Vestibular and motor contributions to mobility: limitations of seniors awaiting discharge from hospital care.

BACKGROUND AND PURPOSE: Following hospitalization, seniors are at risk of impaired mobility and increased risk of falling, which can lead to injuries and re-admission. The primary purpose of this paper was to evaluate the ability of hospitalized seniors to use vestibular inputs for balance control. The secondary purpose was to examine the influence of vestibular function and lower limb muscle strength on mobility. METHODS: Experimental and correlation designs were used. Patients (aged 65-90 years), preparing for discharge from an inpatient geriatric rehabilitation unit, were recruited. Vestib

Estimating whole-body vibration limits of manual wheelchair mobility over common surfaces.

Whole-body vibration (WBV) experienced during manual wheelchair use was quantified across several types of terrain (tile, sidewalk, decorative bricks, expanded metal grates). Over-ground travel was controlled using a robotic propulsion system. Vibrations along the vertical axis were measured with a triaxial accelerometer mounted to the seat of the wheelchair. Root-mean-square acceleration values were compared to the health guidance exposure limits established by the European Council using the WBV calculator tool published by the Health and Safety Executive (HSE). Vibrations along the vertical

Brain-Computer Interfaces for Children With Complex Communication Needs and Limited Mobility: A Systematic Review.

Brain-computer interfaces (BCIs) represent a new frontier in the effort to maximize the ability of individuals with profound motor impairments to interact and communicate. While much literature points to BCIs' promise as an alternative access pathway, there have historically been few applications involving children and young adults with severe physical disabilities. As research is emerging in this sphere, this article aims to evaluate the current state of translating BCIs to the pediatric population. A systematic review was conducted using the Scopus, PubMed, and Ovid Medline databases. Studie

Handgrip Strength as a Complementary Test for Mobility Limitations Assessment in Acutely Hospitalized Oldest Old.

Clinically viable screening tools for detecting individuals at heightened risk for mobility limitations is warranted. However, it remains unclear in acutely hospitalized patients (>75 years) whether handgrip strength might be a good predictor for "proxy" muscle strength. To examine the reliability and validity of handgrip strength test in acute hospitalized older adults in comparison with other standardized muscular strength tests, and to examine its ability to discriminate between presence and absence of mobility limitations. Association between obesity and the aggravation of limited range of ankle mobility in chronic venous disease.

AIM: The aim of the present study was to show that an increase in weight leads to the aggravation of limited joint mobility. METHOD: One hundred sixteen patients with varicose veins of the lower limbs and body mass index (BMI) higher than 30 kg/m2 were evaluated at the Belczak Clinic in Maringá, Brazil. All patients were evaluated by the same physician, and the goniometric readings were performed on all patients by a physiotherapist. The participants were then divided into three groups based on BMI: Group I-BMI between 30 and 40. Group II-BMI between 40 and 50, and Group III-BMI > 50. RESULT

Progressive Resistance Training Improves Torque Capacity and Strength in Mobility-Limited Older Adults.

BACKGROUND: Progressive resistance training (PRT) is consistently shown to improve muscle strength in older adults. The efficacy of PRT to improve muscle fatigue in older adults with demonstrated mobility limitations remains unclear. METHODS: Mobility-limited (Short Physical Performance Battery [SPPB] ≤ 9) older adults (age 70-92 years) were recruited for this study and randomized to either PRT or home-based flexibility (FLEX) 3 d/wk for 12 weeks. Muscle fatigue and strength outcomes were assessed at baseline and 12 weeks. The primary outcome was torque capacity, a composite measure of streng

What is a Clinically Meaningful Improvement in Leg-Extensor Power for Mobility-limited Older Adults?

BACKGROUND: Muscle power is a key predictor of physical function in older adults; however, clinically meaningful improvements in leg-extensor muscle power have yet to be identified. The purpose of this study is to establish the minimal clinically important improvement (MCII) and substantial improvement (SI) for leg-extensor power and muscle contraction velocity in mobility-limited older adults. METHODS: Data were extracted from three randomized trials of leg-extensor muscle power training interventions (3- to 6-month duration). Measurements of leg-extensor power and velocity were obtained usi

A comparison of straight- and curved-path walking tests among mobility-limited older adults.

BACKGROUND: Habitual gait speed (HGS) and the figure-of-8 walking test (F8WT) are measures of walking ability that have been associated with mobility outcomes and disability among older adults. Our objective was to contrast the physiologic, health, and behavioral attributes underlying performance of these two walking tests among older adults with mobility limitations. METHODS: HGS and F8WT were the primary outcomes. HGS was measured as time needed to walk a 4-m straight course at usual pace from standstill position. F8WT was measured as time to walk in a figure-of-8 pattern at self-selected u

Validity of an exercise test based on habitual gait speed in mobility-limited older adults.

OBJECTIVE: To evaluate whether a customized exercise tolerance testing (ETT) protocol based on an individual's habitual gait speed (HGS) on level ground would be a valid mode of exercise testing older adults. Although ETT provides a useful means to risk-stratify adults, age-related declines in gait speed paradoxically limit the utility of standard ETT protocols for evaluating older adults. A customized ETT protocol may be a useful alternative to these standard methods, and this study hypothesized that this alternative approach would be valid. DESIGN: We performed a cross-sectional analysis of
