A trend analysis in referrals during pregnancy and labour in Dutch midwifery care 1988-2004.

OBJECTIVE: To assess the trends and patterns of referral from midwives to obstetricians within the Dutch maternity care system from 1988 to 2004, and the differences in referral patterns between nulliparous and parous women. DESIGN: A descriptive study. SETTING: The Dutch midwifery database (LVR1), which monitored 74% (1988) to 94% (2004) of all midwifery care in the Netherlands between 1988 and 2004. POPULATION: A total of 1 977 006 pregnancies, attended by a primary care level midwife. METHODS: The indications for referral from midwifery to obstetric care were classified into fifteen gro

Quality of intrapartum midwifery care in Austria: Implications for birth satisfaction and maternal mental health.

PROBLEM: A substantial proportion of people who give birth experience childbirth as negative or traumatic, which may adversely affect postpartum mental health. BACKGROUND: Respectful, supportive and woman-centred intrapartum care is a recognised determinant of positive birth experiences, yet evidence linking perceived midwifery care quality with birth satisfaction and trauma-related outcomes in Austria remains limited. AIM: To examine the associations between perceived quality of intrapartum midwifery care, birth satisfaction and birth-related post-traumatic stress disorders (PTSD). METHODS

Autistic persons in the perinatal context: A mapping review of midwifery care, experiences, needs, and evidence.

BACKGROUND: Autistic people represent a substantial and probably under-recognised population within reproductive and maternity services. Maternity care nevertheless continues to rely on neurotypical assumptions about communication, sensory tolerance, pain expression, emotional display, and help-seeking. AIM: To map the scope, distribution, and thematic content of literature on autistic people's experiences and support needs across pregnancy, childbirth, postpartum care, and infant feeding, and to identify implications for maternity service design and midwifery practice. METHODS: A mapping re

"United in diversity?" A comparative study of initial midwifery education and practice in 30 European countries.

AIM: To explore initial midwifery education and scope of professional practice across Europe. BACKGROUND: Free movement of professionals is a core value of the European Union, enabling midwives to practice across European countries. The European Directives established minimum standards for midwifery education. However, various health care delivery crises may challenge professional competencies and legal responsibilities. DESIGN: An exploratory research design was used to obtain overall insight into various issues not well understood about midwifery education and practice in European countrie

Perception of simulation-based education among nursing and midwifery students in Tanzania: a qualitative study.

BACKGROUND: While many nursing programs in developed countries have implemented simulation-based education as a pedagogic method of teaching, implementation of simulation in developing countries like Tanzania is rare. Traditional methods of auditorium lectures are widely conducted in low-income nursing and midwifery education institutions. Such pedagogy provides students with theoretical knowledge yet with limited hands-on exposure for clinical skills, which might affect the professional integration of students and quality care delivery. This study explored perceptions of simulation-based educ

A survey of students' experiences of returning to midwifery studies after maternity leave: A pilot survey.

AIM: To identify the barriers and facilitators having an impact on the progression and completion of studies for students who give birth during their midwifery program. BACKGROUND: Balancing the demands of pregnancy and new motherhood with the rigorous academic requirements of a tertiary-level midwifery program is challenging for students wishing to progress and complete their studies. Understanding the barriers and facilitators students face when resuming midwifery studies following birth can assist universities in providing educational environments that support students in achieving their m

Implementation of Virtual Antenatal and Postnatal Urgent Midwifery Visits: Evaluation of a Quality Improvement Initiative.

Women seeking care during the perinatal period often face delays or long waits at healthcare facilities due to lack of providers and/or resources, leading to sub-optimal outcomes. We implemented a program whereby patients with concerns could receive same-day care virtually from a midwife rather than presenting to the clinic or hospital for care. Implementation strategies included virtual training, a staged increase in patient volume, and frequent communication between the midwives via text, email, and monthly meetings. Virtual visits included a variety of complaints, the five most common being

Clinical application of airbag bionic midwifery technology in vaginal delivery of pregnancy with scarred uterus.

This was an original article, mainly explored the effect of applying airbag bionic midwifery technology in vaginal delivery of scarred uterus pregnancy. Sixty patients were chosen, and divided into an intervention group (IG) and a control group (CG). The results showed that in contrast to the CG, the IG had shorter first, second and total stages of labor (P<0.01), less amounts of blood loss during delivery and 2 h after delivery (P<0.01), lower visual analogue scale (VAS) score (P<0.01), higher natural delivery rate, lower forceps assisted delivery rate and cesarean section rate (P<0.05), lowe

The psychological support for women who underwent a stillbirth during their pregnancy: the quality of midwifery care.

PURPOSE: This review aims to investigate the role of midwifery care in perinatal death. Specifically, it aims to investigate the type and implications in the clinical practice of psychological and psychiatric support interventions for women/couples. METHODS: A scoping review was conducted following the PRISMA methodology. For this purpose, the following databases were queried: PubMed, APA PsycInfo, CINAHL Plus with Full Text, and ERIC, considering only studies published in the 2002-2022 time frame. RESULTS: 14 studies were found to be eligible by the literature review. These researches were

Midwifery as a Future Career: Determinants of Motivation Among Prep Students in Harar, Eastern Ethiopia.

BACKGROUND: Midwifery is a profession that deals with care and advice during pregnancy, labor, childbirth and postpartum period including support for the newborn. Like other professions, the sustainability of midwives depends on recruiting new professionals who are inspired to train as their future career. In this regard, the inspiration of preparatory students to embracing the profession and secure the future midwife workforce is critical. In Ethiopia, there is no literature on the assessment of students' intention toward the midwifery profession. Hence, this study is crucial to fill data sca

Cost-effectiveness of continuity of midwifery care for women with complex pregnancy: a structured review of the literature.

BACKGROUND: Critical evaluation of the cost-effectiveness and clinical effectiveness of continuity of midwifery care models for women experiencing complex pregnancy is an important consideration in the review and reform of maternity services. Most studies either focus on women who experience healthy pregnancy or mixed risk samples. These results may not be generalised across the childbearing continuum to women with risk factors. This review critically evaluates studies that measure the cost of care for women with complex pregnancies, with a focus on method and quality. AIMS / OBJECTIVES: To c

The Cost of Nurse-Midwifery Care: Use of Interventions, Resources, and Associated Costs in the Hospital Setting.

INTRODUCTION: Obstetrical care often involves multiple expensive, and often elective, interventions that may increase costs to patients, payers, and the health care system with little effect on patient outcomes. The objectives of this study were to examine the following hospital related outcomes: 1) use of labor and birth interventions, 2) inpatient duration of stay, and 3) total direct health care costs for patients attended by a certified nurse-midwife (CNM) compared with those attended by an obstetrician-gynecologist (OB-GYN), within an environment of safe and high-quality care. Client perspectives of midwifery care in the transition from subfertility to parenthood: a qualitative study in the Netherlands.

BACKGROUND: Pregnancy, childbirth and the postpartum period after fertility treatment are considered "normal" in the Netherlands, with no indication of an increased obstetric risk, and can therefore be monitored by a primary care midwife. However, there is little evidence on the experiences of couples and women who finally get pregnant after fertility treatment and a lack of training for midwives exists on this subject. The aim of this study was to map the midwifery care needs of the subfertile client with past fertility problems. METHODS: In 2011, we interviewed two couples and seven women w

Nursing and midwifery practice for maintenance of vascular access device patency. A cross-sectional survey.

BACKGROUND: Up to 85% of hospital in-patients will require some form of vascular access device to deliver essential fluids, drug therapy, nutrition and blood products, or facilitate sampling. The failure rate of these devices is unacceptably high, with 20-69% of peripheral intravenous catheters and 15-66% of central venous catheters failing due to occlusion, depending on the device, setting and population. A range of strategies have been developed to maintain device patency, including intermittent flushing. However, there is limited evidence informing flushing practice and little is known abou

The impact and cost of scaling up midwifery and obstetrics in 58 low- and middle-income countries.

BACKGROUND AND METHODS: To guide achievement of the Millennium Development Goals, we used the Lives Saved Tool to provide a novel simulation of potential maternal, fetal, and newborn lives and costs saved by scaling up midwifery and obstetrics services, including family planning, in 58 low- and middle-income countries. Typical midwifery and obstetrics interventions were scaled to either 60% of the national population (modest coverage) or 99% (universal coverage). FINDINGS: Under even a modest scale-up, midwifery services including family planning reduce maternal, fetal, and neonatal deaths by

Preventive nursing of neonatal clavicular fracture in midwifery: a report of six cases and review of the literature.

PURPOSE: To summarize and analyze the obstetric factors and medical care for neonatal clavicle fracture during delivery. MATERIALS AND METHODS: In 4,456 vaginal deliveries, only six newborns were found with a clavicle fracture in our hospital from October 2002 to October 2011. RESULTS: Clinical findings showed that dystocia and improper midwifery manoeuvres are the two major reasons which lead to newborn clavicular fractures. CONCLUSION: More attention should be paid to non-violent traction and proper treatment of shoulder dystocia.

Continuity of midwifery care and gestational weight gain in obese women: a randomised controlled trial.

BACKGROUND: The increased prevalence of obesity in pregnant women in Australia and other developed countries is a significant public health concern. Obese women are at increased risk of serious perinatal complications and guidelines recommend weight gain restriction and additional care. There is limited evidence to support the effectiveness of dietary and physical activity lifestyle interventions in preventing adverse perinatal outcomes and new strategies need to be evaluated. The primary aim of this project is to evaluate the effect of continuity of midwifery care on restricting gestational w
