MRI-based deep learning model predicts recurrent nasopharyngeal carcinoma in post-radiation nasopharyngeal necrosis.

BACKGROUND: The pretreatment identification of post-radiation nasopharyngeal necrosis (PRNN) combined with recurrent nasopharyngeal carcinoma (referred to as cancer-infiltrative PRNN) is crucial for the diagnosis and treatment of PRNN. As the first study to identify recurrent nasopharyngeal carcinoma in patients with PRNN, we aimed to develop a deep learning (DL)-based predictive model using routine MRI to distinguish cancer-infiltrative PRNN from cancer-free PRNN. METHODS: MRIs of 437 patients with PRNN were manually labeled and randomly divided into training and validation cohorts. Video Sw

The value of nasopharyngeal brush versus plasma EBV DNA detection in diagnosing local recurrence of nasopharyngeal carcinoma.

Nasopharyngeal carcinoma (NPC) remains a major challenge due to the high incidence of recurrence, often leading to therapeutic failure. Early detection of recurrence is critical, yet effective surveillance methods are limited. This study aims to evaluate the utility of Epstein-Barr virus (EBV) DNA detection in nasopharyngeal brush samples for identifying recurrent NPC. A total of 187 NPC patients who had undergone radical radiotherapy with or without chemotherapy were included. EBV DNA levels in both nasopharyngeal brush and plasma samples were analyzed using real-time quantitative PCR. Head a

Septin 9 Methylation in Nasopharyngeal Swabs: A Potential Minimally Invasive Biomarker for the Early Detection of Nasopharyngeal Carcinoma.

Nasopharyngeal carcinoma (NPC) is highly prevalent in Southeast Asia, and an unfavorable outcome is usually attributed to advanced stage NPC. Current methods for the early diagnosis of NPC have limitations in clinical practice. The aim of this study was to investigate the diagnostic ability of Septin 9 methylation for NPC. A quantitative methylation-sensitive PCR (qMS-PCR) assay was developed to measure the methylation status and levels of Septin 9 in nasopharyngeal tissues and paired swabs from patients with NPC, chronic nasopharyngitis, and healthy donors. Methylated Septin 9 was detected in

Nasopharyngeal Epstein-Barr Virus Load: An Efficient Supplementary Method for Population-Based Nasopharyngeal Carcinoma Screening.

Serological detection of Epstein-Barr virus (EBV) antibodies is frequently used in nasopharyngeal carcinoma (NPC) mass screening. However, the large number of seropositive subjects who require close follow-up is still a big burden. The present study aimed to detect the nasopharyngeal EBV load in a high-risk population seropositive for antibodies against EBV, as well as to examine whether assay for nasopharyngeal EBV DNA load might reduce the number of high-risk subjects for follow-up and improve early detection of NPC. A prospective and population-based cohort study was conducted in southern C

Resolution of severe sleep-disordered breathing with a nasopharyngeal obturator in 2 cases of nasopharyngeal stenosis complicating uvulopalatopharyngoplasty.

Obstructive sleep apnea-hypopnea syndrome (OSAHS) is characterized by episodic decrements in airflow due to upper airway obstruction. Uvulopalatopharyngoplasty (UPPP) is a potential therapy for OSAHS. Nasopharyngeal stenosis is a rare complication of UPPP that worsens OSAHS. We report two patients referred for OSAHS worsened by nasopharyngeal stenosis complicating UPPP. Both patients were treated with carbon dioxide laser release of adhesions and placement of a nasopharyngeal obturator. Follow-up polysomnograms demonstrated resolution of OSAHS correlating with subjective resolution of symptoms

Screening nasopharyngeal carcinoma by detection of the latent membrane protein 1 (LMP-1) gene with nasopharyngeal swabs.

BACKGROUND: Nasopharyngeal carcinoma (NPC) is a common head and neck cancer in Taiwan. The goals of the current study were to investigate whether a nasopharyngeal swab technique could provide enough DNA for polymerase chain reaction (PCR) analysis of the Epstein-Barr virus (EBV)-derived latent membrane protein 1 (LMP-1) gene and to determine the feasibility and reliability of diagnosing NPC by detection of LMP-1 in the nasopharynx. METHODS: 320 adults underwent nasopharyngoscopy and nasopharyngeal swab to obtain cells for the LMP-1 PCR assay; some patients also underwent nasopharyngeal biopsy

[No point mutation of the 2.8 kb EcORI fragment of the nasopharyngeal carcinoma transforming gene TX in nasopharyngeal carcinoma].

We have previously cloned a nasopharyngeal carcinoma transforming gene from human nasopharyngeal carcinoma cell line CNE-2 using mouse JB6 cell line as the recipient cells. This transforming gene, designated Tx, exhibits moderate transforming activity upon transfection into JB6 cells. Several lines of evidence indicate that the 2.8 kb EcoRI fragment which lies in the middle of the Tx gene is responsible for the transforming activity. The sequence of the 2.8 kb EcoRI fragment was determined, which showed striking homology with the human immunoglobulin light chain C region gene. Using the modifi

De-Escalation Strategies in Nasopharyngeal Carcinoma.

PURPOSE OF REVIEW: The severe long-term toxicities of standard therapy for nasopharyngeal carcinoma underscore the need for treatment de-escalation. This review summarizes the rationale and evolving approaches to reduce treatment intensity while maintaining efficacy, with the goal of balancing survival outcomes and quality of life. RECENT FINDINGS: De-escalation strategies primarily focus on two domains: (1) radiotherapy optimization through target volume reduction, response-adapted dose de-escalation, and advanced techniques for organ preservation; (2) systemic therapy adjustments, including

Novel Therapeutic Development for Nasopharyngeal Carcinoma.

Nasopharyngeal carcinoma (NPC) is a rare malignancy with a distinct epidemiological pattern and is most often associated with Epstein-Barr virus (EBV). EBV plays a critical role in NPC pathogenesis, with viral proteins driving oncogenesis by altering immune regulation, apoptosis, and tumor progression. The unique molecular landscape of NPC presents both challenges and opportunities for therapeutic development, particularly in the recurrent and metastatic (R/M) setting, where treatment resistance remains a major hurdle. While platinum-based chemotherapy has traditionally been the standard of ca

The Role of Ubiquitination Regulation in Nasopharyngeal Carcinoma.

Nasopharyngeal carcinoma (NPC) is one of the most common malignant head and neck tumors. Despite being sensitive to radiation, the prognosis of some NPC patients remains unoptimistic due to the resistance to therapeutic approaches. Ubiquitination is a crucial post-translational modification of proteins, and is key to maintaining protein homeostasis in vivo. Studies have shown that ubiquitination modification can regulate cell proliferation, apoptosis, and other processes, thereby affecting the development of NPC. In this review, we summarize the research on ubiquitination regulation in NPC, ai

Nasopharyngeal Foreign Body with Unusual Presentation.

INTRODUCTION: An unusual nasopharyngeal foreign body in a very young child with no clinical symptoms is a rare case presentation. CASE REPORT: A nine-month-old child presented with a suspected history of foreign body ingestion without any clue to the parents about the nature of the foreign body. X-ray of the nasopharynx revealed a sharp unusual metallic "Louis Vuitton" shoe logo that the child had accidentally inserted into the nasopharynx via the oral cavity while playing. Foreign body was removed under general anesthesia without complications. CONCLUSION: X-ray nasopharynx should be includ

Radiotherapy for recurrent juvenile nasopharyngeal angiofibroma.

Juvenile nasopharyngeal angiofibroma (JNA) is a rare paediatric tumour known for its local destructiveness and high recurrence rate. Surgery is the primary treatment modality for JNA, though other options, such as hormonal therapy, embolisation and radiotherapy, exist for inoperable cases. The location of the tumour makes surgical intervention challenging. A 14-year-old male presented with epistaxis and headaches as the chief complaints and was diagnosed with nasopharynx angiofibroma by computed tomography (CT) scan in 2018. Pre-operative embolisation was performed and followed by surgical rem

Recurrent Nasopharyngeal Carcinoma and the Role of Surgery.

Recurrent nasopharyngeal carcinoma (rNPC) presents unique challenges as reirradiation comes with significant treatment-related morbidity in swallowing, middle ear function, and large-vessel integrity. Advances in endoscopic technology have made surgery for rNPC an increasingly viable option for select patients and may play a role in providing a better quality of life to patients with this challenging disease. In carefully selected patients, endoscopic and open surgical approaches may provide comparable disease control while mitigating long-term treatment-related morbidity.

Effect of radiotherapy interruption on nasopharyngeal cancer.

Nasopharyngeal carcinoma (NPC) is a malignant tumor originating from the epithelial cells of the nasopharynx with a unique geographic distribution, and is particularly prevalent in East and Southeast Asia. Due to its anatomical location, the surgery is difficult to access and the high sensitivity of nasopharyngeal cancer to radiotherapy (RT) makes it the main treatment modality. Radical radiotherapy is the first-line treatment for early-stage nasopharyngeal carcinoma and the cornerstone of multidisciplinary treatment for patients with locally advanced nasopharyngeal carcinoma. Nevertheless, ra

Bulky Recurrent Juvenile Nasopharyngeal Angiofibroma.

Juvenile nasopharyngeal angiofibroma is a rare and highly vascularized tumor that accounts for 0.05 to 0.5% of all head and neck neoplasms. The aim of this work was to present a case of a large recurrent juvenile nasopharyngeal angiofibroma coexisting with a facial lipoma in a 16-year-old boy. The patient was referred to our institution because of frequent unilateral epistaxis. Computed tomography revealed a hypervascular tumor with ethmoidal cell destruction and spread to the nasopharynx. Operative treatment of nasal cavity tumors was carried out using a transpalatal approach. After 6 months,

Preoperative emobilisation of juvenile nasopharyngeal angiofibroma.

INTRODUCTION: Juvenile nasopharyngeal angiofibroma(JNA) are highly vascular benign tumours originating in the sphenopalatine fossa and may extend to the pterygopalatine fossa, paranasal sinuses, and nasal cavity. The management of JNA has evolved greatly with development of endoscopy. Treatment modality has changed from open approach to endoscopic approach due to various advantages offered by the endoscopic approach. Bleeding during the surgery can compromise the exposure and surgical excision of JNA endoscopically. There have been many techniques to decrease intraoperative bleeding including

Two Cases of Primary Nasopharyngeal Tuberculosis.

Nasopharyngeal tuberculosis can arise from both a primary infection and a secondary spread via the lymphatic or hematogenous system from a primary pulmonary lesion. Primary nasopharyngeal tuberculosis is rare and difficult to detect earlier because of the nonspecific presentations of the disease. As upper airway tuberculosis can be contagious, early initial diagnosis and suspicion of the physicians are needed in clinical practice. Recently, we successfully diagnosed and treated the disease by antitubercular medications of two cases of primary nasopharyngeal tuberculosis. Herein, we report our
