Finite sample size errors in the context of multiple error sources in quantitative medical imaging: An evaluation for breast magnetic resonance diffusion-weighted imaging.

BACKGROUND: Selecting appropriate sample sizes in magnetic resonance imaging studies is a complex process that requires to balance statistical rigor with the practical challenges of measuring a large patient population. In this Institutional Review Board approved study, we evaluate the dominant error types ("finite N" errors versus precision errors) for apparent diffusion coefficient (ADC)-based lesion characterization in diffusion-weighted magnetic resonance imaging (DWI) of the female breast in a local dataset and compare our results with current literature. METHODS: First, in a literature

Methotrexate toxicity due to a medication dispensing error compounded by an interaction with proton pump inhibitors.

Methotrexate at a low weekly dose is used for immunomodulation in autoimmune conditions such as rheumatoid arthritis and psoriasis. With an elimination half-life of a few hours, the drug is primarily cleared through the renal route. Elimination of the drug is delayed in the presence of polypharmacy or deranged renal function. Here, we present a case of a 55-year-old lady who was on chronic once-weekly methotrexate therapy for rheumatoid arthritis. A recent change in the branded generic formulation of methotrexate by her pharmacist led to confusion with another prescription medication she was r

Factors associated with the prevalence of refractive errors among medical students in the United Arab Emirates.

BACKGROUND: Refractive errors (REs) remain a leading cause of visual impairment worldwide. Medical students frequently report a high burden of REs, possibly attributable to genetic influences and lifestyle factors like extended near-work activities and limited outdoor exposure. Limited data exist on the prevalence and determinants of REs among medical students in the United Arab Emirates (UAE). This study aimed to determine the prevalence of REs in this population and to identify the key potential risk factors associated with them. METHODS: A cross-sectional survey was conducted among medical

Exploring Combat Tourniquet Application Errors and Duration Among Skilled and Novice Medical Trainees.

INTRODUCTION: The use of tourniquets in combat medicine continues to be a key focus as they have consistently been shown to combat one of the leading causes of preventable death on the battlefield, massive hemorrhage to extremities. The present study analyzed tourniquet application among combat medics (68W) and combat lifesavers (CLSs) in a training environment to determine whether trainees' performance is consistent among one another and whether performance can be associated with participant demographics such as experience or role. MATERIALS AND METHODS: Study participants treated male and f

Impact of Using Educational Multimedia on Knowledge, Skill, and Reduction of Medical Error Following Bronchoscopy.

BACKGROUND: Achieving procedural skills is one of the pillars of health higher education which is in line with the social responsibility of medical education. Since it is not possible to encounter important cases in bronchoscopy during the training course, the common cases that the students encounter in their future work environment were prepared as an educational video. Therefore, the purpose of this study was to find out the impact of using bronchoscopy educational video intervention on medical assistants' knowledge, skill, and medical error comparing it with the traditional method at Dr. Ma

Bleomycin overdose in a patient with stage II Hodgkin lymphoma: A case report of a medication error.

Bleomycin is an antibiotic with cytotoxic properties that is commonly used in combination regimens for the treatment of Hodgkin lymphoma. The inconsistency in the dosage nomenclature of bleomycin appears universally, which increases the risk of medication errors. However, to the best of our knowledge, no such cases have been reported. Herein, we present a rare case report of a medication error caused by a bleomycin overdose. At the third cycle of chemotherapy, a 25-year-old patient with stage II Hodgkin lymphoma received a 10 times higher dose of bleomycin (150 USP) than that usually used as p

Improving cancer patient care with combined medication error reviews and morbidity and mortality conferences.

BACKGROUND: To reduce the occurrence of medication errors, a systemic approach was developed combining anti-neoplastic medication error reviews and morbidity and mortality conferences (M&MCs). We report the first experience of implementing this strategy in oncology. METHODS: The case reports submitted to combined reviews were prepared by physicians and pharmacists, and medication error(s) were described and chronological and root-cause analyses were performed. RESULTS: Ten combined reviews were conducted, which involved the departments of haematology, medical oncology, pneumology, gastroente

Effect of reducing interns' work hours on serious medical errors in intensive care units.

BACKGROUND: Although sleep deprivation has been shown to impair neurobehavioral performance, few studies have measured its effects on medical errors. METHODS: We conducted a prospective, randomized study comparing the rates of serious medical errors made by interns while they were working according to a traditional schedule with extended (24 hours or more) work shifts every other shift (an "every third night" call schedule) and while they were working according to an intervention schedule that eliminated extended work shifts and reduced the number of hours worked per week. Assessing the Influence of a Statewide Dosing Reference Aid on Prehospital Pediatric Medication Dosing Errors: A Mixed-Methods Simulation-Based Investigation.

OBJECTIVES: Errors occur in 31% of prehospital pediatric medication administrations (PMA) in Michigan despite the implementation of the MI-MEDIC pediatric dosing reference aid. Reference aids may not effectively target the causes of dosing errors and could introduce new types of error. Our study examines the frequency, magnitude, and immediate causes of PMA dosing errors in emergency medical services (EMS) after implementation of the MI-MEDIC dosing reference aid. METHODS: We used a mixed method, mobile simulation-based approach to examine PMA dosing errors. Emergency medical services crews w

Data quality in a survey of registered medical cannabis users with cancer: nonresponse and measurement error.

Cannabis use among individuals with cancer is best understood using survey self-report. As cannabis remains federally illegal, surveys could be subject to nonresponse and measurement issues impacting data quality. We surveyed individuals using medical cannabis for a cancer-related condition in the Minnesota Medical Cannabis Program (MCP). Although survey responders are older, there are no differences by race and ethnicity, gender, or receipt of reduced cannabis registry enrollment fee. Responders made a more recent purchase and more recently completed an independent symptom assessment for the

Screening for primary aldosteronism: How to adjust existing antihypertensive medications to avoid diagnostic errors.

BACKGROUND: Screening for primary aldosteronism is infrequently performed in primary care. This is partly because screening is complicated by the need to adjust existing antihypertensive medications. This article provides an approach to screening patients who are already taking antihypertensive medication. OBJECTIVE: The objective of this article is to describe how to alter antihypertensive medications to allow accurate screening for primary aldosteronism. DISCUSSION: The ideal time to screen for primary aldosteronism is prior to initiating antihypertensive medications. If the patient is alr

Catatonia is not schizophrenia: Kraepelin's error and the need to recognize catatonia as an independent syndrome in medical nomenclature.

Catatonia is a motor dysregulation syndrome described by Karl Kahlbaum in 1874. He understood catatonia as a disease of its own. Others quickly recognized it among diverse disorders, but Emil Kraepelin made it a linchpin of his concept of dementia praecox. Eugen Bleuler endorsed this singular association. During the 20th century, catatonia has been considered a type of schizophrenia. In the 1970s, American authors identified catatonia in patients with mania and depression, as a toxic response, and in general medical and neurologic illnesses. It was only occasionally found in patients with schi

Statistical process control within the Six Sigma DMAIC framework for monitoring medication dispensing errors in hospital pharmacies: a Saudi case study.

This study employed the Six Sigma DMAIC methodology to monitor and analyze medication dispensing errors in hospital pharmacies in Saudi Arabia, with a focus on Mecca and Jeddah cities. Using aggregated data from the Ministry of Health (MOH) Statistical Yearbook 2024, a synthetic dataset was generated through a Conditional Tabular Generative Adversarial Network (CTGAN) to enable the Statistical Process Control (SPC) analysis. The p-chart application demonstrated how process stability can be evaluated within a synthetic healthcare dataset, while the calculated sigma level (2.49) represents the p
