Chronic Nodular Pulmonary Aspergillosis in a Patient with Rheumatoid Arthritis.

Chronic pulmonary aspergillosis, which features nodular lesions known as Aspergillus nodules, is a relatively uncommon disorder. We herein report a case of slowly progressing chronic multiple nodular pulmonary aspergillosis in a 59-year-old man with rheumatoid arthritis, dyspnea, and fatigue. One nodule was surgically resected. The surgical specimen featured central necrosis and was located adjacent to a respiratory bronchiole and pulmonary artery, without parenchymal invasion. Branching septate hyphae, compatible with Aspergillus, were seen inside this necrotic nodule. Chronic pulmonary asper

Pulmonary and cardiac cysticercosis: helical CT findings.

The authors aim to report the chest CT findings of a patient with disseminated cysticercosis, emphasising the pulmonary and cardiac features. The main finding consisted of multiple pulmonary, cardiac and chest wall nodules. The present case demonstrates that cysticercosis should be considered in the differential diagnosis of multiple pulmonary nodules, mainly in those patients with similar lesions in the cardiac muscle and/or in the chest wall.

Computed tomography halo sign in pulmonary nodules: frequency and diagnostic value.

On computed tomography (CT) scanning, a ground-glass opacity zone surrounding a pulmonary nodule has been named the computed tomography (CT) halo sign. To investigate the frequency and diagnostic value of the CT halo sign, the authors reviewed the CT examinations of 305 patients with proven diseases producing solitary or multiple nodules. The CT halo sign was seen in 22 patients (7%). Eleven patients had a solitary nodule; five patients had multiple nodules; and six patients had nodules associated with areas of pulmonary consolidation, or ground-glass opacity, or both. Solitary nodules were th

Combining Commercial Cancer Protein Biomarkers and Benign Fungal Antibodies Improves Diagnostic Accuracy in Pulmonary Nodules.

OBJECTIVE: To evaluate whether adding cancer and histoplasma biomarkers to existing clinical prediction models improves diagnostic accuracy for malignancy in indeterminate pulmonary nodules (IPNs). SUMMARY BACKGROUND DATA: IPNs remain a diagnostic challenge, particularly in regions where granulomatous disease exists. Clinical prediction models such as the Mayo model perform moderately well but only use clinical and radiographic information. Blood-based biomarkers for cancer proteins and fungal exposure may enhance diagnostic discrimination. METHODS: Two geographically distinct cohorts with 6

Intelligent immune clonal optimization algorithm for pulmonary nodule classification.

Computer-aided diagnosis (CAD) of pulmonary nodules is an effective approach for early detection of lung cancers, and pulmonary nodule classification is one of the key issues in the CAD system. However, CAD has the problems of low accuracy and high false-positive rate (FPR) on pulmonary nodule classification. To solve these problems, a novel method using intelligent immune clonal selection and classification algorithm is proposed and developed in this work. First, according to the mechanism and characteristics of chaotic motion with a logistic mapping, the proposed method utilizes the characte

Two Cases of Pulmonary Hyalinizing Granuloma Diagnosed via Thoracoscopic Lung Biopsy.

Pulmonary hyalinizing granuloma is a rare, benign tumor that arises from the pulmonary parenchyma and is difficult to diagnose preoperatively and intraoperatively. These typically present on chest computed tomography images as gradually enlarging, bilateral multiple pulmonary nodules. Herein, we report two cases wherein intraoperative frozen section analysis suggested fibrosis, but histopathological examination of the surgical specimens confirmed the diagnosis of pulmonary hyalinizing granuloma. Both patients were females in their 60s with incidental chest computed tomography findings of bilat

Thoracoscopic biopsy should be considered for pulmonary nodules with the halo sign during immunotherapy for hepatocellular carcinoma.

Multiple pulmonary nodules developing during immune checkpoint inhibitor (ICI) therapy pose a broad differential diagnosis that includes infection, vasculitis, malignancy, and immune-related adverse events (irAEs). A 77-year-old man receiving atezolizumab–bevacizumab therapy for recurrent hepatocellular carcinoma developed multiple bilateral pulmonary nodules with surrounding ground-glass opacities (halo sign). Although bronchoalveolar lavage cultured Aspergillus versicolor, fungal markers were negative, and the nodules progressed despite antifungal therapy. Tumor markers for hepatocellular ca

Single-step localization and excision of small pulmonary nodules using a mobile 3D C-arm.

OBJECTIVES: The use of a hybrid operating room equipped with robotic C-arm cone-beam computed tomography for single-step localization and excision of small pulmonary nodules finds high cost barriers. The new generation of 3D C-arm system not only depicts soft tissues with high contrast but also offers a more affordable and sustainable solution. This approach has been chiefly applied in the field of orthopedic surgery. In this case series, we describe the use of a mobile 3D C-arm system for localizing and removing small pulmonary nodules. METHODS: Between July and September 2020, we identified
