[Clinical utility of home blood pressure monitoring in patients under treatment].

INTRODUCTION: A low number of patients who are treated with antihypertensive drugs achieve therapeutic goals. Home blood pressure monitoring is an excellent tool for studying this population. OBJECTIVES: To determine the prevalence of patients with controlled and uncontrolled hypertension, as well as white-coat-effect and masked hypertension, and to evaluate the relationship with target organ damage in different groups. METHODS: Blood pressure readings were performed simultaneously in the clinic and in the home using the same validated oscillometric equipment on 83 hypertensive patients on t

Ambulatory Blood Pressure Monitoring in Pediatrics.

PURPOSE OF REVIEW: This is a review of ambulatory blood pressure monitoring (ABPM) use in pediatrics, summarizing current knowledge and uses of ABPM. RECENT FINDINGS: Updated guidelines from the American Academy of Pediatrics have emphasized the value of ABPM. ABPM is necessary to diagnose white coat hypertension, masked hypertension, and nocturnal hypertension associated with specific conditions. There is growing evidence that ABPM may be useful in these populations. ABPM has been demonstrated to be more predictive of end-organ damage in pediatric hypertension compared to office blood pressu

Normotensive patients with obstructive sleep apnoea: changes in 24-h ambulatory blood pressure monitoring with continuous positive airway pressure treatment.

BACKGROUND: Continuous positive airway pressure (CPAP) treatment reduces blood pressure (BP) in obstructive sleep apnoea (OSA) and hypertensive patients, but there is a lack of data about the effects of CPAP on the BP in normotensive patients. OBJECTIVE: The aim of the study was to evaluate BP changes in normotensive OSA individuals receiving CPAP treatment. METHODS: We selected 131 normotensive outpatients with an apnoea/hypopnoea index (AHI) greater than 15 events/hour who required CPAP treatment. All patients underwent a sleep study and 24-h ambulatory BP monitoring (ABPM) at baseline and

[Blood pressure self measurement (BPSM)].

Blood pressure self measurement (BPSM) offers advantages over measurements in the doctor's office, and may improve the overall management of hypertension. BPSM permits the identification of "white-coat hypertension" and "masked hypertension". Longitudinal studies are needed to determine the true importance of BPSM. Doctors must be aware of the strengths and weaknesses of BPSM and be able to provide their patients with information about available devices for self measurement, in particular about the accuracy and reliability of the devices they are using. Patient training in blood pressure measu

Comparative assessment of pulse transit time-derived blood pressure and ambulatory blood pressure monitoring in patients with obstructive sleep apnea.

STUDY OBJECTIVES: Nocturnal hypertension is common in patients with obstructive sleep apnea (OSA) and contributes to elevated cardiovascular risk. Pulse transit time (PTT) offers a noninvasive method to estimate nocturnal blood pressure (BP) during sleep, but its clinical validity in OSA remains unclear. This study aimed to assess the correlation between PTT-derived nocturnal BP (PTT-BP) and 24-hour ambulatory BP monitoring (ABPM-BP) in patients with suspected OSA. METHODS: Adults undergoing full-night PSG for suspected OSA were prospectively enrolled and underwent 24-hour ABPM the following

Race and sex differences in ambulatory blood pressure measures among HIV+ adults.

Ambulatory blood pressure monitoring (ABPM) can identify phenotypes that cannot be measured in the clinic. Determining race and sex disparities in ABPM measures among HIV+ individuals may improve strategies to diagnose and treat hypertension in this high-risk population. We compared ABPM measures between 24 African-American and 25 white HIV+ adults (36 men and 13 women). Awake systolic blood pressure (SBP) and diastolic blood pressure (DBP) were similar in African-Americans and whites. After multivariable adjustment, sleep SBP and DBP were 9.7 mm Hg (95% confidence interval [95% CI]: 4.7, 14.8

[Hypertension in cardiovascular diseases].

The clinical signs of coarctation of the aorta are easy to recognize. Praeductal stenosis may be masked by patent duct in the newborn. The treatment is surgical. The indications for angiography and for surgery are determined by clinical findings and symptoms of hypertension or cardiac failure. Stenoses of the abdominal aorta or stenoses of other arteries causing hypertension are rare in comparison to classical coarctation. Since the life time expectancy is reduced and the complications of hypertension due to aortic or arterial stenosis are numerous and grave early detection and surgical treatm

Gender Differences in Hypertension.

Hypertension is the leading risk factor for global mortality and morbidity and remains the major preventable cause of cardiovascular diseases. Gender differences in risk factors and awareness, treatment, and control of hypertension have been well established in humans. There are significant differences in epidemiology and clinical characteristic of hypertension between men and women. Moreover, gender differences are linked with several specific types of hypertension, including postmenopausal hypertension, white coat hypertension, masked hypertension, and hypertensive disorders of pregnancy. White-coat effect on orthostatic hypotension: a nationwide survey of Japanese general practitioners.

OBJECTIVE: The relationship between the white-coat effect (WCE), defined as white-coat hypertension under treatment, and the frequency of orthostatic hypotension (OH) is not known. We conducted an orthostatic test in patients with WCE to determine the frequency of OH. METHODS: This was a cross-sectional study of 5631 patients with hypertension visiting general practitioners nationwide, in which 4305 patients with hypertension recorded their home blood pressure (BP) and consented to the orthostatic test. Patients with hypertension were divided into four groups: controlled hypertension (CHT), m

Antihypertensive therapy of late arterial hypertension in children following successful coarctation correction.

BACKGROUNG: Late arterial hypertension (AH) is the most significant complication of coarctation of the aorta (CoA). Only a few clinical studies described antihypertensive treatment of late AH following successful CoA repair. The primary objective of this multicentre cross-sectional study was to describe real-life antihypertensive therapy for late AH in children after hemodynamically successful CoA repair. The secondary objective was to describe antihypertensive therapy used within different haemodynamic phenotypes of AH. METHOD: Blood pressure status, echocardiographic parameters and central

Integrating Out-of-Office Blood Pressure in the Diagnosis and Management of Hypertension.

Guidelines for the diagnosis and monitoring of hypertension were historically based on in-office blood pressure measurements. However, the US Preventive Services Task Force recently expanded their recommendations on screening for hypertension to include out-of-office blood pressure measurements to confirm the diagnosis of hypertension. Out-of-office blood pressure monitoring modalities, including ambulatory blood pressure monitoring and home blood pressure monitoring, are important tools in distinguishing between normotension, masked hypertension, white-coat hypertension, and sustained (includ

2025 Thai Hypertension Society Guidance for Ambulatory Blood Pressure Monitoring in Adults.

More than 40 years of research has consolidated ambulatory blood pressure monitoring (ABPM) as a validated choice in out-of-office blood pressure (BP) measurement methods other than home BP measurement (HBPM). ABPM can evaluate 24-h BP and BP variability. ABPM improves the diagnosis of hypertension phenotypes such as white coat hypertension, masked hypertension, dipper or non-dipper. BP values derived from ABPM had better prognostic values than clinic BP. Ambulatory BP devices have been available in Thailand for several years. This recommendation of the Thai Hypertension Society for ABPM was d

Clinical value of ambulatory blood pressure: Is it time to recommend for all patients with hypertension?

Hypertension is a very common disease, and office measurements of blood pressure are frequently inaccurate. Ambulatory Blood Pressure Monitoring (ABPM) offers a more accurate diagnosis, more detailed readings of average blood pressures, better blood pressure measurement during sleep, fewer false positives by detecting more white-coat hypertension, and fewer false negatives by detecting more masked hypertension. ABPM offers better management of clinical outcomes. For example, based on more accurate measurements of blood pressure variability, ABPM demonstrates that taking antihypertensive medica

Anomalous left coronary artery from pulmonary artery with mitral stenosis.

The usual presentation of anomalous left coronary artery from pulmonary artery is severe left-sided heart failure and mitral valve insufficiency presenting during the first months of life. The manifestations of left heart failure may be masked if pulmonary artery pressure remains high. We believe this is a rarest of rare case of anomalous left coronary artery from pulmonary artery with severe mitral stenosis and pulmonary hypertension in which pulmonary hypertension, along with good collateral circulation helped to preserve left ventricular function.

Assessing Machine Learning for Diagnostic Classification of Hypertension Types Identified by Ambulatory Blood Pressure Monitoring.

BACKGROUND: Inaccurate blood pressure (BP) classification results in inappropriate treatment. We tested whether machine learning (ML), using routine clinical data, can serve as a reliable alternative to ambulatory BP monitoring (ABPM) in classifying BP status. METHODS: This study employed a multicentre approach involving 3 derivation cohorts from Glasgow, Gdańsk, and Birmingham, and a fourth independent evaluation cohort. ML models were trained using office BP, ABPM, and clinical, laboratory, and demographic data, collected from patients referred for hypertension assessment. Seven ML algorith

Ambulatory blood pressure monitoring before and after resection of catecholamine-secreting pheochromocytoma or paraganglioma.

Pheochromocytomas/paragangliomas are catecholamine-secreting tumors that are a rare cause of hypertension and associated with high cardiovascular risk. We aimed to evaluate changes in 24-h ambulatory blood pressure (BP) monitoring (ABPM) following tumor resection. Individuals with evidence of pheochromocytoma/paraganglioma prospectively underwent 24-h ABPM and plasma and urine catecholamine measurement 2-3 weeks before and 6-8 weeks after pathologically-confirmed tumor resection. Patients with metastatic disease were excluded. Changes in office, 24-h, daytime, and nighttime mean systolic and d
