Multiple myeloma in primary biliary cirrhosis.

A patient who had primary biliary cirrhosis and, simultaneously, multiple myeloma (IgG, lambda) is reported. In chronic liver diseases, polyclonal hypergammaglobulinaemia is common, monoclonal gammopathies have also been found in rare instances, being attributed to chronic antigenic stimulation of the reticuloendothelial system by antigens from the intestinal flora. The mechanisms (as chronic antigenic stimulation, immunologic disturbances and granulomata formation) which apply to explain the development of monoclonal gammopathies in other conditions are also found in primary biliary cirrhosis

Patient Barriers to Accessing Surgical Cleft Care in Vietnam: A Multi-site, Cross-Sectional Outcomes Study.

BACKGROUND: Most people who lack adequate access to surgical care reside in low- and lower-middle-income countries. Few studies have analyzed the barriers that determine the ability to access surgical treatment. We seek to determine which barriers prevent access to cleft care in a resource-limited country to potentially enable barrier mitigation and improve surgical program design. METHODS: A cross-sectional, multi-site study of families accessing care for cleft lip and palate deformities was performed in Vietnam. A survey instrument containing validated demographic, healthcare service access

Midterm outcomes of sleeve gastrectomy in the elderly.

BACKGROUND: The increase in life expectancy presents health systems with a growing challenge in the form of elderly obesity. Bariatric surgery has been shown to be a safe and effective treatment for obesity with reduction of excess weight and improvement in obesity-related co-morbidities. However, only recently have surgeons begun performing these operations on elderly patients on a larger scale, making data regarding mid- and long-term outcomes scarce. The objective of this study was to evaluate the safety and midterm efficacy of laparoscopic sleeve gastrectomy (LSG) in patients aged ≥60 year

Brain cytokines and disease.

Cytokines (e.g. various interleukins and subfamily members, tumor necrosis factors, interferons, chemokines and growth factors) act in the brain as immunoregulators and neuromodulators. Over a decade ago, the integrative article 'Immunoregulators in the Nervous System' (Neurosci Biobehav Rev 1991; 15: 185-215) provided a comprehensive framework of pivotal issues on cytokines and the nervous system that recently have been extensively studied. Cytokine profiles in the brain, including cytokine generation and action, have been studied in multiple models associated with neuropathophysiological con

Accumulation Of Chronic Conditions At The Time Of Death Increased In Ontario From 1994 To 2013.

With falling mortality rates for several diseases, patients are living longer with complex multimorbidities. We explored the burden of multimorbidity at the time of death, how it varies by socioeconomic status, and trends over time in Ontario, Canada. We calculated the proportions of decedents with varying degrees of multimorbidity and types of conditions at death, and we analyzed the trend from 1994 to 2013 in the number of conditions at the time of death. The prevalence of multimorbidity at death increased from 79.6 percent in 1994 to 95.3 percent in 2013. An upward trend in the number of co

Pirfenidone use in fibrotic diseases: What do we know so far?

BACKGROUND: Pirfenidone has demonstrated significant anti-inflammatory and antifibrotic effects in both animal models and some clinical trials. Its potential for antifibrotic activity positions it as a promising candidate for the treatment of various fibrotic diseases. Pirfenidone exerts several pleiotropic and anti-inflammatory effects through different molecular pathways, attenuating multiple inflammatory processes, including the secretion of pro-inflammatory cytokines, apoptosis, and fibroblast activation. OBJECTIVE: To present the current evidence of pirfenidone's effects on several fibro

Time's up. descriptive epidemiology of multi-morbidity and time spent on health related activity by older Australians: a time use survey.

Most Western health systems remain single illness orientated despite the growing prevalence of multi-morbidity. Identifying how much time people with multiple chronic conditions spend managing their health will help policy makers and health service providers make decisions about areas of patient need for support. This article presents findings from an Australian study concerning the time spent on health related activity by older adults (aged 50 years and over), most of whom had multiple chronic conditions. A recall questionnaire was developed, piloted, and adjusted. Sampling was undertaken thr

Distribution of chronic primary and secondary pain in a patient sample with psychiatric disorders.

OBJECTIVES: Chronic pain often coexists with psychiatric disorders, yet less is known about the specific types of chronic pain conditions that may co-occur. This study aimed to classify chronic pain according to ICD-11 categories and provide an overview of the types and frequency of these conditions in a patient sample with psychiatric disorders and coexisting chronic pain. METHODS: This descriptive cross-sectional study consecutively recruited 151 patients (78.8 % females, mean age 36 years) from psychiatric outpatient units. Common psychiatric diagnoses included anxiety disorders (27.8 %),

Symptoms-Based Phenotypes Among Women With Dysmenorrhea: A Latent Class Analysis.

Dysmenorrhea is highly prevalent and may increase women's risk for developing other chronic pain conditions. Although it is highly variable, symptom-based dysmenorrhea phenotypes have not been identified. The aims of the study were to identify symptom-based dysmenorrhea phenotypes and examine their relationships with demographic and clinical characteristics. In a cross-sectional study, 762 women with dysmenorrhea rated severity of 14 dysmenorrhea-related symptoms. Using latent class analysis, we identified three distinctive phenotypes. Women in the "mild localized pain" phenotype ( n = 202, 26

Chronic Non-Healing Ulcers Associated with Atopic Inflammation: A Case Report.

Chronic non-healing ulcers are the undesirable outcome of delayed wound healing influenced by many factors. It can be seen in patients with diabetes, autoimmune conditions and multiple primary skin conditions. But chronic non-healing ulcers secondary to atopic inflammation are rarely reported in the literature. In this study, we reported a case with wounds caused by the wrong tattoo and surgery, activation of atopic inflammation caused delayed wound healing and the formation of chronic non-healing ulcers. The patient's atopic inflammation was relieved and stabilized with oral cyclosporine and

System-based pain groups are uniquely associated with sociodemographic and psychiatric correlates among those with posttraumatic stress disorder (PTSD).

OBJECTIVE: Chronic pain is a prevalent comorbidity among those with posttraumatic stress disorder (PTSD). Chronic pain can be divided into specific pain (i.e., involving a single body system) and complex pain (i.e., involving multiple body systems). We explored sociodemographic and psychiatric differences between pain groups among those with PTSD. METHOD: Data came from the National Epidemiologic Survey on Alcohol and Related Conditions (NESARC-III, N = 36,309). Pain groups were determined through health professional confirmed self-report of chronic pain conditions within three body systems (

Quality of life in multiple endocrine neoplasia type 2A compared with normative and disease populations.

BACKGROUND: Patient-reported outcomes are measured in chronic disease states to inform intervention and management decisions while minimizing negative outcomes. We hypothesized that health-related quality of life in patients with multiple endocrine neoplasia type 2A would be worse than the general US population but similar to other chronic diseases. METHODS: Adults ≥18 years with multiple endocrine neoplasia type 2A were recruited to complete the Patient-Reported Outcomes Measurement Information System 29-item questionnaire (n = 45). Scores based on the Patient-Reported Outcomes Measurement I

Youth labour force absence and chronic health conditions in Australia.

BACKGROUND: Among older workers, chronic disease is known to be a key reason for early retirement. AIMS: To determine whether chronic health conditions act as a barrier to young Australians (aged 15-29) participating in the labour force. METHODS: Multiple logistic regression analysis to assess the adjusted odds ratio of people with different chronic health conditions being out of the labour force compared to those with no chronic conditions. Negative binomial regression models to predict the number of years people with different chronic health conditions would remain out of the labour force

Identifying multimorbidity clusters in an unselected population of hospitalised patients.

Multimorbidity (multiple coexisting chronic health conditions) is common and increasing worldwide, and makes care challenging for both patients and healthcare systems. To ensure care is patient-centred rather than specialty-centred, it is important to know which conditions commonly occur together and identify the corresponding patient profile. To date, no studies have described multimorbidity clusters within an unselected hospital population. Our aim was to identify and characterise multimorbidity clusters, in a large, unselected hospitalised patient population. Linked inpatient hospital episo

Postinfectious Syndromes and Long-Term Sequelae after Giardia Infections.

Giardiasis, caused by the parasite Giardia duodenalis, is a common infection throughout the world. Acute infections can be asymptomatic, cause mild gastrointestinal symptoms, or be associated with severe, prolonged diarrhea. Most Giardia infections are self-limiting; however, a subset of symptomatic and asymptomatic persons experience infection-associated chronic conditions that can affect multiple body systems. Those conditions include stunting and impaired cognitive function in children, irritable bowel syndrome, chronic fatigue, arthritis, and fibromyalgia, all of which can persist for mont
