Bluetooth wireless database for scoliosis clinics.

A database system with Bluetooth wireless connectivity has been developed so that scoliosis clinics can be run more efficiently and data can be mined for research studies without significant increases in equipment cost. The wireless database system consists of a Bluetooth-enabled laptop or PC and a Bluetooth-enabled handheld personal data assistant (PDA). Each patient has a profile in the database, which has all of his or her clinical history. Immediately prior to the examination, the orthopaedic surgeon selects a patient's profile from the database and uploads that data to the PDA over a Blue

Effects of computerized decision support system implementations on patient outcomes in inpatient care: a systematic review.

OBJECTIVES: To systematically classify the clinical impact of computerized clinical decision support systems (CDSSs) in inpatient care. MATERIALS AND METHODS: Medline, Cochrane Trials, and Cochrane Reviews were searched for CDSS studies that assessed patient outcomes in inpatient settings. For each study, 2 physicians independently mapped patient outcome effects to a predefined medical effect score to assess the clinical impact of reported outcome effects. Disagreements were measured by using weighted kappa and solved by consensus. An example set of promising disease entities was generated ba

Iatrogenic injury and unexpected hospital death in the newborn.

Neonates are unusually vulnerable to iatrogenic injury due to small body size, delicate tissues, and immature immune systems. Investigation of an unexpected neonatal death in the hospital should begin with a review of the medical record and discussion with medical staff involved in the patient׳s care. Postmortem investigation should include a complete and well-documented autopsy. Additional investigations, such as microbiological studies and chemical and toxicological studies of postmortem and antemortem fluid samples, may be crucial in arriving at a diagnosis. Causes of iatrogenic injury incl

Patient Safety During Transitions: Identifying and Mitigating Risks.

Transitions of care, such as intrahospital handoffs and hospital discharge, are high-risk periods for adverse events like medication errors and diagnostic delays, often stemming from communication lapses and care fragmentation. Evidence-based strategies, including multidisciplinary team collaboration, standardized handoff protocols, and electronic health record tools, can mitigate these risks and improve patient safety. Emerging technologies, such as artificial intelligence and remote monitoring systems, offer opportunities to personalize care and provide real-time support. Engaging patients a

Noncompliance with medication regimens in severely and persistently mentally ill schizophrenic patients.

Medication noncompliance contributes significantly to recurrence of symptoms and readmission to the hospital of schizophrenic patients. The purpose of this study was to determine factors identified by patients, family members, and nurses for patients' noncompliance. The Health Belief Model provided a theoretical framework. The sample consisted of 11 triads with a noncompliant schizophrenic patient, a family member, and a primary nurse in each triad. A structured interview was developed to assess stated reasons for noncompliance and factors relating to the patient's illness, medication practice

Implementation of computerized provider order entry in a neonatal intensive care unit: Impact on admission workflow.

OBJECTIVE: The study objective was to determine if computerized provider order entry (CPOE) systems impaired or enhanced workflow in the neonatal intensive care unit (NICU) by comparing the timing of administration of the first dose of antibiotics before and after CPOE system implementation. METHODS: We conducted a pre-post intervention comparative study of the length of time between admission and administration of initial antibiotics in neonates before and after a CPOE system was implemented. Clinical information and timing of antibiotic administration were collected on all inborn infants, w

Primary anesthesia provider characteristics and risk factors for intraoperative medication errors: a retrospective cohort study.

BACKGROUND: Intraoperative medication errors, although uncommon, can result in considerable patient harm. Evidence remains limited regarding anesthesia provider-level and perioperative risk factors. This study aimed to evaluate whether anesthesia provider characteristics-particularly experience level and team composition-are statistically associated with intraoperative medication errors after multivariable adjustment. METHODS: We conducted a retrospective observational study of 100,093 surgical cases managed under anesthesia at a university-affiliated tertiary hospital in Japan between August

Live stream of prehospital point-of-care ultrasound during cardiopulmonary resuscitation - A feasibility trial.

BACKGROUND: Current resuscitation guidelines recommend that skilled persons could use ultrasound to detect reversible causes during cardiopulmonary resuscitation (CPR) where the examination can be safely integrated into the Advanced Life Support (ALS) algorithm. However, in a prehospital setting performing and rapidly interpreting ultrasound can be challenging for physicians. Implementing remote, expert-guided, and real-time transmissions of ultrasound examinations offers the opportunity for tele-support, even during an out-of-hospital cardiac arrest (OHCA). The aim of this feasibility study w

Post-analytical errors with portable glucose meters in the hospital setting.

Point-of-care testing (POCT) for blood glucose is widely used in different clinical settings and, thanks to its rapid turnaround time (TAT), it is commonly recommended in patients with critical conditions calling for urgent monitoring. However data in the literature on its reliability and safety are scarce. The aim of the present study was therefore to investigate the type, frequency and clinical risks associated with POCT using portable glucose meters in the hospital setting by evaluating the post-analytical phase when the data are manually transcribed in patients' record. We checked all regi

A review of verbal order policies in acute care hospitals.

BACKGROUND: Although verbal and telephone orders (VOs) are commonly used in the patient care process, there has been little examination of the strategies and tactics used to ensure their appropriate use or how to ensure that they are accurately communicated, correctly understood, initially documented, and subsequently transcribed into the medical record and ultimately carried out as intended. A systematic review was conducted of hospital verbal and telephone order policies in acute care settings. METHODS: A stratified random sample of hospital verbal and telephone order policy documents were

Understanding Relapse in Bipolar Disorder at a Tertiary Mental Health Facility in Uganda.

BACKGROUND: Bipolar disorder (BD) is a chronic psychiatric condition characterized by recurrent episodes of mania, hypomania, and depression, affecting approximately 1 in 150 adults globally. BD contributes significantly to disability and years of life lost, with a notable prevalence in Africa. The high relapse rates in BD, exacerbated by limited mental health resources and underdiagnosis, highlight the need for targeted interventions. This study explores the factors contributing to relapse among BD patients at Butabika National Referral Mental Hospital, Uganda, a crucial mental health facilit

Standardization as a mechanism to improve safety in health care.

BACKGROUND: A growing body of literature shows that when patterns of care are widely divergent, clinical outcomes suffer and, as a result, safety may be compromised. A multispecialty group at Luther Midelfort, Mayo Health System (LM, MHS) initiated efforts to reduce variance in the clinical practice patterns of providers. The pilot initiative, which entailed standardization of a sliding-scale insulin protocol, served as a template throughout the LM, MHS for reducing variance and enhancing safety. STANDARDIZING INSULIN ADMINISTRATION: A single sliding-scale insulin protocol for regular insulin

Prehospital ABC (Age, Bystander and Cardiogram) scoring system to predict neurological outcomes of cardiopulmonary arrest on arrival: post hoc analysis of a multicentre prospective observational study.

BACKGROUND: There is currently limited evidence to guide prehospital identification of patients with cardiopulmonary arrest on arrival (CPAOA) to hospital who have potentially favourable neurological function. This study aimed to develop a simple scoring system that can be determined at the contact point with emergency medical services to predict neurological outcomes. METHODS: We analysed data from patients with CPAOA using a regional Japanese database (SOS-KANTO), from January 2012 to March 2013. Patients were randomly assigned into derivation and validation cohorts. Favourable neurological

The effect of electronic medical record application on the length of stay in a Chinese general hospital: a department- and disease-focused interrupted time-series study.

A key purpose of electronic medical records (EMR) introduced in medical institutions is to improve work efficiency. The average length of stay (LOS) is just an important indicator to evaluate work efficiency of medical care in hospitals. Recently, there have been reports about effects of EMR application on LOS in medical institutions, but they have been mostly based on the overall analysis of a region or a hospital and not of specific clinical departments and diseases or based on longer time periods. Therefore, in this study, we selected four clinical departments and four diseases with the lar

Linking the Virtual Pediatric System and Pediatric Hospital Information System Databases to Create Novel PICU Research Datasets.

BACKGROUND: Lack of a robust, national PICU database hinders epidemiologic and quasi-experimental research of PICU care. Combining the virtual pediatric system (VPS) and pediatric health information system (PHIS) databases would overcome this data gap. However, limits on the use of variables containing protected health information (PHI) have prevented prior large-scale VPS-PHIS linkage. We sought to link VPS and PHIS records from 31 hospitals without using PHI. METHODS: We performed a database linkage study utilizing a sequential direct + probabilistic linkage approach with records from 31 ho

Diversification of health care services: the effects of ownership, environment, and strategy.

The present findings suggest that the trend toward greater diversification of hospital services is likely to be most strongly influenced by state Medicaid policies and certain hospital characteristics. Increasing Medicaid eligibility and payment levels is likely to have a positive effect on services diversification. Growth in the number of inpatient services provided and a more severe case mix are also likely to be involved with greater service diversification. Affiliation with a not-for-profit hospital system is likely to be associated with more diversified hospital services but not affiliati

An analysis of administrative data found that proximate clinical event ratios provided a systematic approach to identifying possible iatrogenic risk factors or complications.

OBJECTIVE: A method to generate hypotheses about iatrogenic risk factors and complications from administrative data was developed and tested using hospitalization of the elderly for depression as a model. STUDY DESIGN AND SETTING: Hospital claims were selected for 30,998 elderly inpatients admitted for the first time for depression. Common principal diagnoses and procedures in hospitalizations within 90 days of the index depression admission were tallied. For each of these proximate clinical events, the ratio of how many happened before the index admission to how many occurred afterward was c

Computer based nursing documentation means to achieve the goal.

New functions have been integrated in the Giessen Hospital Information System WING to support the classification of all intensive care patients into the Therapeutic Intervention Scoring System (TISS). The use of those functions has been pushed when health insurance bodies demanded evidence for the correct classification of ICU beds. This article presents an overview on this development from the start in just one intensive care unit to the complete coverage of six intensive care units and three intensive monitoring units with a total of 109 beds. For those units complete TISS data has been docu
