Ossification of the posterior longitudinal ligament of the spine (OPLL) is a common musculoskeletal disease among people after middle age. The OPLL presents with serious neurological abnormalities due to compression of the spinal cord and nerve roots. The OPLL is caused by genetic and environment factors; however, its etiology and pathogenesis still remain to be elucidated. To determine the susceptibility loci for OPLL, we performed a genome-wide linkage study using 214 affected sib-pairs of Japanese. In stratification analyses for definite cervical OPLL, we found loci with suggestive linkage

Some histological analyses of the ossification of the posterior longitudinal ligament (OPLL) have been reported, but no ultrastructural studies of the ligamentum flavum (LF) in patients with OPLL have been published to date. To understand the pathology of the ossification of the spinal ligament, we examined, by electron microscopy, ultrastructural changes in the LF in cases of OPLL and made a comparison with the LF in cervical spondylotic myelopathy (CSM). Subjects were three men and two women with cervical OPLL who underwent longitudinal spinous process-splitting laminoplasty. During surgery,

Cyst of the longitudinal posterior ligament of the spine is a new entity described here on the basis of 10 cases. This is a lesion, perhaps of traumatic origin, which occurs in young, athletic subjects suffering from classical lumbosciatica. Computerized tomography showed an image resembling nucleus pulposus herniation. In one of our cases magnetic resonance imaging provided the preoperative diagnosis. Following surgery the outcome was favourable, with less postoperative complications than with discal herniation. The macroscopic and histological findings were the same in all cases, making the

A 72-year-old male noted weakness in the right lower extremity in August 1979, and clumsiness of the finger movement in the right hand in April 1980. In June 1980, he became tetraplegic and was unable to stand or walk. He also developed hypesthesia in the four extremities and difficulty in voiding. On admission to this hospital (July 12, 1980), neurological examination showed spastic tetraparesis, more pronounced on the right side. Deep tendon reflexes were markedly increased in the four extremities. Chaddock reflex in the right lower extremity and Trömner reflex in the both upper extremities

Ossification of the posterior longitudinal spinal ligament (OPLL) was characterized by calcified longitudinal band along the posterior margin of vertebrae, but it has not been possible to know how the cord is compressed within the narrowed spinal canal. Our neuroradiological studies based on CT-view of the 15 cases of OPLL suffering from various degree of myelopathy revealed: 1) Computed tomography precisely delineated shape of OPLL, which was quite polymorphic, like mushroom, irregular cubic and round. OPLL ranged more than two vertebrae was not uniform, but exhibited different configuration

Ossification of the posterior longitudinal ligament (OPLL) is a debilitating spinal disorder characterized by heterotopic bone formation and severe neurological deficits, yet its underlying pathogenic mechanisms remain poorly understood. Here, we report that glycolytic reprogramming orchestrated by the ADAM12/IGF1 axis is a critical driver of OPLL pathogenesis. Through integrated multi-omics analysis of human OPLL tissues, we uncovered a profound metabolic shift featuring enhanced glycolysis and suppressed oxidative phosphorylation as ligament progenitor cells differentiated into osteoblasts.

Ossification of the posterior longitudinal ligament (OPLL) is a chronic degenerative spinal condition in which bone gradually forms within the ligament, compressing the spinal cord and causing progressive neurological decline. While genetic predisposition and metabolic disturbances are well-recognized contributors, growing evidence points to macrophages as central players in shaping the tissue environment that drives pathological bone formation. Macrophages are remarkably plastic cells whose phenotypes and functions are continuously molded by cues from their local tissue environment. While pro

INTRODUCTION: Ossification of the posterior longitudinal ligament (OPLL) is a hyperostotic spinal condition resulting from progressive endochondral ossification of the posterior longitudinal ligament, leading to spinal canal narrowing and spinal cord compression. Primarily seen in East Asian countries, OPLL has been increasingly recognized in North American cohorts. This study aims to evaluate the demographic characteristics and outcomes of patients undergoing surgery for OPLL in a North American cohort.
METHODS: This was a single-center retrospective cohort study of patients with OPLL underg

BACKGROUND: Changes in lower limb joint coordination have been shown to increase localized stress on knee joint soft tissue-a known precursor of osteoarthritis. While 50% of individuals who undergo anterior cruciate ligament reconstruction (ACLR) develop radiographic osteoarthritis, it is unclear how underlying joint coordination during gait changes post-ACLR. The purpose of this study was twofold: to determine differences in lower limb coordination patterns during gait in ACLR individuals 2, 4, and 6 months post-ACLR and to compare the coordination profiles of the ACLR participants at each ti

OBJECTIVE: Dural ossification (DO) is common in patients with ossification of the posterior longitudinal ligament (OPLL). The existence of DO makes surgery challenging and increases the risk of complications. The aim of this study was to investigate the incidence, distribution and radiological characteristics of DO associated with OPLL.
METHODS: From January 2017 to January 2019, 55 patients with cervical OPLL were treated in our single center using an anterior cervical approach microsurgery. Preoperative CT images of decompressed segments were evaluated to identify imaging signs of DO. The '

BackgroundIntervertebral disc calcification (IDC) combined with ossification of the posterior longitudinal ligament (OPLL) in cervical discs is rarely reported. This case study presents a rare case of IDC combined with OPLL in the C2-C3 segment.Case summaryHere, we present a case of a 6-year-old Asian boy with severe neck pain and stiffness. Physical examination showed no neurological or other abnormalities. Digital radiography and computed tomography (CT) revealed a calcified intervertebral disc and OPLL at the C2-C3 vertebrae. The spinal canal compromise at C2-C3 was approximately 50% on mag

PURPOSE: The pathogenesis of ossification of posterior longitudinal ligament (OPLL) is not completely clear. Previous study has confirmed a single-pass type I endoplasmic reticulum (ER) membrane protein kinase (PERK), which is a major transducer of the ER stress, participates in the process of OPLL in vitro. This study aimed to demonstrate the role of ER stress in mechanical stress (MS)-induced OPLL.
METHODS: The posterior longitudinal ligaments were collected intraoperatively. The expression of ER stress markers in ligament tissue samples was compared between OPLL and non-OPLL patients in vi

Cervical spondylotic myelopathy (CSM) is the chronic and slow deterioration of cervical spinal cord function. The pathophysiology of this condition is multifactorial, including compression, repetitive trauma, and vascular compromise of the spinal cord. Clinically, it presents as a progressive decline in patients' appendicular neurological function. The natural history of this disease varies but, it is well-known that the duration and degree of compression correlate negatively with prognosis. A mild degree of CSM tends to stabilize with potential improvement over time while more severe CSM tend

Thoracic ossification of the posterior longitudinal ligament (T‑OPLL) is one of the most common factors that causes thoracic spinal stenosis, resulting in intractable myelopathy and radiculopathy. Our previous study reported that the rs201153092 polymorphism present in the collagen 6A1 (COL6A1) gene was a potentially pathogenic locus for the development of T‑OPLL. The present study aimed to determine whether the rs201153092 mutation causes abnormal expression of COL6A1 in Han Chinese patients with T‑OPLL, and to examine the effects of this mutation on osteogenesis by establishing a model of os
