Health challenges including behavioral problems in long-duration spaceflight.

Over the past 60 years, our ability to live and work in space has evolved. From short sojourns in small spacecraft to landing on the moon and residing in an orbiting international space station, we have learned to adapt to an extreme environment and safely return home. Human missions to the Moon, Mars, and exploration of deep space are different. This paper summarizes the challenges of providing medical care, specifically mental health care during long-duration flights. Considerable information about challenges that crews bound for Mars will face is available. Literature regarding this issue i

Adequacy of in-mission training to treat tibial shaft fractures in mars analogue testing.

Long bone fractures are a concern in long-duration exploration missions (LDEM) where crew autonomy will exceed the current Low Earth Orbit paradigm. Current crew selection assumptions require extensive complete training and competency testing prior to flight for off-nominal situations. Analogue astronauts (n = 6) can be quickly trained to address a single fracture pattern and then competently perform the repair procedure. An easy-to-use external fixation (EZExFix) was employed to repair artificial tibial shaft fractures during an inhabited mission at the Mars Desert Research Station (Utah, USA

Arrhythmias Occurring in Children during HEMS Intervention: A Retrospective Cohort Study.

BACKGROUND: Arrhythmias in patients during medical transport remain a challenge for medical personnel. Helicopter emergency medical service (HEMS) crews, as the only medical rescue teams in Poland to conduct rescue flights, keep detailed documentation of monitoring vital functions over short time intervals during the flight. AIMS: The aim of this study was to determine the characteristics of cardiac arrhythmia in pediatric patients (up to 12 years of age) transported by HEMS operatives, considering life-threatening rhythms and those that occur during out-of-hospital cardiac arrest (OHCA). Identifying Predisposing Factors for Hyperopic Shift in Long-Duration Spaceflight.

INTRODUCTION: NASA identifies spaceflight-associated neuro-ocular syndrome (SANS) as a significant health concern for astronauts on missions exceeding 30 d. This retrospective analysis evaluates age, mission duration, and preflight bodyweight as risk factors for hyperopic shift, a component of SANS. Results may guide the development of prevention strategies for future missions and provide insight into similar conditions on Earth. METHODS: Deidentified data were obtained from NASA' s Medical Evaluation Documents, Volume B database for astronauts who participated in space missions >30 d. Refrac

Initial 67th Combat Support Hospital Optometry Services in Taszar, Hungary, during Operation Joint Endeavor.

BACKGROUND: Existing U.S. Army personnel and equipment authorization documents do not recognize the need for optometry services at deployed combat support hospitals (CSHs). The specific hospital tasked with the mission of supporting Operation Joint Endeavor from a support base in Taszar, Hungary, did not have any documentation authorizing the assignment of ophthalmology assets. Current Army doctrine stipulates the presence of an area support medical battalion, with assigned optometry assets, to deploy and operate near a CSH. However, in the winter of 1995, when Operation Joint Endeavor began,

Cardiovascular risks and hazards associated with deep space exploration.

Space flight exposes astronauts to a unique environment characterized by microgravity, ionizing radiation, and other stressors that can profoundly affect the human body. Deep-space exploration-type missions to the Moon (NASA's Artemis and Gateway programs), Mars, and beyond will introduce prolonged exposure to health hazards, including but not limited to isolation and confinement, sleep disruption, and exposure to different types and quantities of particle radiation (i.e., high-energy and atomic number - HZE). There are also logistical implications of traveling farther from low Earth orbit (LE

Microbial resilience in space: Biofilms, risks and strategies for space exploration.

Biofilms are a community of microorganisms that can form on any surface, posing several challenges and significant medical issues. Their formation is not just limited to Earth but has also been observed in space stations and are termed as space biofilms. This is a major concern as certain biofilms can lead to high-risk compromising crew's health, while others have the capacity to corrode spacecraft and equipment, leading to instrument malfunction, which can jeopardize the mission. Additionally, the way biofilms form and behave in space is different from how they do on Earth due to microgravity

Difficulty Ventilating: A Case Report on Ventilation Considerations of an Intubated Asthmatic Undergoing Air Medical Critical Care Transport.

The air medical transport of intubated patients is a high-risk mission that requires preplanning before helicopter launch. This case describes a scenario in which the helicopter emergency medical services (HEMS) team was unable to ventilate a patient because of the mechanical limitations of the transport ventilator. The HEMS mission was ultimately aborted, and the patient had to be transported by a ground crew equipped with a hospital-based ventilator. In addition to the optimal medical management of the patient in status asthmaticus, critical care transport crews must be familiar with the tre

Psychiatric components of a health maintenance facility (HMF) on Space Station.

The operation of a permanently manned space station brings a special challenge to psychiatry due to the combined physiological, emotional, and social stresses which will affect its occupants. Psychiatric diagnostic and therapeutic techniques will be a part of the Space Station's general medical support, known as the Health Maintenance Facility (HMF). The psychiatric component of HMF will promote psychological health throughout the mission and permit treatment, if necessary, of those psychiatric disorders which could impact mission success. This paper will discuss these emotional disorders and
