy, seizure prophylaxis, and infection. This narrative review discusses current evidence and controversies to op

OBJECTIVES: Heart rate (HR) is one of the physiological variables in the early assessment of trauma-related haemorrhagic shock, according to Advanced Trauma Life Support (ATLS). However, its efficiency as predictor of mortality is contradicted by several studies. Furthermore, the linear association between HR and the severity of shock and blood loss presented by ATLS is doubtful. This systematic review aims to update current knowledge on the role of HR in the initial haemodynamic assessment of patients who had a trauma.
DESIGN: This study is a systematic review and meta-regression that follow

The 2025 pediatric advanced life support guidelines, developed by the American Heart Association and the American Academy of Pediatrics, are a resource for healthcare professionals caring for infants and children under 18 years of age in peri-arrest and other emergency care situations. The new guidelines address evolving pediatric cardiac arrest patterns, characterized predominantly by respiratory failure or shock, with persistent disparities in out-of-hospital survival and neurological outcomes. Major revisions include setting a blood pressure target above the 10th centile post-cardiac arrest

OBJECTIVES: The ENd-of-life deciSion-making dURing Extracorporeal life support (ENSURE) survey aimed to assess current clinical practices and perspectives on end-of-life decisions for patients on extracorporeal life support (ECLS) across Europe. Understanding the challenges surrounding ECLS withdrawal may help mitigate moral distress in this complex decision-making process.
DESIGN: The survey covered 20 questions to be assessed online and was composed of two sections: the first dealt with general questions on hospital details, ICU profile, ECLS provision, and participants' profession, and the

If patients on the intensive care unit (ICU) are awake and life-sustaining treatment is suspended because of the patients' request, because of recovering from the disease, or because independence from organ function supportive or replacement therapy outside the ICU can no longer be achieved, these patients can suffer before they inevitably die. In The Netherlands, two scenarios are possible for these patients: (1) deep palliative (terminal) sedation through ongoing administration of barbiturates or benzodiazepines before withdrawal of treatment, or (2) deliberate termination of life (euthanasi

BACKGROUND: A recent publication from Norwegian health authorities describes necessary routines for end-of-life decisions in hospitals. There are no comparable national recommendations regarding patients in nursing homes. 40% of deaths in Norway occur in nursing homes.
METHODS: All nursing home physicians in Bergen received an open questionnaire on the practice of ethics, end-of-life-decisions, and palliative care.
RESULTS: 15 of the nursing homes physicians responded to the questionnaire, representing three quarters of the nursing homes and 1483 out of 1782 nursing home patients in Bergen (

Locked-in syndrome (LIS) is a rare neurologic disorder rendering an individual quadriplegic and anarthric with preserved self-awareness and normal if not near-normal cognition. A lesion to the ventral pons causes the classic form of LIS, and patients can typically interact with their environment with eye/eyelid movements. LIS patients may live for years with preserved quality of life (QoL) and cognitive function, but with severe disability. However, medical providers and family often underestimate the patient's QoL, and choose less aggressive care. Prompt assessment of decisionality in LIS pat

The patient was a 30-year-old woman with right tongue cancer and SCC (T4N2bM0). After it had relapsed, the outpatient palliative care conference intervened. The purpose of intervention was as follows: pain control, nutrition management, home recuperation support, and relief of a spiritual pain. We could achieve the patient's desire to recuperate at home as long as possible by sharing a goal and information. The patient was treated for a total of 457 days. Since the relapse, she was treated for 274 days, and was hospitalized for 49 days. By offering necessary home care supports, the patient and

Guidelines and practices in trauma care constantly evolve based on evidence available, and every healthcare provider who treats trauma should be up-to-date in trauma-care concepts. The Eleventh Edition of Advanced Trauma Life Support, released in 2025, contains a complete revamp of the foundational principles of acute trauma care, content design, delivery, and training, based on medical and educational evidence. In this edition, a significant update is the emphasis on control of exsanguinating/major haemorrhage (ABCDE to x-ABCDE where x stands for control of exsanguinating haemorrhage in traum

Colorectal cancer (CRC) remains a leading cause of cancer morbidity and mortality worldwide, especially in older adults where frailty complicates treatment outcomes. Multimodal prehabilitation-comprising nutritional support, physical exercise, and psychological interventions-has emerged as a promising strategy to enhance patients' resilience before CRC surgery. Clinical studies demonstrate that prehabilitation significantly reduces postoperative complications, shortens hospital stays, and improves functional recovery. Nutritional interventions focus on counteracting malnutrition and sarcopenia

Pulmonary Fibrosis (PF) describes a group of lung diseases characterised by progressive scarring (fibrosis). Symptoms worsen over time and include breathlessness, tiredness, and cough, giving rise to psychological distress. Significant morbidity accompanies PF, so ensuring patients' care needs are well defined and provided for, represents an important treatment strategy. The purpose of this systematic review was to synthesise what is currently known about the psychosocial morbidity, illness experience and needs of people with pulmonary fibrosis and their informal caregivers.
