Identification of a potential fibromyalgia diagnosis using random forest modeling applied to electronic medical records.

BACKGROUND: Diagnosis of fibromyalgia (FM), a chronic musculoskeletal condition characterized by widespread pain and a constellation of symptoms, remains challenging and is often delayed. METHODS: Random forest modeling of electronic medical records was used to identify variables that may facilitate earlier FM identification and diagnosis. Subjects aged ≥18 years with two or more listings of the International Classification of Diseases, Ninth Revision, (ICD-9) code for FM (ICD-9 729.1) ≥30 days apart during the 2012 calendar year were defined as cases among subjects associated with an integra

An analytical approach to characterize morbidity profile dissimilarity between distinct cohorts using electronic medical records.

We describe a two-stage analytical approach for characterizing morbidity profile dissimilarity among patient cohorts using electronic medical records. We capture morbidities using the International Statistical Classification of Diseases and Related Health Problems (ICD-9) codes. In the first stage of the approach separate logistic regression analyses for ICD-9 sections (e.g., "hypertensive disease" or "appendicitis") are conducted, and the odds ratios that describe adjusted differences in prevalence between two cohorts are displayed graphically. In the second stage, the results from ICD-9 sect

[Explorative laparotomy combined with splenectomy in malignant lymphomas (evaluation of 52 medical records) (author's transl)].

52 medical records of patients with malignant lymphomas, who had been treated by explorative laparotomy and splenectomy, were evaluated retrospectively. In all patients lymphography and splenic scintigraphy had been performed prior to surgical treatment, and in most of them additional liver scintigraphy. The splenic scintigram was true-positive in 11 cases, false-positive in 6, false-negative in 8, and true-negative in 27 cases. Lymphography was true-positive in 9 and false-positive in 17 cases, false-negative in 4 and true-negative in 22 cases. Comparison shows that explorative laparotomy and

Nursing Students' Experiences of Electronic Medical Records During Placements: A Mixed-Method Sequential Explanatory Design.

As the adoption of electronic medical records has become a global trend, the need to integrate electronic medical records education into undergraduate nursing curricula has been internationally recognized. There are very few nursing colleges in China put an emphasis on the skills required to properly use nursing informatics. This study explored Chinese nursing students' experiences with Electronic Medical Records during clinical placements. This research used a sequential explanatory mixed-methods design. First, a survey of 355 participants assessed electronic medical records exposure, attitud

A Sentence Classification-Based Medical Status Extraction Pipeline for Electronic Health Records: Institutional Case Study.

BACKGROUND: Clinical data warehouses store large volumes of unstructured text containing valuable information about patients' medical status. Traditional extraction systems based on named entity recognition (NER) identify medical terms but often fail to capture the contextual cues needed for accurate interpretation. Existing approaches to context-aware extraction differ in their reliance on expert annotation, computational power, and lexical resources, leading to uneven feasibility across institutions. Combined with heterogeneity in documentation practices and data-sharing restrictions, these

Analysis of circumstances in police-related fatalities comparing medical examiner records with a public database.

Assessing circumstances surrounding police-related deaths is hampered by the absence of an official national database. The National Vital Statistics System, which compiles death certificates filed by medical examiners and coroners, misses approximately one-half of deaths due to police use of force. The void has been filled by the emergence of several unofficial databases sourced from public records, which have been used, for example, to estimate the number of fatal police shootings of unarmed individuals. However, these public databases have not been validated against official records. Further

Automated Identification of Surgical Site Infections From Electronic Medical Records: Retrospective Observational Predictive Modeling Study.

BACKGROUND: Surgical site infections (SSIs) affect 160,000 to 300,000 patients annually, increasing postoperative mortality, causing significant complications, and incurring US $3.5 to US $10 billion in excess costs each year. Effective SSI surveillance can inform strategies to mitigate these outcomes. Traditional SSI surveillance methods, primarily manual chart reviews, are costly and labor-intensive. OBJECTIVE: This study aimed to evaluate whether an automated SSI surveillance system built using newer natural language processing methods and deep learning could outperform previous approaches

Discovering sequential patterns and interrelations among multiple diseases in electronic medical records using cSPADE algorithm.

BackgroundThe intricate relationships between diseases are characterized by the sequence and temporal intervals of their onset, which are critical for understanding the essence of comorbidity and predicting disease progression. This study seeks to investigate the interdependencies and chronological order of various diseases that occur in the same patient by employing sequential pattern mining algorithms. Specifically, the research endeavors to delineate the disparities in the time intervals between the onset of distinct disorders and to scrutinize the concordance and discordance in disease seq

Longitudinal Analysis of Electronic Health Records Reveals Medical Conditions Associated with Subsequent Alzheimer's Disease Development.

BACKGROUND: Several health conditions are known to increase the risk of Alzheimer's disease (AD). We aim to systematically identify medical conditions that are associated with subsequent development of AD by leveraging the growing resources of electronic health records (EHRs). METHODS: This retrospective cohort study used de-identified EHRs from two independent databases (MarketScan and VUMC) with 153 million individuals to identify AD cases and age- and gender-matched controls. By tracking their EHRs over a 10-year window before AD diagnosis and comparing the EHRs between AD cases and contro

Medical phenome of musicians: an investigation of health records collected on 9803 musically active individuals.

Previous studies suggest that musicians may be at higher risk for a set of medical problems; however, this literature has been limited by relatively small sample sizes, self-reports, and lack of controls. To address such limitations, we examined trends in the medical care of musicians in an Electronic Health Record database. On the basis of a collection of keywords and regular expressions in the patients' clinical notes, we identified 9803 "musicians" that we matched for sex, median age (across the medical record), ethnicity, race, the length of record, and the number of visits with 49,015 con
