Dural enhancement and thickening in acute mastoiditis.

Dural enhancement and thickening in imaging studies observed in acute mastoiditis patients is an uncommon phenomenon. It is infrequently seen in dural sinus thrombosis, and may be caused by infiltration of inflammatory cells and an increased number of thin-walled blood vessels. We present a three-year-old boy who presented with acute mastoiditis, complicated by subperiosteal abscess. Computerized tomography (CT) demonstrated subperiosteal abscess, and the child underwent mastoidectomy. Despite adequate treatment, symptoms worsened and neurological sequelae were suspected. CT and magnetic reson

Acute mastoiditis in children with a cochlear implant.

OBJECTIVES: Cochlear implantation is performed at a young age, when children are prone to acute otitis media. Acute mastoiditis is the most common complication of otitis media, but data on its management in the presence of a cochlear implant are sparse. The objective of this study was to assess the characteristics, treatment, and outcome of acute mastoiditis in children with a cochlear implant. METHODS: The medical files of all children who underwent cochlear implantation at a pediatric tertiary medical center in 2000-2014 were retrospectively reviewed. Those diagnosed with acute mastoiditis

Algorithmic management of pediatric acute mastoiditis.

OBJECTIVE: Today, no uniformly accepted diagnostic and therapeutic criteria have been established for the management of pediatric acute mastoiditis. The aim of this study is determine the efficacy and safety of an algorithmic approach for treating pediatric acute mastoiditis. METHODS: The medical records of all children (n=167) with a diagnosis of AM admitted in our center during the period 2002-2010 were retrospectively studied. Data concerning medical history, symptomatology, laboratory and imaging findings, presence of complications, treatment methods and final outcomes were reviewed and a

Conservative management of acute mastoiditis in children.

OBJECTIVE: To determine whether treatment of acute mastoiditis in children using antibiotics combined with retroauricular puncture and grommet insertion is effective compared with "standard management" with mastoidectomy. DESIGN: Retrospective study. SETTING: Tertiary pediatric center. PATIENTS: We identified 50 patients younger than 14 years with acute mastoiditis (mean age, 32 months). Individuals with subacute mastoiditis and cholesteatoma were excluded from this study. All the children had received antibiotic drug treatment. Before 2002, a subperiosteal abscess (SA) was managed by masto

Acute mastoiditis: increase in the incidence and complications.

OBJECTIVE: Acute mastoiditis is a serious complication of acute otitis media, but it can be treated successfully in most of the cases with broad spectrum intravenous antibiotics and myringotomy. In the last 5 years we have perceived that there have been more complicated cases in the otic infectious pathology and the frequency was also higher. METHODS: We reviewed the cases of mastoiditis in the last 10 years (1996-2005) in The Niño Jesús University Children Hospital in Madrid to confirm the clinic impression, the bacteriology, treatments and evolution of the children and analyze the causes of

Acute mastoiditis in children: the "Ferrara" experience.

OBJECTIVE: This study aims to investigate the clinical features and outcomes of acute mastoiditis in children referred to the ENT/Audiology Department of the University of Ferrara from January 1994 to December 2005. It also aims to discuss risk factors and to find predictors for surgery. METHODS: A retrospective study on case sheets of children with an acute mastoiditis diagnosis was carried out. Fifty-five cases fulfilled the inclusion criteria: they presented otoscopical evidence of acute otitis media and inflammatory findings of the mastoid area such as post-auricular swelling, redness or

Acute mastoiditis in Norway: no evidence for an increase.

OBJECTIVE: In a large Norwegian newspaper in November 2005, an otolaryngologist at Rikshospitalet claimed that the increasing number of children hospitalized for acute mastoiditis was worrying and questioned the restrictive use of antibiotics in Norway. Based on latter he recommended that all children below age 2 with symptoms of uncomplicated acute otitis media should receive antibiotics. Our purpose was to incidence variation and characteristics of acute mastoiditis in children. METHODS: Registry based study with complete data on hospitalization for acute mastoiditis and cortical mastoidect

Acute mastoiditis and osteomyelitis of the temporal bone.

OBJECTIVE: Acute mastoiditis becomes clinically significant when infection spreads through the periosteum and induces periosteitis. This study describes an atypical complication of acute mastoiditis: osteomyelitis of the temporal bone. PATIENTS AND METHODS: The study sample included all patients admitted for acute mastoiditis between September 2001 and December 2003 who had symptoms, signs and imaging findings of osteomyelitis of the temporal bone beyond the mastoid area. The files were reviewed for diagnosis, work-up, radiographic findings and treatment. RESULTS: The study group included 6

Tuberculous mastoiditis: when is surgery indicated?

Mycobacterium tuberculosis is a rare cause of mastoiditis, but diagnosis is often delayed, with potentially serious results. We present the case of a 7-year-old child who failed to improve even once the diagnosis was made and appropriate medical treatment initiated. At mastoidectomy, a bony sequestrum was found which had not been evident on CT scanning. We review the diagnosis and management of this condition and suggest that failure to respond to drug therapy even in the absence of demonstrable complications be added to the list of indications for surgical intervention.

Acute mastoiditis: a 10-year review.

PURPOSE: To study how antibiotic treatment and an early decision to aspirate subperiosteal abscesses can reduce hospitalization periods and costs in patients with acute mastoiditis. PATIENTS AND METHODS: During a 10-year period, 134 patients were diagnosed with acute mastoiditis at the Department of Ear, Nose, and Throat and Head and Neck Surgery (Kaplan Medical Center, Rehovot, Israel). The diagnoses was based on physical findings of retroauricular swelling and erythema and tenderness, with protrusion of the auricle forward and downward. The majority of patients (77) reached medical treatmen

Acute mastoiditis--relevant once again.

During recent years, a significantly increased incidence of acute mastoiditis was observed at the University Children's Hospital, Tübingen, Germany (1975-1979: 1.4 patients/year; 1987-1992: 4.2 patients/year; p < 0.05). We therefore reviewed the records of all patients with acute mastoiditis (n = 58) that had been treated at the Children's Hospital between 1975 and 1992 and at the Clinic of Otorhinolaryngology between 1978 and 1992. The male to female ratio was 1.8:1 and 60.4% of the patients were younger than 24 months. Retroauricular swelling was more often observed (n = 49) than protrusion

[Secretory otitis media and mastoiditis in children].

In 1984-1987 the authors followed up a selected group of 15 children with a long-standing history of secretory otitis media and latent mastoiditis. In there of them the condition was complicated by labyrinthitis. Mastoidectomy which was performed in 12 children contributed to the healing of chronic inflammatory changes in the middle ear.

[Causes of failure in radical surgery of the mastoid].

We review the surgical findings of 85 revisional radical mastoidectomies for recurrent otic discharge, with the purpose to determinate the causes of surgical failure. Cholesteatoma was found in 38.8% of the cases. The other causes were unexentered cells and granulation tissue in the sinodural angle, mastoid tip, tegmental cells and root of the zygoma. The facial recess was the place of residual cholesteatoma in 47% and the supratubal recces in 40% of the cases. We emphasized the big importance of a good meatoplasty, this was the cause of failure in 62.4% of cases. A dry ear was obtained in 94%

Swimming with a mastoid cavity: what are the risks?

The authors have found no studies and little guidance in the literature on the advisability of people swimming with a modified radical mastoid cavity. Two groups of such outpatients were therefore followed up at 2-monthly intervals for 6 months. One group of 56 patients swam (81% without ear protection) and the other group of 55 patients maintained their ears quite dry. Swimming did not increase the incidence of an infected mastoid cavity. The most important risk of swimming was vertigo which occurred in 5% of swimmers.

Acute mastoiditis: a review of 69 cases.

Sixty-nine cases of acute mastoiditis were managed at the Kaplan Hospital in Israel during a 10-year period. About two thirds (68%) recovered with conservative therapy consisting of wide myringotomy and intravenous antimicrobial therapy, usually ampicillin and cloxacillin. One third (22 patients) were managed surgically by complete mastoidectomy. Indications for surgery were subperiosteal abscess, based on clinical findings in 20 patients; one had septic fever that did not respond to antimicrobial therapy, and one had signs of meningeal irritation. Mastoid radiographs played no role in the dec
