Mediastinal lipoblastoma causing diaphragmatic eventration: a case report and review of the literature.

Lipoblastomatous tumors are quite rare neoplasms derived from embryonic white fatty tissues. The majority is seen in infancy and early childhood. Generally they arise in extremities. Here, we present a 12-month-old female infant presenting with a mediastinal and cervical mass, showing left diaphragmatic eventration. We reviewed these cases and discussed clinical manifestations, radiologic, and histopathologic findings and treatment of these tumors.

Study of total lipid peroxidation and antioxidant activity in pulmonary and mediastinal malignant and benign tumor tissue.

Total lipid peroxidation and antioxidant activity were studied in pulmonary and mediastinal malignant and benign neoplasms. In malignant tumors total lipid peroxidation increased and antioxidant activity decreased; the intensity of these shifts depended on histological characteristics and degree of malignancy: the most pronounced changes were observed in thymoma and adenocarcinoma tissues and minimum changes were found in bronchoalveolar carcinoma tissue.

Right brachiocephalic cavernous hemangioma: a rare vascular mimic of a superior mediastinal mass.

BACKGROUND: Cavernous hemangiomas of the mediastinum are rare vascular malformations that can mimic neoplasms on imaging. Their imaging features often overlap with those of malignant superior mediastinal masses, presenting a diagnostic challenge. CASE PRESENTATION: We present the case of a 35-year-old male in whom a right superior mediastinal mass was incidentally detected during preoperative evaluation. Imaging revealed a well-defined lesion involving the right brachiocephalic vein, internal jugular vein, and superior vena cava. Due to initial concern for malignancy, an open surgical biopsy w

Type II pleuropulmonary blastoma in a fetus: case report of a rare mesenchymal mediastinal tumor and literature review.

Mediastinal tumors are exceedingly rare during fetal development, presenting significant diagnostic challenges and potentially leading to severe outcomes such as stillbirth or metastatic disease if not promptly identified and managed. Pleuropulmonary blastomas are primitive mesenchymal tumors often linked to mutations in the DICER1 gene, indicating a hereditary pattern associated with other common adult neoplasms with dominant inheritance. This report describes a case involving a 20-year-old Caucasian woman whose pregnancy was complicated by a stillbirth in the second trimester. Initial suspic

The oncocyte that went places: diagnosis with EUS-guided FNA.

Although uncommon, Hurthle cell neoplasms have a characteristic histologic and cytologic features which often aid in the diagnosis of these lesions. In fine needle aspiration biopsies, determining malignant potential on cytologic features is challenging unless other evidence of malignancy (such as metastases) are known. The role of EUS-guided FNA in diagnosing superior mediastinal masses has been described previously. It's role in evaluating subcarinal metastases from lung cancer primaries have also heen studied. However we describe metastatic thyroid Hurthle cell carcinoma to the subcarina di

Complete Resection of Mediastinal Undifferentiated Pleomorphic Sarcoma.

A 70-year-old man was referred to our hospital for mediastinal malignant neoplasm. Chest computed tomography revealed a 20-cm mass in the middle mediastinum with displacement of the superior vena cava, left innominate vein, and right upper lobe. Tumor resection, right upper lobectomy, combined resection of the left brachiocephalic vein and superior vena cava, and revascularization were performed. It was pathologically diagnosed as an undifferentiated pleomorphic sarcoma, and the patient experienced no recurrence for 4 years. Although the prognosis of mediastinal pleomorphic sarcoma cases is po

[Treatment of Hodgkin's disease with bulky mediastinal involvement. Study of 28 cases].

BACKGROUND: Bulky mediastinal involvement in Hodgkin disease (HD) is usually considered an adverse prognostic factor. Patients treated with radiotherapy alone (RT) or chemotherapy alone (CT) have a high relapse rate. The combined chemotherapy plus radiotherapy is the therapy of choice. The objective of the present study was to report our results and complications in the treatment of 28 patients with HD with bulky mediastinal involvement. MATERIALS AND METHODS: A retrospective analysis was made of a series of 28 patients with bulky mediastinal disease treated in our hospital from 1973-1993, wi

Cardiac injury as late toxicity of mediastinal radiation therapy for Hodgkin's disease patients.

BACKGROUND: During the last 20 years Hodgkin's disease (HD) has become one of the most curable neoplasms; in fact, more than 75-80% of patients are expected to achieve long-term relapse-free survival with appropriate therapy. However, overall survival has been affected by intercurrent or treatment-induced diseases such as the increased risk of cardiac toxicity in patients who received mediastinal irradiation. METHODS: The incidence of cardiac abnormalities after mediastinal radiotherapy was assessed in 102 consecutive HD patients who underwent this treatment from January 1970 to December 1980

Asymptomatic Glomus Tumor of the Mediastinum.

Glomus tumors are rare benign neoplasms that predominate in limbs. Infrequently, they can occur in a wide anatomic distribution, to include sites not known to contain glomus cells. Although glomus tumors are usually small, pain and tenderness are common clinical symptoms. We report the case of a 69-year-old man with an asymptomatic large mediastinal glomus tumor, who underwent surgical resection.

Neonatal primary pulmonary neoplasms, clinical, imaging, and differential diagnosis.

Most prenatal/neonatal pulmonary masses are non-neoplastic congenital bronchopulmonary malformations. However, both benign and malignant primary pulmonary neoplasms do occur. While rare, these neoplasms can be difficult to differentiate from one another, as well as from other congenital and some acquired pulmonary lesions, including occasional mediastinal and chest wall masses. Misdiagnosis or delayed identification of a pulmonary neoplasm can result in incorrect or delayed management decisions and therapy, adversely affecting outcomes. Atypical timing of presentation, rapid growth, and unusua

Primary ovarian diffuse large B-cell lymphoma.

Primary ovarian lymphoma is a rare malignancy, representing <1% of ovarian neoplasms and a small subset of extranodal lymphomas, with diffuse large B-cell lymphoma (DLBCL) being the most frequent subtype. Diagnosis is often delayed because symptoms overlap with more common gynaecological or infectious conditions. We report a woman in her 20s presenting with hoarseness, exertional dyspnoea and low-grade fever. Imaging showed mediastinal lymphadenopathy, and repeated biopsies revealed necrotising granulomatous inflammation without malignancy. Persistent symptoms prompted fluorodeoxyglucose (FDG)

Mediastinal lymphoma in dogs is homogeneous compared to thymic epithelial neoplasia and is more likely to envelop the cranial vena cava in CT images.

In order to identify CT signs that could be used to distinguish cranial mediastinal lymphoma and thymic epithelial neoplasia, a retrospective case-control study was done. Associations between CT signs and diagnosis were tested using binary logistic regression and results expressed as odds ratio and 95% confidence interval. Sixty-two dogs that had thoracic CT and confirmed diagnosis of lymphoma (n = 33) or thymic neoplasia (n = 29) were sampled. Thymic neoplasms included 24 thymomas and five thymic carcinomas. Dogs with thymic epithelial neoplasia were significantly older than dogs with lymphom

A Rare Case of a Middle Mediastinal Mass With Intense and Diffuse Contrast Enhancement.

Masses of the mediastinum have a wide differential diagnosis. Paragangliomas are rare neoplasms of neuroendocrine origin. These masses may be either parasympathetic or sympathetic in origin and have variable presentation. Paragangliomas may very rarely form in the mediastinum and make up a small fraction of masses originating in this location. We present a case of a previously healthy 49-year-old female who was incidentally found to have a middle mediastinal mass with intense and diffuse contrast enhancement on computerized tomography (CT) of the chest with contrast which was ultimately determ

Mediastinal thymolipoma: a rare occurrence with striated myoid cells.

We describe a 49-year-old man who presented to hospital with unstable angina and who underwent emergency coronary artery bypass grafting. During the surgical procedure, an incidental anterior mediastinal mass was discovered. Histological examination of this mediastinal mass revealed a thymolipoma containing numerous polygonal, striated myoid cells that were immunoreactive for desmin, muscle-specific actin and myoglobin. Electron microscopy demonstrated numerous Z band structures confirming myoid differentiation. Even though the appearance of myoid cells in thymolipoma may be alarming, this tum

Diagnosis of mediastinal masses in dogs by flow cytometry.

BACKGROUND: Biopsy of mediastinal masses can be invasive, but the procedure may be necessary if cytology of a mass aspirate is inconclusive. The 2 most common mediastinal masses, lymphoma and thymoma, may both be comprised of small lymphocytes. We investigated the ability of flow cytometry to distinguish between these 2 neoplasms. HYPOTHESIS: Flow cytometry of mediastinal mass aspirates may provide a definitive diagnosis of thymoma or lymphoma, reducing the need for biopsy. ANIMALS: Dogs with mediastinal masses presenting to the Veterinary Teaching Hospital/Animal Cancer Center were included

Computed tomography in the preoperative evaluation of bronchogenic carcinoma.

Preoperative determination of the extent of bronchogenic carcinoma at presentation was assessed by CT using a 4th generation scanner with a 3-second scan time in 98 patients whose disease was later surgically staged. Prospective CT interpretations correctly staged 33 of 35 (94%) resectable lesions, and disease in 41 of 45 (91%) patients who had lesions that were not resectable for cure. No definite opinion was rendered on the scans of the remaining 18 patients. Retrospectively analysis using slightly modified criteria resulted in a correct prediction of resectability in 28 of 30 (93%) patients

An Unusual Presentation of a Posterior Mediastinal Schwannoma Associated with Traumatic Hemothorax.

Schwannomas of the thoracic cavity are typically an asymptomatic, benign neurogenic neoplasm of the posterior mediastinum. In this case, we present a traumatic hemothorax as the initial presentation for a previously undiscovered mediastinal mass. The patient presented with shortness of breath and right-sided chest pain after being struck in the chest with a soccer ball. An operative exploration was pursued due to persistent hemothorax with hemodynamic instability despite resuscitation and adequate thoracostomy tube placement. The intraoperative etiology of bleeding was discovered to be traumat
