Skeletal muscle-heart crosstalk in chronic kidney disease: hierarchical signaling networks underlying myocardial metabolic reprogramming and fibrotic remodeling.

Cardiovascular disease is a major complication of chronic kidney disease (CKD) and often develops alongside skeletal muscle wasting and sarcopenia. These abnormalities are usually studied as separate consequences of CKD, but they may also be linked through shared systemic stressors and inter-organ communication. CKD exposes both skeletal muscle and the myocardium to a persistent uremic milieu characterized by toxin retention, chronic inflammation, oxidative stress, hypoxia, and metabolic disturbance. In this setting, skeletal muscle-heart crosstalk may shift from an adaptive homeostatic progra

Non-Work-Matched HIIT and MIIT Partially Restore Exerkine-Related and Mitochondrial Gene Expression in Diabetic Rat Skeletal Muscle.

Skeletal muscle mitochondrial dysfunction and altered myokine signaling contribute to insulin resistance in type 2 diabetes. This study compared the effects of non-work-matched high-intensity interval training (HIIT) and moderate-intensity interval training (MIIT) on skeletal muscle exerkine/myokine- and mitochondrial biogenesis-related gene expression and systemic metabolic indices in streptozotocin-nicotinamide-induced diabetic rats. Twenty-four male Wistar rats were initially allocated to healthy control, diabetic control, MIIT, or HIIT groups; after predefined treadmill-familiarization exc

Exercise-Induced Myonectin Protects Against Cerebral Ischemia-Reperfusion Injury.

BACKGROUND: There exists muscle-brain signaling via myokines. However, not all members of myokines have been thoroughly studied. Here, we identify myonectin, an exercise-induced myokine, as a new regulator of cerebral ischemia-reperfusion (I/R) injury. METHODS: Cerebral ischemia was induced by middle cerebral artery occlusion (MCAO/R) in wild-type mice and skeletal muscle-specific myonectin-knockout mice. Laser speckle contrast imaging (LSCI) and TTC staining were used to assess cerebral injury. HT22 and BV2 cells were utilized to mimic I/R-associated cellular damage using oxygen-glucose depr

Exercise Suppresses Head and Neck Squamous Cell Carcinoma Growth via Oncostatin M.

Major advances have been made in cancer treatment, but the prognosis for elderly cancer patients with sarcopenia and frailty remains poor. Myokines, which are thought to exert preventive effects against sarcopenia, have been reported to be associated with the prognosis of various cancers, but their effect on head and neck squamous cell carcinoma (HNSCC) is unknown. The aim of this study was to clarify the influence of exercise on the control of HNSCC and to examine the underlying mechanism involved. Mice were injected with HSC-3-M3 cells, a human cell line of highly metastatic and poorly diffe

Role of Obesity, Physical Exercise, Adipose Tissue-Skeletal Muscle Crosstalk and Molecular Advances in Barrett's Esophagus and Esophageal Adenocarcinoma.

Both obesity and esophageal adenocarcinoma (EAC) rates have increased sharply in the United States and Western Europe in recent years. EAC is a classic example of obesity-related cancer where the risk of EAC increases with increasing body mass index. Pathologically altered visceral fat in obesity appears to play a key role in this process. Visceral obesity may promote EAC by directly affecting gastroesophageal reflux disease and Barrett's esophagus (BE), as well as a less reflux-dependent effect, including the release of pro-inflammatory adipokines and insulin resistance. Deregulation of adipo

Underlying Mechanisms behind the Brain-Gut-Liver Axis and Metabolic-Associated Fatty Liver Disease (MAFLD): An Update.

Metabolic-associated fatty liver disease (MAFLD) includes several metabolic dysfunctions caused by dysregulation in the brain-gut-liver axis and, consequently, increases cardiovascular risks and fatty liver dysfunction. In MAFLD, type 2 diabetes mellitus, obesity, and metabolic syndrome are frequently present; these conditions are related to liver lipogenesis and systemic inflammation. This study aimed to review the connection between the brain-gut-liver axis and MAFLD. The inflammatory process, cellular alterations in hepatocytes and stellate cells, hypercaloric diet, and sedentarism aggravat

[Physical Activity and Obesity - Underlying Mechanisms, Practical Actions].

Individuals with obesity who undergo surgical or pharmacological therapies achieve good results in terms of weight and cardiometabolic risk reduction. It is not uncommon for those affected to equate the extent of weight loss achieved, with long-term treatment success. What is overlooked is that, in addition to obesity, significant weight loss also carries a risk of sarcopenia. Sarcopenic obesity and sarcopenia, in turn, increase the risk of cardiometabolic diseases. Physical activity has the potential to counteract cardiometabolic disease risk caused by obesity and sarcopenia. The underlying m

Fibroblast growth factor 21 is expressed and secreted from skeletal muscle following electrical stimulation via extracellular ATP activation of the PI3K/Akt/mTOR signaling pathway.

Fibroblast growth factor 21 (FGF21) is a hormone involved in the regulation of lipid, glucose, and energy metabolism. Although it is released mainly from the liver, in recent years it has been shown that it is a "myokine", synthesized in skeletal muscles after exercise and stress conditions through an Akt-dependent pathway and secreted for mediating autocrine and endocrine roles. To date, the molecular mechanism for the pathophysiological regulation of FGF21 production in skeletal muscle is not totally understood. We have previously demonstrated that muscle membrane depolarization controls gen

[The effect of physiotherapy treatments on the immune system].

The authors provide an overview of the effects of physiotherapy on the immune response. Immunomodulation plays a decisive role in the analgesic and anti-inflammatory mechanisms underlying physiotherapy. Exercise is the most prominent physiotherapy treatment, for which the most evidence exists. The skeletal muscle is a secretory organ that releases myokines in response to movement. One of the best-known of these is irisin, a movement-induced myokine. Irisin plays an important role in inhibiting oxidative stress, reducing systemic inflammatory responses, and providing neuroprotection. It also ha

Exercise-induced muscle-derived cytokines inhibit mammary cancer cell growth.

Regular physical activity protects against the development of breast and colon cancer, since it reduces the risk of developing these by 25-30%. During exercise, humoral factors are released from the working muscles for endocrinal signaling to other organs. We hypothesized that these myokines mediate some of the inhibitory effects of exercise on mammary cancer cell proliferation. Serum and muscles were collected from mice after an exercise bout. Incubation with exercise-conditioned serum inhibited MCF-7 cell proliferation by 52% and increased caspase activity by 54%. A similar increase in caspa

Role of physical exercise, vagal nerve stimulation, and vagotomy in inflammatory bowel disease.

Inflammatory bowel disease (IBD), comprising Crohn's disease and ulcerative colitis, represents the two predominant clinical entities within this spectrum of gastrointestinal disorders. Current evidence indicates that the etiology of IBD is multifactorial, involving a complex interplay between host genetic susceptibility and environmental determinants. In recent years, non-pharmacological strategies such as physical exercise and vagus nerve stimulation have gained increasing attention as adjunctive therapeutic approaches. Vagus nerve stimulation has emerged as a promising therapeutic modality,

Cognitive outcomes in patients treated with neuromuscular electrical stimulation after coronary artery bypass grafting.

OBJECTIVE: Mechanisms of neurocognitive injury as post-operative sequelae of coronary artery bypass grafting (CABG) are not understood. The systemic inflammatory response to surgical stress causes skeletal muscle impairment, and this is also worsened by immobility. Since evidence supports a link between muscle vitality and neuroprotection, there is a need to understand the mechanisms by which promotion of muscle activity counteracts the deleterious effects of surgery on long-term cognition. METHODS: We performed a clinical trial to test the hypothesis that adding neuromuscular electrical stim

Influence of Nutritional Status and Physical Exercise on Immune Response in Metabolic Syndrome.

Metabolic Syndrome (MetS) is a cluster of metabolic alterations mostly related to visceral adiposity, which in turn promotes glucose intolerance and a chronic systemic inflammatory state, characterized by immune cell infiltration. Such immune system activation increases the risk of severe disease subsequent to viral infections. Strong correlations between elevated body mass index (BMI), type-2-diabetes and increased risk of hospitalization after pandemic influenza H1N1 infection have been described. Similarly, a correlation between elevated blood glucose level and SARS-CoV-2 infection severity

Activation of the muscle-to-brain axis ameliorates neurocognitive deficits in an Alzheimer's disease mouse model via enhancing neurotrophic and synaptic signaling.

Skeletal muscle regulates central nervous system (CNS) function and health, activating the muscle-to-brain axis through the secretion of skeletal muscle-originating factors ("myokines") with neuroprotective properties. However, the precise mechanisms underlying these benefits in the context of Alzheimer's disease (AD) remain poorly understood. To investigate muscle-to-brain axis signaling in response to amyloid β (Aβ)-induced toxicity, we generated 5xFAD transgenic female mice with enhanced skeletal muscle function (5xFAD;cTFEB;HSACre) at prodromal (4-months old) and late (8-months old) sympto

Sustained elevation of MG53 in the bloodstream increases tissue regenerative capacity without compromising metabolic function.

MG53 is a muscle-specific TRIM-family protein that presides over the cell membrane repair response. Here, we show that MG53 present in blood circulation acts as a myokine to facilitate tissue injury-repair and regeneration. Transgenic mice with sustained elevation of MG53 in the bloodstream (tPA-MG53) have a healthier and longer life-span when compared with littermate wild type mice. The tPA-MG53 mice show normal glucose handling and insulin signaling in skeletal muscle, and sustained elevation of MG53 in the bloodstream does not have a deleterious impact on db/db mice. More importantly, the t

Irisin as a multi-target renoprotective mediator in chronic kidney disease: from molecular mechanisms to therapeutic perspectives.

BACKGROUND: Chronic kidney disease (CKD) is a progressive, multifactorial disorder affecting approximately 9-13% of the global population and is driven by interrelated mechanisms including inflammation, oxidative stress, endothelial dysfunction, and fibrosis. Despite advances in therapies such as SGLT2 inhibitors, significant residual risk persists, necessitating the identification of novel multi-target therapeutic strategies. OBJECTIVE: This review aims to provide an integrated view of the molecular pathways involved in CKD progression and to critically study the emerging evidence for irisin

Vascular and musculoskeletal interactions: structure, function, and therapeutic strategies for sarcopenia and osteoporosis.

Muscle and bone, key tissues that maintain the body's structure and function, are closely connected via mechanical and biochemical interactions. The vascular system plays a crucial role in musculoskeletal health, not only by providing nutrients and oxygen but also by regulating growth, regeneration, and metabolism. This review systematically examines the vascular structure and function of the musculoskeletal system, and explores the complex biochemical signaling networks involving myokines, osteokines, and vascular-derived factors. Vascular dysfunction is a key contributor to the pathogenesis

FNDC4 and FNDC5 Attenuate SARS-CoV-2 S1-Induced Inflammatory Responses in Human Adipose Tissue.

BACKGROUND: Adipose tissue is recognised as a SARS-CoV-2 reservoir and potential infection site. We herein characterised SARS-CoV-2 entry points in visceral (VAT) and subcutaneous (SAT) adipose tissue from people with obesity and determined whether the adipo-myokines FNDC4 and FNDC5 modulate SARS-CoV-2 spike glycoprotein subunit 1 (S1)-induced inflammation in adipocytes and macrophages. METHODS: Plasma concentrations of FNDC4, FNDC5 and angiotensin-converting enzyme 2 (ACE2) were measured in 183 participants with obesity and normal weight. Expression of SARS-CoV-2 host cell entry receptors wa

Pre-Dialysis Serum Myostatin Concentration Is Independently Associated with Intradialytic Muscle Cramps in Hemodialysis Patients.

Muscle cramps are among the most common acute complications of hemodialysis (HD), often occurring even in the absence of electrolyte disturbances. Most cramps are painful and may cause patients to fear future dialysis sessions. We hypothesized that susceptibility to muscle cramps may be related to impaired skeletal muscle myokine secretion. No significant differences in pre-dialysis myokine levels were observed between patients with and without muscle cramps (myostatin 36.2 vs. 86.9 (p = 0.09), fibronectin type III domain-containing protein 5 (FNDC5) 0.6 vs. 0.6 (p = 0.61), interleukin-6 (IL-6
