A rare constellation of empyema, lung abscess, and mediastinal abscess as a complication of endobronchial ultrasound-guided transbronchial needle aspiration.

The introduction of endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) brought about significant advancement in the field of bronchoscopy. The major indications for EBUS-TBNA are lung cancer staging and diagnosis of mediastinal lymphadenopathy. This procedure is minimally invasive and cost saving, and no complications have been described in large-scale studies. In this report, we present a case of empyema, lung abscess, and mediastinal abscess that developed in a patient undergoing EBUS-TBNA; the patient subsequently recovered uneventfully after aggressive surgical de

Successful Staged Surgery for Esophagopulmonary Fistula with Lung Abscess During Neoadjuvant Chemoradiotherapy.

Esophagopulmonary fistula (EPF) with lung abscess is a rare but serious complication during neoadjuvant chemoradiotherapy (nCRT) for esophageal squamous cell carcinoma and is often associated with poor prognosis. We report a 52-year-old man with cT3N3M0 esophageal squamous cell carcinoma who developed fever and hemoptysis during the third week of nCRT. Computed tomography showed a newly developed right lower lobe lung abscess adjacent to the primary tumor, suspicious for esophageal perforation with fistulous communication. Because the lesion and infection appeared localized, staged aggressive

Practice patterns and inpatient outcomes in lung abscess: a cross sectional study.

BACKGROUND: Lung abscess is typically managed with antibiotics, but drainage procedures are sometimes performed when there is inadequate clinical response or when source control is needed. Real-world inpatient practice patterns remain incompletely described in contemporary U.S. data. Our aim was to evaluate national inpatient practice patterns, predictors of procedural intervention, and associated in-hospital outcomes and resource utilization among patients hospitalized with lung abscess managed conservatively, with bronchoscopic drainage, or with percutaneous drainage. METHODS: Using the Nat

Clinical characteristics of apical segment lung abscess: a 10-year retrospective study.

BACKGROUND: Lung abscess in the apical segment of the lung is not rare and is often underestimated in clinical practice. However, the clinical features of apical segment lung abscess (AL) have scarcely been reported. Hence, this study aimed to determine the clinical characteristics of AL and explore moderate therapeutic strategies. METHODS: This was a retrospective, single-center cohort study. We reviewed the medical records of consecutive patients who were admitted to Shanghai Pulmonary Hospital in Shanghai, China, from January 2009 to December 2018. This study collected information on patie

Lung Abscess as Delayed Manifestation of Pulmonary Arterial Narrowing After Sleeve Resection.

A patient who had undergone right upper bilobectomy because of a carcinoid experienced lung abscesses 17 months after operation. After recurrences, despite different antibiotic agents, dual-energy computed tomography showed subtotal stenosis of the right lower lobe pulmonary artery with marked pulmonary perfusion-reduction. Rare causes of lung-abscesses should be considered.

Infected intraparenchymal bronchogenic cyst mimicking recurrent lung abscess in a young adult.

A 23 year old female with a past history of a lung abscess diagnosed at the age of 13 years presented with recurrent episodes of productive cough. Chest radiograph and a high resolution CT scan of the thorax led to the diagnosis of a left lower lobe lung abscess. She underwent a successful thoracotomy and a left lower lobe lobectomy. Histopathological examination revealed the diagnosis of an infected congenital bronchogenic cyst. The recent literature on this is reviewed.

Transdiaphragmatic migration of forgotten gauze sponge: an unreported entity of lung abscess.

Migration of left in foreign bodies like gauze sponge into intestine, stomach, urinary bladder and their extrusion through umbilicus or rectum is well known. We report a unique case wherein a gauze sponge was left in abdominal cavity 25 years back at the time of initial surgery. This patient who had not undergone any thoracic surgery in the past was recently subjected to posterolateral thoracotomy for a right sided lung abscess and a large sponge gauze was unbelievably recovered from the area of lung abscess.

[A case of mediastinal teratoma--differentiation from lung abscess and bronchogenic carcinoma].

A 38-year-old man was admitted with persistent productive cough and right anterior chest pain. Chest X-ray showed two large masses connected with each other, one in the right lung field and the other in the anterior mediastinum. A tentative diagnosis of either lung abscess or bronchogenic carcinoma was initially made, because of elevated serum tumor markers (SLX and SCC) and persisting refractory inflammatory sings. However, open chest drainage revealed a few fine hairs and atheromatous materials within the masses, and the diagnosis of teratoma was made. We removed these masses, and investigat

Streptococcus intermedius lung abscess in children: a case report and literature review.

Streptococcus intermedius is a gram-positive commensal associated with suppurative infections. Pediatric pulmonary abscesses due to S. intermedius are rare, and outcomes in children with severe neurological comorbidities are poorly characterized. A 5-year-old boy with refractory epilepsy and global developmental delay who presented with a right upper lobe abscess, pneumonitis, and pleural effusion. Targeted next-generation sequencing (tNGS) of bronchoalveolar lavage fluid (BALF), drainage pus culture, and tNGS of drainage pus all confirmed the presence of S. intermedius. To our knowledge, this

Surgical Resection of a Refractory Lung Abscess Secondary to Classic Hodgkin Lymphoma.

Lung abscesses pose diagnostic and management challenges, particularly when they are associated with underlying causes, such as lymphoma. We report the case of a previously healthy adolescent with a lung abscess and lymphadenopathy. Despite antibiotics, improvement was minimal, and biopsy results were inconclusive. Surgery revealed an enlarged, adherent lymph node. Pathologic examination confirmed classic Hodgkin lymphoma invading the hilum and causing bronchial and vascular obstruction and secondary abscesses. This case highlights the importance of including lymphoma in the differential diagn

Two Cases of Lung Abscess and Pleuritis in Severe COVID-19 Patients.

We report two patients who were treated with remdesivir, steroids, and tocilizumab for severe coronavirus disease 2019 (COVID-19) and developed lung abscesses and pleuritis. Although complications due to bacterial infections are often reported in COVID-19 patients, these severe infections are rare. Patients receiving tocilizumab are at a high risk of developing serious bacterial infections, and the diagnosis is often delayed because symptoms such as fever and elevated C-reactive protein levels are often minimal. The possibility of complications owing to severe bacterial infections should be co

Disseminated Nocardia Brain Abscess Presenting as Primary Lung Cancer With Brain Metastasis.

We present a challenging case of disseminated Nocardia brasiliensis infection manifesting as brain and skin abscesses. Nocardia is an important potential pathogen to consider in patients with a relevant travel history to endemic regions or atypical presentations, such as brain and skin abscesses. About one-third of patients with Nocardia infections are immunocompetent, and their symptoms are nonspecific. This case shows the limitations of imaging studies in diagnosing Nocardia brain abscesses, as the patient's non-magnetic resonance (MR) conditional pacemaker precluded MRI evaluation and led t

Enterococcus faecalis-associated lung abscess in a male adolescent- a case report.

BACKGROUND: Enterococci are rarely considered pulmonary pathogens; they are usually regarded as colonizers of the airway. The authors present the case of a previously healthy male adolescent, with complaints of fatigue and chest pain, who was diagnosed with Enterococcus faecalis-associated acute primary lung abscess. CASE PRESENTATION: A previously healthy 17-year old boy was admitted to the pediatric ward due to a one-week history of fatigue, inspiratory left side chest pain, dry cough and nasal obstruction. On admission at the emergency department, he was afebrile, with no signs of respirat
