Identification and Assessment of Drug-User Groups Among Nightlife Attendees: Self-Reports, Breathalyzer-Tests and Oral Fluid Drug Tests.

BACKGROUND AND OBJECTIVES: Even though nightlife studies with potentially intoxicated participants provide the much needed information on drug use, they face additional methodological challenges. This study aimed to explore the utility of such studies by (i) classifying nightlife attendees based on their self-reported drug use and by (ii) examining whether these classifications were meaningful when assessed against other sources of data, including oral fluid drug tests. METHODS: Self-reported questionnaires, oral fluid samples and blood alcohol concentration readings were collected in a sampl

Assessment of the MFSAF v4.0 and PROMIS Fatigue 7a to Measure Outcomes in Myelofibrosis: Validation from a Non-interventional Mixed Methods Study.

INTRODUCTION: Myelofibrosis (MF) is a bone marrow cancer causing impactful symptoms, notably excessive fatigue. While the Myelofibrosis Symptom Assessment Form (MFSAF) v4.0 and PROMIS Fatigue 7a are frequently used in clinical trials, they lack published evidence for content validity in MF. This study evaluated the content validity, psychometric properties, and patient-perceived meaningful change for both measures in an MF population. METHODS: This observational, mixed-methods study involved qualitative concept elicitation (CE) and cognitive debriefing (CD) interviews (n = 17) with patients w

Creating livable lives: A qualitative exploration of life after homelessness.

The transition from homelessness to a housed life involves complex psychosocial processes that cannot be reduced to the mere fact of securing accommodation. While Permanent Supportive Housing (PSH) approaches have shown success in achieving housing stability, less attention has been paid to how people construct meaningful lives after securing permanent housing. Through field observations and in-depth interviews conducted over multiple time points, we developed a relational framework synthesising recognition theory, feminist care ethics, and Indigenous scholarship to analyse participants' exper

Collaborative Goal Setting: A Clinical Approach for Adults With Mild Traumatic Brain Injury.

Purpose Speech-language pathologists (SLPs) have many available procedural options when setting treatment goals. Extant literature supports goal setting protocols that include and value the perspectives of the client; however, in practice, rehabilitation professionals may lean toward expert models of care when planning treatment. Our purpose is to describe a newly developed approach for SLPs and individuals with mild traumatic brain injury (mTBI) to work together to build meaningful, relevant goals. Method We utilized a multiple case study format to exemplify goal setting procedures. Specifica

Common Challenges in Predicting Opioid-related Outcomes Using Machine Learning.

Machine learning (ML) models have been commonly utilized to predict various opioid-related outcomes and risks, including post-operative opioid use, opioid use disorder (OUD), misuse, or overdose. Despite their promising performance, the clinical utility and cross-study comparability of ML models are constrained. This is mainly due to variability in outcome definitions, data heterogeneity, class imbalance and analysis concerns, inadequate external or prospective validation, and limited real-world deployment and ethical concerns. Herein, we present an updated overview of these challenges in pred

Marital Dynamics, Physical and Non-Physical Violence, and Indian Women's Reproductive Control.

Intimate partner violence is highly prevalent worldwide. Previous research has linked partner violence with reduced contraceptive use, but little is known about the role of non-physical violence in non-Western contexts. Using data from the 2015-16 India Demographic and Health Survey, this study investigates how unequal gender dynamics, marital violence, and coercive control influence unmet need for contraception. Findings suggest that coercive control may play an important role in shaping women's contraceptive use and reproductive autonomy. Health practitioners and advocates should consider co

Is there a preferred diuretic class for patients with renal impairment and hypertension?

It is widely believed that "high-ceiling" loop diuretics are required in patients with renal impairment and that "low-ceiling" thiazides are not sufficiently potent to cause meaningful natriuresis and diuresis. If this statement is true, at what level of renal function do thiazides lose their punch? Another related issue is the enlightened use of diuretic combinations. Use of diuretics in chronic kidney disease and hypertension has been addressed in the many guidelines, but further insight is provided in this installment of the "Controversies" series by two well-known authorities, William Elli

Events per person year--a dubious concept.

In 1982 a new measure was introduced in research into osteoporosis and is now used everywhere in the literature. The so called "fracture rate" relates the number of fractures (single in some patients, multiple in others) to the cumulative time of observation of all patients. This concept, however, has no sound basis. Counting events instead of patients usually violates basic statistical assumptions and invalidates the use of common statistical tests and estimators. Its clinical interpretation is rather dubious. The use of such a measure impedes the search for valid and clinically meaningful ou

Parental and School Influences on Korean Adolescent Alcohol Use: A Multilevel Study.

This study examined the moderating effect of school-based alcohol prevention education on the relationship between parental permissiveness and adolescent alcohol use using data from the 20th Korea Youth Risk Behavior Web-based Survey (N = 54,653). Individual-level variables included parental permissiveness, gender, grade, and socioeconomic status. School-based prevention education was treated as a group-level variable. Parental permissiveness was positively associated with adolescent alcohol use, while school-based prevention education was linked to reduced alcohol use. School-based programs e

Rauwolfia derivatives and breast cancer. A case/control study in Olmsted County, Minnesota.

In response to three reports of an association between rauwolfia derivatives and breast cancer, a case/control study was undertaken in Olmsted County, Minnesota. Comparison of a cohort of women with breast cancer to an age-match cohort of women with cholelithiasis failed to show any meaningful differences in the two groups except with respect to a history of hypertension. The rates of use of rauwolfia derivatives by hypertensive cases and controls were identical--which, along with other considerations, leads to the conclusion that an association between the use of rauwolfia derivatives and bre

Radiofrequency Ablation for Recurrent Thyroid Cancers: 2025 Korean Society of Thyroid Radiology Guideline.

Radiofrequency ablation (RFA) is a minimally invasive treatment modality used as an alternative to surgery in patients with benign thyroid nodules, recurrent thyroid cancers (RTCs), and primary thyroid microcarcinomas. The Korean Society of Thyroid Radiology (KSThR) initially developed recommendations for the optimal use of RFA for thyroid tumors in 2009 and revised them in 2012 and 2017. As new meaningful evidence has accumulated since 2017 and in response to a growing global interest in the use of RFA for treating malignant thyroid lesions, the task force committee members of the KSThR decid

The restore-adapt-abandon framework: supporting safe and meaningful living after ventricular assist device implantation.

BACKGROUND: Ventricular assist device (VAD) therapy improves survival and symptoms in patients with advanced heart failure but can impose lifestyle restrictions that challenge meaningful living. Engagement in meaningful activities is protective against all-cause mortality and positively impacts health, well-being, and quality of life, yet little is known about how VAD recipients adapt these activities post-implantation. OBJECTIVES: To explore how VAD recipients navigate meaningful activity engagement post-implantation, and present a clinically-relevant framework characterizing how VAD recipie

Left posterior parietal theta burst stimulation affects gestural imitation regardless of semantic content.

OBJECTIVE: Neuro-imaging studies have suggested that the ability to imitate meaningless and meaningful gestures may differentially depend on superior (SPL) and inferior (IPL) parietal lobule. Therefore, we hypothesized that imaging-guided neuro-navigated continuous theta burst stimulation (cTBS) over left SPL mainly affects meaningless and over left IPL predominantly meaningful gestures. METHODS: Twelve healthy subjects participated in this study. High resolution structural MRI was used for imaging guided neuro-navigation cTBS. Participants were targeted with one train of cTBS in three experi

Instructional design communication through AI-assisted storytelling: a hybrid model grounded in educational psychology.

INTRODUCTION: Artificial intelligence (AI)-assisted storytelling is increasingly transforming educational communication and instructional design, particularly in isolated and remote learning environments. However, challenges related to collaboration, meaningful interaction, technological accessibility, and ethical use remain. Drawing on educational psychology, this study investigates how AI-assisted storytelling communication strategies can support cognitive engagement, motivation, creativity, collaboration, and stakeholder engagement in AI-supported educational communication systems. Low-Dose Colchicine for Secondary Prevention in Patients With Chronic Coronary Syndrome: A Multicenter Real-World Study.

AIMS: Chronic coronary syndrome (CCS) remains associated with recurrent ischemic symptoms and impaired quality of life despite contemporary guideline-directed medical therapy. Low-dose colchicine has demonstrated prognostic benefits in randomized trials; however, real-world data regarding its effects on functional capacity and angina-related outcomes are limited. METHODS: This multicenter, real-world observational study included 614 patients with CCS receiving low-dose colchicine therapy for secondary prevention. Functional capacity and angina-related quality of life were assessed before and

Target product profiles in laboratory medicine: a framework for defining test requirements and supporting outcome-based performance specifications.

Despite rapid advances in diagnostics, many tests fail to deliver meaningful clinical or system value because their intended use, performance requirements, and implementation context were not clearly articulated early in development. Target product profiles (TPPs) offer a structured, consensus-based framework to define intended use, outline minimal and desirable performance specifications, and set operational expectations for testing technologies. This paper introduces the topic of TPPs, summarising their purpose, structure, and typical development pathways. We describe a three-phase developme

A codevelopment process to advance methods for the use of patient-reported outcome measures and patient-reported experience measures with people who are homeless and experience chronic illness.

INTRODUCTION: People who experience social disadvantage including homelessness suffer from numerous ill health effects when compared to the general public. Use of patient-reported outcome measures (PROMs) and patient-reported experience measures (PREMs) enables collection of information from the point of view of the person receiving care. Involvement in research and health care decision-making, a process that can be facilitated by the use of PROMs and PREMs, is one way to promote equity in care. METHODS: This article reports on a codevelopment and consultation study investigating the use of P
