Pyogenic liver abscesses in a child spreading to pulmonary and subcutaneous tissues: case report.

Pyogenic liver abscess is a rare and life-threatening disease in children. Our case is noteworthy because of the rapid advancement of liver abscesses without any other systemic disorder. A 16-year-old girl was admitted to the hospital with fatigue, pallor, weight loss and high fever. In physical examination a fluctuating mass was observed under the scapular area and hepatosplenomegaly was found. In computed tomography, three septated cystic lesions which looked like abscesses were demonstrated in the liver. The abscess was drained through percutaneous route. Right pleural empyema with clinical

Schistosomiasis mansoni is associated with pyogenic liver abscesses in the state of Minas Gerais, Brazil.

The association between pyogenic liver abscesses and schistosomiasis has been confirmed by clinical and experimental studies. In this retrospective study of 78 patients with pyogenic liver abscesses the association with schistosomiasis has been investigated. Pyodermitis, a known focus of bacteremia, was observed in 19 patients (24%). Blood eosinophilia was observed in 30 patients (39%). Staphylococcus aureus was cultured from abscesses in 17 out of 38 patients (45%). Forty-one out of 57 patients (53%) had stool examination. Schistosoma mansoni was the main parasite identified. Eggs of S. manso

Solitary and multiple pyogenic liver abscesses: characteristics of the patients and efficacy of percutaneous drainage.

OBJECTIVES: Although percutaneous drainage has emerged as one of the first line of therapies for pyogenic liver abscesses, the presence of multiple abscesses may warrant surgical drainage, which remains controversial in the literature. We studied whether the multiplicity of the lesions influences the outcome of the treatment. METHODS: Ultrasonography-guided percutaneous drainage was carried out in 48 patients with pyogenic liver abscesses. The abscesses were solitary in 38 patients and multiple (two to seven lesions) in 10 patients. Clinical characteristics and the efficacy of the treatment w

The antemortem diagnosis of pyogenic liver abscess due to perforation of the gut by a foreign body.

Perforation of the gastrointestinal tract by ingested foreign bodies is rare; the diagnosis of pyogenic liver abscess resulting from such perforations is usually made at post-mortem. We present a case of perforation of the gut, due to an ingested dental plate, with a resultant pyogenic liver abscess, which presented as a pyrexia of unknown origin.

Beyond the Liver: A Unique Case of Pyogenic Liver Abscess Caused by Peptostreptococcus micros Penetrating the Chest Wall.

Liver abscesses are uncommon pyogenic infections with diverse microbiology, often involving enteric gram-negative bacilli such as Escherichia coli and Klebsiella pneumoniae. Standard management includes antibiotic therapy and abscess drainage. We present a case of a 37-year-old male with chronic right upper quadrant abdominal pain, who was found to have an enlarging liver mass infiltrating the chest wall and right-side chest ribs, ultimately diagnosed as a large pyogenic liver abscess (PLA) extending into the chest wall. Notably, the abscess was attributed to Peptostreptococcus micros, a rarel

A Rare Case of Cerebral Abscess due to Fusobacterium nucleatum with Native Aortic Valve Infective Endocarditis and Pyogenic Liver Abscess.

Fusobacterium nucleatum is a commensal pathogen typically found in the oral cavity, digestive tract, and urogenital system which has been associated with Lemierre's syndrome, periodontal diseases, sinusitis, endocarditis, and intra-abdominal and brain abscesses. Our case is of a 62-year-old male who presented with headaches, nausea, and vision loss. Brain imaging identified a right occipito-parietal brain abscess. Following surgery and abscess drainage, Fusobacterium nucleatum was isolated from intraoperative cultures, and the infectious disease service was consulted for antibiotic recommendat

Bacteroides fragilis Empyema Secondary to Ruptured Pyogenic Liver Abscess: A Rare Cause of Septic Shock.

Pyogenic liver abscess is an uncommon but potentially life-threatening intra-abdominal infection associated with significant morbidity and mortality. Although hepatic abscesses most commonly present with systemic infection or intra-abdominal complications, transdiaphragmatic spread into the pleural cavity resulting in empyema is rare. Bacteroides fragilis, an anaerobic gram-negative bacillus that forms part of the normal gastrointestinal flora, is a well-recognized pathogen in polymicrobial intra-abdominal infections but is infrequently reported as a primary pathogen in empyema originating fro

Emergence of hypervirulent Klebsiella pneumoniae ST111 with a novel virulence plasmid causing pyogenic liver abscess.

OBJECTIVE: Hypervirulent Klebsiella pneumoniae (hvKp)-associated pyogenic liver abscess is most commonly linked to K1/K2 capsule serotypes and canonical lineages such as ST23, ST65, and ST86. Reports of hvKp outside these lineages remain rare but carry significant public health implications. We investigated an unusual case of liver abscess in an immunocompetent patient caused by hvKp sequence type 111 (ST111). METHODS: Five isolates (KP1-KP5) recovered from a patient with pyogenic liver abscess were characterized by antimicrobial susceptibility testing, whole-genome sequencing, and pulsed-fie

When Clinical Improvement Does Not Reflect Radiological Response: A Culture-Negative Giant Pyogenic Liver Abscess.

Background: Management of giant pyogenic liver abscesses (PLA) remains challenging, particularly in culture-negative cases, where clinical improvement may not reflect adequate local disease control. Case Description: A 65-year-old woman with well-controlled type 2 diabetes mellitus presented with several weeks of systemic symptoms, marked inflammatory response, cholestatic liver injury, and acute kidney dysfunction. Contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI) revealed a large, multiloculated hepatic lesion measuring approximately 10 cm, consistent with a gia

Pyogenic liver abscess following biliary stent placement in pancreatic cancer patients: a retrospective case series.

Biliary stent placement is widely used in clinical, especially in patients with pancreatic cancer complicated with obstructive jaundice. Pyogenic liver abscess (PLA) is a severe complication following biliary stent placement which predominantly occurs in the right lobe of the liver, with an incidence rate ranging from 4.3% to 13.5% and a mortality rate up to 30%. It is related to the following mechanisms: retrograde bacterial infection; bile stasis and increased bile duct pH; stent-related bile duct injury; biofilm formation; immune system suppression. The main causative pathogens are gram-neg

Successful Multidisciplinary Treatment of Severe Pyogenic Liver Abscess Caused by Fish Bone: A Case Report.

Pyogenic liver abscess (PLA) is a potentially fatal infection that can lead to sepsis and shock. Among the various causes of PLA, ingestion of foreign bodies such as fish bones is relatively rare. Unless there are specific symptoms such as painful swallowing, patients rarely remember having ingested foreign bodies, making it often difficult to identify the cause of PLA. In addition, the treatment strategy and perspectives for PLA caused by foreign bodies are controversial. Herein, we present a successful case of multidisciplinary treatment for sepsis due to a PLA caused by a fish bone with a l

An Atypical Presentation of a Pyogenic Liver Abscess Caused by Fusobacterium nucleatum Post-ruptured Appendix.

A rare formation of a pyogenic liver abscess can be found in patients with a ruptured appendix. Here, we discuss a 49-year-old male with a past medical history significant for drug-induced pancreatitis from metformin, type II diabetes, obesity, and hypertension who presented with right upper quadrant pain. He was diagnosed with a 12.6 cm pyogenic liver abscess. Upon further chart review, the patient had a laparoscopic appendectomy done nine months ago, with the findings of a ruptured appendix. The liver abscess cultures grewFusobacterium nucleatum - a common culprit of acute appendicitis repor

A case of hepatocolic fistula after percutaneous drainage for a gas-containing pyogenic liver abscess.

We describe a rare case of gas-containing pyogenic liver abscess which penetrated the adjacent colon, forming a hepatocolic fistula, after percutaneous transhepatic abscess drainage (PTAD) had been performed. To the best of our knowledge, this is the first report of hepatocolic fistula associated with a gas-forming liver abscess in a diabetic patient, with radiological and surgical confirmation of the fistula.

Association of Biliary Source Klebsiella pneumoniae Pyogenic Liver Abscess With Colon Adenocarcinoma: A Case Report.

BACKGROUND: Pyogenic liver abscesses (PLAs) commonly result from hematogenous spread or biliary tract infections, most often due to cholecystitis or cholangitis. Klebsiella pneumoniae is a recognized pathogen in PLA and has been associated with underlying gastrointestinal malignancies, particularly colorectal cancer. While screening for malignancy is well established in cryptogenic K. pneumoniae liver abscess, its role when a clear biliary source is present is less well described. CASE PRESENTATION: We report a 76-year-old man with a history of untreated hepatitis C and prior renal cell carci

Association of Type 2 Diabetes Mellitus and Risk of Colorectal Cancers in Pyogenic Liver Abscess.

BACKGROUND: Pyogenic liver abscess (PLA) has been reported as a potential clinical marker for colorectal cancer (CRC). Patients with type 2 diabetes mellitus (T2DM) are generally considered at elevated risk for infections and cancers due to impaired immune function. However, the specific relationship between T2DM and CRC among individuals with PLA remains unclear. METHODS: This retrospective cohort study was conducted using the TriNetX Global Collaborative Network. Adult patients (≥20 years) with PLA between 2010 and 2024 were identified and stratified by the presence of T2DM. Patients with a
