Respiratory viral infections, including human metapneumovirus (hMPV), are increasingly recognized as a significant cause of morbidity and mortality in lung transplant recipients (LTRs). We present a case of hMPV pneumonia in a bilateral LTR, likely transmitted by the donor, resulting in severe acute respiratory distress syndrome and leading to death. Next-generation sequencing of hPMV genomes was performed from both the recipient's bronchoalveolar lavage and the organ conservation liquid and strongly supported the presence of donor-derived infection. There is still limited reported evidence to

The host cytokine IL-6 plays an important role in host defence and prevention of lung injury from various pathogens, making IL-6 an important mediator in the host's susceptibility to respiratory infections. The cellular response to IL-6 is mediated through a Janus kinase/signal transducer and activator of transcription 3 (JAK/STAT3) signal transduction pathway. Human metapneumovirus (hMPV) is an important causative agent of viral respiratory infections known to inhibit the IFN-mediated activation of STAT1. However, little is known about the interactions between this virus and other STAT signal

Diffuse alveolar haemorrhage (DAH) is a rare but potentially life-threatening syndrome with diverse etiologies. Human metapneumovirus (hMPV) infection has rarely been reported as a cause of DAH. We report two cases of DAH associated with hMPV infection in immunocompetent adults. In both patients, bronchoalveolar lavage fluid (BALF) showed progressively haemorrhagic returns and hemosiderin-laden macrophages, consistent with DAH. Multiplex polymerase chain reaction (PCR) using BALF detected hMPV, while extensive investigations excluded autoimmune, bacterial, and other viral causes. These cases s

Respiratory infections by human metapneumovirus (HMPV) are common in children, those with weakened immune systems, and older people. With its important role in viral entry, viral fusion (F) glycoprotein is a prime target for designing drugs. To discover new inhibitors of the HMPV fusion protein as a class of drugs that can target this protein and stop it from causing disease, this study employs a computational drug design approach that includes density functional theory (DFT), molecular dynamics (MD), and machine learning (ML). With the help of molecular dynamics simulations, this study verifi

The prefusion conformation of human metapneumovirus fusion protein (hMPV Pre-F) is critical for eliciting the most potent neutralizing antibodies and is the preferred immunogen for an efficacious vaccine against hMPV respiratory infections. Here we show that an additional cleavage event in the F protein allows closure and correct folding of the trimer. We therefore engineered the F protein to undergo double cleavage, which enabled screening for Pre-F stabilizing substitutions at the natively folded protomer interfaces. To identify these substitutions, we developed an AI convolutional classifie

Ornithobacterium rhinotracheale is an avian respiratory pathogen that affects turkeys. The objective of this study was to evaluate the clinical efficacy of gamithromycin (GAM) against O. rhinotracheale in turkeys. The birds were inoculated oculonasally with 10(8) colony-forming units (cfu) of O. rhinotracheale, preceded by infection with avian metapneumovirus. In addition to a negative (CONTR-) and a positive control group (CONTR+) there were two treated groups administered GAM (6 mg/kg) either subcutaneously (GAM SC) or orally (GAM PO) by administration as a single bolus at one-day post-bacte

Human metapneumovirus (hMPV) is a common cause of respiratory tract infection in the paediatrics population. Recently, we and others have shown that retinoic acid-inducible gene 1 (RIG-I)-like receptors (RLRs) are essential for hMPV-induced cellular antiviral signalling. However, the contribution of those receptors to host immunity against pulmonary hMPV infection is largely unexplored. In this study, mice deficient in mitochondrial antiviral-signalling protein (MAVS), an adaptor of RLRs, were used to investigate the role(s) of these receptors in pulmonary immune responses to hMPV infection. Human metapneumovirus (HMPV) is a significant pathogen, causing widespread acute respiratory infections (ARIs). Rapid and accurate detection is crucial for timely diagnosis and outbreak control. To address the limitations of real-time reverse transcription quantitative polymerase chain reaction (RT-qPCR)-based HMPV detection, we developed a novel method integrating recombinase-aided amplification (RAA) with CRISPR-Cas13a technology. Based on the alignment of 335 HMPV whole-genome sequences, a conserved region of the nucleocapsid (N) gene was selected for RAA primer and CRISPR RNA (crRNA) desig

UNLABELLED: To examine factors associated with severe respiratory disease at initial presentation in pediatric hMPV-associated LRTIs. We analyzed patients aged 1 month to 18 years with PCR-confirmed hMPV-associated LRTI (January 2018-January 2024). Clinical severity was stratified via the Modified Tal Score at admission (mild ≤ 5, moderate 6-10, severe ≥ 11). Each patient contributed one episode. Multivariable binary logistic regression identified risk factors for severe respiratory disease; ROC analysis evaluated biomarker diagnostic accuracy. A total of 676 hMPV-positive patients were identi

OBJECTIVE: Human metapneumovirus (HMPV) is a clinically important respiratory pathogen causing severe lower respiratory tract infections, particularly in infants, elderly individuals, and immunocompromised patients. Despite its substantial disease burden and global circulation, no HMPV-specific antiviral therapy has been approved to date. This unmet clinical need underscores the importance of identifying rapidly translatable therapeutic strategies. The present study aimed to identify potential inhibitors of the HMPV fusion (F) protein through a structure-based drug repurposing approach, levera

Background: Human metapneumovirus (hMPV) is a respiratory pathogen that causes illness ranging from mild upper respiratory tract infections to severe pneumonia, particularly in individuals with comorbidities. Fatal cases of hMPV-induced hemorrhagic pneumonia are rare and likely under-reported. Diagnosis is often delayed due to overlapping symptoms with other respiratory viruses and the rapid progression of the disease. Case presentation: We report the case of a 55-year-old man with a complex medical history, including liver cirrhosis and diabetes mellitus, who developed acute viral pneumonia.

BACKGROUND: Human metapneumovirus (HMPV) is a recently discovered respiratory pathogen of the family Paramyxoviridae, the same family as that of respiratory syncytial virus (RSV). Premature children are at high risk of severe RSV infections, however, it is unclear whether HMPV infection is more severe in hospitalized children with a history of severe prematurity.
METHODS: We conducted a retrospective analysis of the clinical respiratory presentation of all polymerase chain reaction-confirmed HMPV infections in preschool-age children (≤5 years) with and without history of severe prematurity (<
