Platelet Effects of Anti-diabetic Therapies: New Perspectives in the Management of Patients with Diabetes and Cardiovascular Disease.

In type 2 diabetes, anti-thrombotic management is challenging, and current anti-platelet agents have demonstrated reduced efficacy. Old and new anti-diabetic drugs exhibited-besides lowering blood glucose levels-direct and indirect effects on platelet function and on thrombotic milieu, eventually conditioning cardiovascular outcomes. The present review summarizes existing evidence on the effects of glucose-lowering agents on platelet properties, addressing pre-clinical and clinical research, as well as drug-drug interactions with anti-platelet agents. We aimed at expanding clinicians' understa

An in-vitro assay using human spermatozoa to detect toxicity of biologically active substances.

Identifying the key toxic players within an in-vivo toxic syndrome is crucial to develop targeted therapies. Here, we established a novel method that characterizes the effect of single substances by means of an ex-vivo incubation set-up. We found that primary human spermatozoa elicit a distinct motile response on a (uremic) toxic milieu. Specifically, this approach describes the influence of a bulk toxic environment (uremia) as well as single substances (uremic toxins) by real-time analyzing motile cellular behavior. We established the human spermatozoa-based toxicity testing (HSTT) for detect

Neutropenic typhlitis in adults.

Typhlitis is a necrotizing process of the cecum, occurring in neutropenic patients who are usually receiving combined chemotherapy. The atonic cecum is affected because of its poor arterial perfusion, the presence of colonic bacteria, and the milieu of immunosuppression. It is a serious but not inevitably fatal complication of the patient's basic illness. Resection of necrotic bowel is necessary. Because this will usually be done on an emergency basis in a very ill patient with unprepared bowel, a two-stage procedure rather than primary anastomosis is advisable. Surgical results may be better

Iatrogenic disease, drug metabolism, and cell injury: lethal synthesis in man.

One important consideration to be appreciated from the examples of "lethal synthesis" presented in this paper is that the mechanisms of toxic substances must be further explored, a true understanding of the complex modalities of cellular injury achieved, and the significance of cellular changes determined before we can preestablish the hazards of chemical or environmental contaminants. The identification of compounds that may modify the intracellular milieu by adduction of marcromolecules and those that may be activated to produce proximate toxins could be accomplished on the basis of their di

From mechanotransduction to manual therapy: advances in piezo/TRP channels and lumbar degeneration.

Degenerative lumbar spondylolisthesis and related lumbar instability involve more than segmental biomechanical imbalance. They also involve disturbed mechanotransduction mediated by mechanosensitive ion channels. This review brings together basic, mechanobiological, and clinical studies on Piezo1, Piezo2, and TRPV4 in the intervertebral disc, cartilage endplate, facet joint, ligamentum flavum, and sensory afferent system, and considers how manual therapy might affect segmental kinematics, load transfer, and the local microenvironment. Piezo1 is most often linked to load-driven amplification of

A Narrative Review of the Role of Immunotherapy in Metastatic Carcinoma of the Colon Harboring a BRAF Mutation.

Patients affected by metastatic carcinoma of the colon/rectum (mCRC) harboring mutations in the BRAF gene (MBRAF) respond poorly to conventional therapy and have a prognosis worse than that of patients without mutations. Despite the promising outcomes of targeted therapy utilizing multi-targeted inhibition of the mitogen-activated protein kinase (MAPK) signaling system, the therapeutic efficacy, especially for the microsatellite stable/DNA proficient mismatch repair (MSS/PMMR) subtype, remains inadequate. Patients with MBRAF/mCRC and high microsatellite instability or DNA deficient mismatch re

Autologous stem cell transplantation meets CAR-T therapy: A synergistic strategy for B-cell lymphoma.

B-cell lymphoma, the most common subtype of non-Hodgkin lymphoma, presents major therapeutic challenges due to molecular heterogeneity and high relapse rates. While autologous hematopoietic stem cell transplantation (ASCT) has been a cornerstone for relapsed/refractory (R/R) B-cell lymphoma, its efficacy is often compromised by minimal residual disease (MRD) persistence and an immunosuppressive tumor microenvironment. Chimeric antigen receptor (CAR)-T cell therapy has transformed treatment paradigms but faces limited long-term durability due to antigen escape and T-cell exhaustion. The integra

Single cell transcriptional profiling of monocytes from asthma patients show a predisposition for an inflammatory response.

BACKGROUND: Asthma is the most common chronic lung disorder among children and young adults worldwide. Despite effective new biologic therapies, ∼10% of patients remain poorly controlled. Monocytes contribute to the proinflammatory milieu in asthmatic lungs but also suppress inflammation and promote tissue healing. AIMS: Compare the transcriptome of blood monocytes in asthma patients and healthy controls and assess the impact of an infusion of interferon y-activated mesenchymal stromal cells (yMSCs) on the transcriptome of asthmatic monocytes. METHODS: scRNA-seq was conducted on blood monocy

Glucose-6-phosphate dehydrogenase deficiency: an etiology for idiopathic priapism?

INTRODUCTION: Efforts to identify the health risk associations for priapism may reveal pathophysiologic mechanisms for the disorder and suggest a scientifically rational approach for correcting it. AIM: We describe a clinical presentation of idiopathic recurrent priapism in a patient with glucose-6-phosphate dehydrogenase (G6PD) deficiency and consider a possible nitric oxide (NO)-dependent mechanistic basis from which the medical condition causes priapism. METHODS: The case report profiled a 35-year-old African-American man with G6PD deficiency who presented with a rapid progression of recu

Sweet's syndrome in the setting of CD34-positive acute myelogenous leukemia treated with granulocyte colony stimulating factor: evidence for a clonal neutrophilic dermatosis.

BACKGROUND: Sweet's syndrome in the setting of hematologic dyscrasias can be categorized into paraneoplastic-associated SS, drug-induced SS, and SS with leukemia cutis. Apart from those cases demonstrating concomitant leukemic infiltrates, it has been surmised that SS is a reactive phenomenon induced by a specific cytokine milieu. METHODS: The authors present a patient with CD34+ acute myelogenous leukemia (AAML) who developed SS in the setting granulocyte colony stimulating factor (GCSF) therapy. Routine light microscopy and molecular studies were carried on the patient's skin biopsy specime

Myeloid-derived suppressor cells in multiple myeloma: mechanistic insights and therapeutic implications.

Multiple myeloma (MM) is an incurable malignancy and ranks as the second most common hematological cancer in western countries. Relapsed disease is common in MM patients, and is largely influenced by the establishment of an immunosuppressive tumor microenvironment (TME) and concomitant systemic immune impairment. Myeloid-derived suppressor cells (MDSCs) are crucial regulators of immune homeostasis and response in various pathological states, including cancer. Within the TME, MDSCs and tumor cells engage in bidirectional crosstalk through both direct and indirect mechanisms, collectively drivin

Moonlighting chromatin: when DNA escapes nuclear control.

Extracellular chromatin, for example in the form of neutrophil extracellular traps (NETs), is an important element that propels the pathological progression of a plethora of diseases. DNA drives the interferon system, serves as autoantigen, and forms the extracellular scaffold for proteins of the innate immune system. An insufficient clearance of extruded chromatin after the release of DNA from the nucleus into the extracellular milieu can perform a secret task of moonlighting in immune-inflammatory and occlusive disorders. Here, we discuss (I) the cellular events involved in the extracellular

Unconjugated hyperbilirubinemia in very low birth weight infants.

In very low birth weight infants, the occurrence of bilirubin-related brain damage has been repeatedly observed at low serum bilirubin concentrations in close association with altered pathophysiologic status (hypoxia, acidosis, hypothermia, and so on). This increased susceptibility is accompanied by increased severity and duration of unconjugated hyperbilirubinemia as compared with more mature infants. Clinical manifestations of kernicterus in very low birth weight infants are almost always nonspecific. No single biochemical or physiologic measurement is sufficient to predict the risk for deve

The first seventy admissions to an adolescent unit in Edinburgh: general characteristics and treatment outcome.

1. The clinical functioning of the Young People's Unit, Edinburgh is described. This involves a relatively permissive, democratic "milieu". Psychotherapeutic efforts are devoted to the resolution of family psychopathology and the achievement of maturational progress for the adolescent. 2. Data on the first seventy admittions to the Unit were examined. Two-thirds of the patients were diagnosed "developmental crisis"; the majority of these also had a personality disorder. 3. Assessments of family functioning showed high levels of disturbance and psychiatric morbidity among the parents. The adole

Therapy of myositis: biological and physical.

PURPOSE OF REVIEW: To give an update on reported use and effects of biological and physical therapies in patients with myositis. RECENT FINDINGS: The most promising biological treatment in polymyositis, dermatomyositis and juvenile dermatomyositis is B-cell blockade by rituximab. Anti-Jo or anti-Mi-2 antibodies were predictors of response suggesting different molecular pathways in different subsets of myositis. T-cell blockade with abatacept is a new possibility, as is blockade of interleukin-1, interleukin-6 or type I interferon, but controlled studies are needed. Metabolic abnormalities may

Therapies for neonatal encephalopathy: Targeting the latent, secondary and tertiary phases of evolving brain injury.

In term and near-term neonates with neonatal encephalopathy, therapeutic hypothermia protocols are well established. The current focus is on how to improve outcomes further and the challenge is to find safe and complementary therapies that confer additional protection, regeneration or repair in addition to cooling. Following hypoxia-ischemia, brain injury evolves over three main phases (latent, secondary and tertiary), each with a different brain energy, perfusion, neurochemical and inflammatory milieu. While therapeutic hypothermia has targeted the latent and secondary phase, we now need ther

Treatment of sickle cell trait hematuria with oral urea.

Unilateral hematuria is a vaso-occlusive complication of the sickle syndromes, especially the sickle cell trait. Treatment is usually ineffective, and the duration is variable, often prolonged and subject to recurrences. In sickle cell trait hematuria is usually the only consequence of sickling, possibly because the unique renal countercurrent mechanism may result in a milieu more prone to sickling in the medullary circulation than in the vasculature of other organs. Urea in vitro inhibits gelation of deoxygenated sickle hemoglobin. In 2 previously reported cases of sickle cell trait hematuria

Navigating the Hemostatic Balance: Anticoagulation and Antiplatelet Therapy in Patients with Thrombocytopenia.

Background: Thrombocytopenia is traditionally perceived as a bleeding-predominant condition; however, growing evidence indicates that many thrombocytopenic states are paradoxically associated with an increased risk of venous and arterial thrombosis. This dual hemostatic derangement poses major therapeutic challenges when anticoagulant or antiplatelet therapy is indicated, particularly in complex settings such as cancer-associated thrombosis, immune thrombocytopenia, and advanced liver disease. Methods: We conducted a narrative review of the literature published between January 2021 and May 202

Cuproptosis and Disulfidptosis Converge to Empower PD-L1 Checkpoint Therapy via Cadict-Induced PD-L1 Translation.

Immune checkpoint blockade (ICB) has emerged as a cornerstone of cancer therapy, yet its effectiveness remains restricted in PD-L1-low malignancies due to insufficient target expression. We herein develop the cuproptosis and disulfidptosis co-delivery targeted (Cadict) nanodrug, an epidermal growth factor receptor (EGFR)-targeted nanoplatform designed to co-induce cuproptosis and disulfidptosis, thereby synergistically augmenting tumor cytotoxicity and sensitizing cancers to anti-PD-L1 therapy. Cadict exploits copper-sulfur (Cu-S) coordination chemistry to co-deliver copper ions and cystine, w
