Primary Headache Disorders- Part 2: Tension-type headache and medication overuse headache.

In Part 2 of Primary Headache disorders, we discuss the fourth Primary Headache Disorder, Tension-Type Headache (TTHA). We are again using the ICHD-III (Beta) definitions of such headaches, taking into consideration episodic and chronic TTHA, as well as the presence or absence of pericranial muscle tenderness. We discuss the pathophysiology and pharmacotherapeutic treatment of TTHA, and the aspects of the Myofascial Pain Syndrome that enhance and help the development of TTHA. We then discuss Medication Overuse Headache (MOH), itself a Secondary headache disorder, but one that is extremely impo

Analysis of risk factors for medication-overuse headache relapse: a clinic-based study in China.

BACKGROUND: Medication overuse headache (MOH) is the third most prevalent headache type after migraine and tension-type headache. A large number of studies on the long-term prognosis have shown that MOH has a high relapse rate after treatment. Although MOH relapse-related risk factors have been reported, no related research has been performed in China. Therefore, the purpose of this study was to analyze and evaluate the risk factors for MOH relapse in China. METHODS: Eighty-six out-patients of Shandong Provincial Hospital who were initially diagnosed with MOH, and who had successful withdrawa

Prevalence of chronic migraine and medication overuse headache in Germany--the German DMKG headache study.

Population-based epidemiological studies about the prevalence of chronic migraine using the 2004 International Headache Society (IHS) classification definition are rare. We analysed the data of the Deutsche Migräne und Kopfschmerz Gesellschaft headache study, which included 7417 adults in three regions of Germany, with respect to their headache. Additionally, body mass index, alcohol consumption and smoking behaviour were recorded. Using the IHS definition from 2004, chronic migraine was diagnosed in 0.2% of the population. Half of these patients also fulfilled the criteria of medication overu

Medication overuse headache: a critical review of end points in recent follow-up studies.

No guidelines for performing and presenting the results of studies on patients with medication overuse headache (MOH) exist. The aim of this study was to review long-term outcome measures in follow-up studies published in 2006 or later. We included MOH studies with >6 months duration presenting a minimum of one predefined end point. In total, nine studies were identified. The 1,589 MOH patients (22% men) had an overall mean frequency of 25.3 headache days/month at baseline. Headache days/month at the end of follow-up was reported in six studies (mean 13.8 days/month). The decrease was more pro

EPIGAME a vignette-based study of management practices of medication overuse in migraine, among French neurologists.

BACKGROUND: Medication overuse headache (MOH) is a common and disabling condition associated with the excessive use of acute headache treatments. This study aimed to investigate MOH management by neurologists in France. METHODS: A vignette-based study was conducted among 107 neurologists (60 from private practice, 47 from hospitals or both hospital and private practice) in France. Participants evaluated four randomized clinical vignettes simulating real-life MOH cases and selected from five treatment strategies: (1) drug withdrawal and immediate initiation of preventive treatment, (2) initiat

Anti-CGRP monoclonal antibodies for chronic migraine with medication-overuse headache: a conservative meta-analysis.

Chronic migraine (CM) is often complicated by medication-overuse headache (MOH), worsening disability. Although withdrawal of overused medications is recommended, adherence is poor and relapse is frequent. Monoclonal antibodies (mAbs) targeting the calcitonin gene-related peptide (CGRP) pathway may offer an effective preventive option without requiring discontinuation, but current evidence is limited by few and heterogeneous randomized controlled trials (RCTs).To systematically assess the efficacy and safety of anti-CGRP mAbs in treating CM with MOH (CM + MOH) under a conservative approach. The

Inhibiting astrocyte activation is effective in triptan-induced medication-overuse headache model and prolonged hyperalgesia.

OBJECTIVE: We hypothesized that sustained astrocyte activation in the trigeminal nucleus caudalis (TNC) drives prolonged hyperalgesia in triptan-induced medication-overuse headache (MOH). This study aimed to investigate the role of astrocyte activation and evaluate the therapeutic potential of targeted astrocyte inhibition. BACKGROUND: Chronic triptan use for migraine can lead to MOH, characterized by persistent hyperalgesia and high relapse rates. The mechanisms underlying this transition remain poorly understood. Although central sensitization and neuroinflammation are implicated, the speci

Excessive iron accumulation in the striatum associated with addictive behaviors of medication-overuse headache: a prospective study.

BACKGROUND: Abnormal iron deposition may be a biomarker for a disrupted central antinociceptive neuronal network, and the relationship between iron deposition and the pathophysiological mechanisms of chronic migraine (CM) with medication overuse (MOH) remains unclear. We investigated iron deposition in the deep gray matter (DGM) of the brain in CM patients with and without MOH using quantitative susceptibility mapping (QSM). METHODS: Forty-eight healthy controls (HCs) and 69 CM patients (36 with MOH; 33 without MOH) were recruited. QSM data were acquired using a 3.0 T Magnetic resonance imagi

Brain structural network modular and connectivity alterations in subtypes of patients with migraine and medication overuse headache.

Migraine, one of the most prevalent and debilitating neurological disorders, can be classified based on attack frequency into episodic migraine (EM) and chronic migraine (CM). Medication overuse headache (MOH), a type of chronic headache, arises when painkillers are overused by individuals with untreated or inadequately treated headaches. This study compares regional cortical morphological alterations and brain structural network changes among these headache subgroups. Sixty participants, including 20 in each of the following patient groups (EM, CM, MOH), and healthy controls (HC) completed th
