Myocardial Infarction With Non-obstructive Coronary Arteries: A Clinical Conundrum.

Myocardial infarction with non-obstructive coronary arteries (MINOCA) is defined by the presence of positive cardiac biomarkers with clinical evidence of infarction, the absence of significant coronary stenosis (≥50%) on angiography, and the lack of alternative diagnosis for the index presentation. MINOCA poses a diagnostic and therapeutic challenge due to the various pathophysiologic mechanisms underlying its presentation. Coronary artery plaque disruption is recognized as a crucial mechanism contributing to MINOCA. Plaque rupture and thrombus formation with subsequent myocardial ischemia may

[Chest pain after angiography without anomalies].

BACKGROUND: Myocardial infarction can be categorized based on ECG-findings (presence or absence of ST-elevations on STEMI/NSTEMI) or on the results from coronary angiography (CAG) (presence or absence of obstructive atherosclerosis). Myocardial infarction without significant obstruction on CAG (Myocardial Infarction with Non-Obstructive Coronary Arteries/Atherosclerosis; MINOCA) occurs in up to a quarter of patients with myocardial infarcts. These patients have a higher risk of future heart diseases; up to a quarter of MINOCA patients suffer from heart diseases during a median follow-up of fou

A Multimodality Imaging Approach for Diagnosis of Myocardial Infarction with Nonobstructive Coronary Arteries.

Myocardial infarction with nonobstructive coronary arteries (MINOCA) refers to a clinical syndrome where a patient meets the criteria for acute MI, but coronary angiography shows no obstructive coronary artery disease. MINOCA accounts for 6% to 8% of all myocardial infarctions and is more common in non-ST-elevation MI than ST-elevation MI. Up to 60% of cases of MINOCA are seen in women, which disproportionately affects younger and middle-aged women. Patients are typically younger than those with obstructive MI and have fewer traditional risk factors. Formal recognition aids the development of

One-Year Prognosis Difference of Myocardial Infarction With or Without Coronary Obstruction in Developing Countries: Insights From the Moroccan Experience.

INTRODUCTION: The debate remains open as to the difference in prevalence of mortality and occurrence of acute events in patients with Myocardial infarction with non-obstructive coronary arteries (MINOCA) and others with Myocardial infarction with coronary arteries disease (MI-CAD). METHODS: We conducted a 2-year retrospective study for patients admitted for Acute coronary syndrome (ACS) to analyze the clinical and prognostic characteristics of patients with MINOCA versus MI-CAD. We defined 1-year all-cause mortality as the primary outcome, and the secondary outcome as a composite of 1-year re

Prognostic incremental value of perivascular adipose tissue in myocardial infarction with non-obstructive coronary arteries: a multicenter cohort study.

BACKGROUND: Perivascular adipose tissue (PVAT) is recognized as a key contributor to atherosclerosis and adverse clinical outcomes. However, its relationship with metabolic status and prognosis in patients with myocardial infarction with non-obstructive coronary arteries (MINOCA) remains unclear. The aim of this study was to investigate the relationship between PVAT and clinical prognosis in patients with MINOCA, and to assess its incremental predictive value beyond the traditional metrics. METHODS: A simple scoring tool, the PVAT score, was constructed based on cardiac magnetic resonance (CM

Prognostic value of soluble ST2 in cardiac magnetic resonance defined myocardial infarction with non-obstructive coronary arteries.

BACKGROUND: Cardiac magnetic resonance (CMR) plays an essential role in identifying the underlying etiology of myocardial infarction with non-obstructive coronary arteries (MINOCA). However, risk stratification in patients with CMR-confirmed MINOCA remains insufficiently defined. Soluble suppression of tumorigenicity 2 (sST2), a biomarker related to myocardial fibrosis and adverse cardiac remodeling, may provide additional prognostic information in this population. This study aimed to evaluate the prognostic value of sST2 in patients with CMR-confirmed MINOCA. METHODS: This single-center retr

The Relationship Between Advanced Lung Cancer Inflammation Index and Adverse Clinical Outcomes in Patients with Myocardial Infarction with No-Obstructive Coronary Arteries.

BACKGROUND: The advanced lung cancer inflammation index (ALI) has been suggested as a reliable prognostic indicator for cardiovascular disease. However, the association between ALI and the prognosis of patients with myocardial infarction with no-obstructive coronary arteries (MINOCA) remains undetermined. METHODS: In the present study, we consecutively included 437 MINOCA patients. All the patients received a follow-up at 1 week, 1, 3, 6, and 12 months and annually after discharge. Stress native mapping does not distinguish patients with previous myocardial infarction with non-obstructive coronary arteries from healthy volunteers.

AIMS: Patients with myocardial infarction with non-obstructive coronary arteries (MINOCA) may be affected by coronary microvascular dysfunction with reduced stress perfusion. Changes in native T1 and T2 reflect changes in myocardial perfusion; therefore, the aim of our study was to investigate whether non-contrast, adenosine stress native T1 and T2 are affected in patients with previous suspected MINOCA. METHODS AND RESULTS: Patients with MINOCA and a normal CMR (n = 15, 59 ± 7 years, 60% female) underwent 1.5T CMR together with age- and sex-matched volunteers. The protocol included native T1

Nilotinib related acute myocardial infarction with nonobstructive coronary arteries: a case report and literature review.

BACKGROUND: Myocardial Ischemia with No Obstructive Coronary Artery Disease (MINOCA) is a common cause of type 2 acute myocardial infarction (AMI) which requires careful differential diagnosis. Coronary artery spasm (CAS) syndrome is one etiology that can lead to MINOCA. Nilotinib, a targeted treatment for chronic myeloid leukemia (CML), has been reported to be related with increased risk of adverse vascular events. CASE PRESENTATION: A 67-year-old male patient was admitted to hospital with acute chest pain. He had a past medical history of CML and a history of treatment with nilotinib for 12

Myocarditis or "true" infarction by cardiac magnetic resonance in patients with a clinical diagnosis of myocardial infarction without obstructive coronary disease: A meta-analysis of individual patient data.

OBJECTIVE: Myocardial Infarction with Non-Obstructed Coronary Arteries (MINOCA) is common, but the causes are to a large extent unknown. Thus, we aimed to study the prevalence of myocarditis and "true" myocardial infarction determined by cardiac magnetic resonance (CMR) imaging in MINOCA patients, and risk markers for these two conditions in this population. METHODS: A search was made in the PubMed and Cochrane databases using the search terms "Myocardial infarction", "Coronary angiography", "Normal coronary arteries" and "MRI". All relevant abstracts were read and seven of the studies fulfil

Myocardial Infarction with Non-obstructive Coronary Arteries Complicated by a Ventricular Septal Rupture: The Challenge of Achieving Hemodynamic Stability Using Impella Devices.

The prognosis of myocardial infarction with non-obstructive coronary arteries (MINOCA) is reported to be as worse as myocardial infarction with obstructive coronary arteries. However, its mechanical complications are still poorly understood. A 71-year-old woman developed MINOCA after ascending aortic replacement surgery. During treatment, the patient experienced cardiogenic shock due to a ventricular septal rupture (VSR). The introduction of Impella devices reduced the left-to-right shunt and improved the patient's hemodynamics. Finally, a scheduled surgical repair was performed under stable c

Prognostic value of coronary Angiographic Microvascular Resistance Index in patients with myocardial infarction with nonobstructive coronary arteries.

BACKGROUND AND OBJECTIVE: Microcirculatory dysfunction is a known cause of myocardial infarction with nonobstructive coronary arteries (MINOCA). Angiography-derived microcirculatory resistance (AMR), a wire-free and adenosine-free index, offers a potential method for early assessment of microvascular function in these patients at the time of angiography. This study aimed to evaluate the prognostic impact of the Angiographic Microvascular Resistance (AMR) Index in patients with MINOCA. METHODS: A retrospective study was conducted on patients with acute MINOCA who underwent coronary angiography

Atherogenic index of plasma as a predictor of cardiovascular outcomes in patients with myocardial infarction with nonobstructive coronary arteries.

Atherogenic index of plasma (AIP) has emerged as a novel marker of atherosclerosis and a predictor of outcomes in patients with coronary artery disease. However, the prognostic significance of AIP in a distinct population with myocardial infarction with nonobstructive coronary arteries (MINOCA) remains unclear. This prospective cohort study included 1179 patients with MINOCA who were stratified by the tertile levels of AIP defined as base 10 logarithm of the ratio of fasting triglyceride to high-density lipoprotein cholesterol. The primary endpoint was major adverse cardiovascular events (MACE
