Renal function in PUSH-AHF: a summary of newly released data from the 2024 Annual Congress of the Heart Failure Association of the ESC.

The Pragmatic Urinary Sodium-based algoritHm in Acute Heart Failure (PUSH-AHF) study, published in August of 2023, was the first randomized clinical trial to compare natriuresis-guided decongestion (based on spot urinary sodium measurement) to standard of care in patients with acute heart failure with congestion receiving loop diuretic therapy. Based on results from their trial, the authors concluded that natriuresis-guided loop diuretic treatment was safe and improved natriuresis and diuresis without impacting long-term clinical outcomes. The original PUSH-AHF trial included limited informati

Kidney function and acid-base status as gatekeepers of natriuretic and diuretic responsiveness in chronic HF: insights from the DEA-HF clinical trial.

AIMS: In chronic heart failure (HF), decongestion is hindered by reduced renal function and diuretic resistance. This study compared the effect of eGFR and serum bicarbonate on three diuretic regimens in congestion-refractory HF patients. METHODS AND RESULTS: This is a prespecified post-hoc analysis of DEA-HF, a randomized, crossover trial (n = 42). Patients received, in random order, weekly treatments of IV furosemide 250 mg; oral metolazone 5mg + IV furosemide 250 mg; and IV acetazolamide 500mg + IV furosemide 250 mg. Primary endpoint: 6h-Natriuresis; secondary: 6h-diuresis and decongestion

Brain involvement in the regulation of renal sodium excretion.

The aim of the present work was to study some aspects of the brain involvement in renal sodium excretion from the point of view of renal regulation of the extracellular fluid volume and composition. It was found on the basis of experiments on conscious sheep that sodium concentration in the cerebrospinal fluid may be of importance for the mechanism of the extracellular fluid volume regulation. Higher concentrations potentiate a homeostatically effective natriuresis during extracellular fluid volume expansion and lower concentrations impair natriuresis. It was also found in sheep with lesions o

Observations on the natriuretic response to intravenous infusions of atrial natriuretic factor in water-loaded anaesthetized rats.

Natriuresis induced by infusions of atrial natriuretic factor (ANF) in water-loaded rats typically occurred in time sequences ('peak-and-shoulder' patterns) with an overlap of two different natriuretic responses. The first response was abrupt, short-lived and potentiated by angiotensin converting enzyme (ACE) inhibition. The second response was a gradual onset of natriuresis which lasted only as long as the ANF infusion and was directly correlated to the level of arterial blood pressure. This phenomenon explains the apparent loss of responsiveness to ANF bolus injections during a late phase. A

Renal excretory responses to single and repeated administration of diuretics in healthy subjects: clinical connotations.

Administration of an initial oral dose of hydrochlorothiazide 25 mg to healthy subjects is followed by increased 24-hour urinary outputs of sodium, chloride, and potassium. On the fourth day of once-daily dosing with hydrochlorothiazide 25 mg, 24-hour natriuresis and chloriuresis are no longer augmented, but the elevation in 24-hour kaliuresis that follows the first dose remains unchanged. Twenty-four-hour urinary calcium output is consistently reduced during repeated once-daily administration of hydrochlorothiazide 25 mg. The first oral dose of the loop diuretic torasemide augments the averag

Elevation of plasma atrial natriuretic peptide in a neurosurgical patient with the syndrome of inappropriate secretion of antidiuretic hormone--case report.

The authors describe a case of subarachnoid hemorrhage with hyponatremia accompanied by elevation of plasma atrial natriuretic peptide (ANP). The early phase of hyponatremia was classified as the syndrome of inappropriate secretion of antidiuretic hormone (ADH) due to subarachnoid hemorrhage. However, in the later phase, hyponatremia and natriuresis were accompanied by suppression of ADH while plasma ANP remained elevated. The patient was effectively treated with demeclocycline and hypertonic saline. The significance of ANP in the pathophysiology of increased natriuresis is discussed.

Mineralocorticoid deficiency in post-operative cerebral salt wasting.

Acute hyponatremia, following neurosurgery, results from inappropriate antidiuretic hormone secretion (SIADH) or cerebral salt wasting (CSW). CSW is due to abnormally high atrial or brain natriuretic peptides (ANP, BNP), which block all stimulators of zona glomerulosa steroidogenesis, resulting in mineralocorticoid deficiency. A 3 year-old girl presented CSW at day 4, after resection of craniopharyngioma and hypophysectomy. Hyponatremia, hyperkalemia and high natriuresis occurred on day 8, with low renin and aldosterone and elevated BNP 120.3 ng/ml (undetectable before surgery). Fludrocortison

[Diuretic-based therapy].

Diuretics are pharmacological agents that increase natriuresis through inhibition of tubular re-absorption of sodium. The mechanisms and site of this inhibition differ with each drug class, accounting for their additive effects on natriuresis increase and their hydroelectrolytic side effects. The response to a given diuretic dose depends on the diuretic concentration on the urine at its action site. This concentration may be decreased by pharmacokinetic factors such as encountered in renal insufficiency or in nephrotic syndrome. These resistance mechanisms of diuretics may be corrected by dose

8-Aminoinosine and 8-Aminohypoxanthine Inhibit Purine Nucleoside Phosphorylase and Exert Diuretic and Natriuretic Activity.

8-Aminoguanine and 8-aminoguanosine (via metabolism to 8-aminoguanine) are endogenous 8-aminopurines that induce diuresis, natriuresis, and glucosuria by inhibiting purine nucleoside phosphorylase (PNPase); moreover, both 8-aminopurines cause antikaliuresis by other mechanisms. Because 8-aminoinosine and 8-aminohypoxanthine are structurally similar to 8-aminoguanosine and 8-aminoguanine, respectively, we sought to define their renal excretory effects. First, we compared the ability of 8-aminoguanine, 8-aminohypoxanthine, and 8-aminoinosine to inhibit recombinant PNPase. These compounds inhibit

Alpha-2A Adrenoceptor Agonist Guanfacine Restores Diuretic Efficiency in Experimental Cirrhotic Ascites: Comparison with Clonidine.

BACKGROUND: In human cirrhosis, adrenergic hyperfunction causes proximal tubular fluid retention and contributes to diuretic-resistant ascites, and clonidine, a sympatholytic drug, improves natriuresis in difficult-to-treat ascites. AIM: To compare clonidine (aspecific α2-adrenoceptor agonist) to SSP-002021R (prodrug of guanfacine, specific α2A-receptor agonist), both associated with diuretics, in experimental cirrhotic ascites. METHODS AND RESULTS: Six groups of 12 rats were studied: controls (G1); controls receiving furosemide and potassium canrenoate (G2); rats with ascitic cirrhosis due

Natriuretic and neurohormonal responses to nesiritide, furosemide, and combined nesiritide and furosemide in patients with stable systolic dysfunction.

BACKGROUND: In patients with heart failure, few data describe the neurohormonal response to nesiritide and furosemide either alone or in combination. This study systematically compared the effects of nesiritide, furosemide, and their combination on natriuresis/diuresis and plasma aldosterone in patients with chronic stable heart failure who were relatively diuretic resistant. HYPOTHESIS: Natriuretic, diuretic, and neurohormonal responses to furosemide and nesiritide will differ when these agents are administered alone vs. in combination. Kidney-brain neural interaction in cardiorenal syndrome: insights from volume-overloaded spontaneously hypertensive rats with aorto-caval fistula.

OBJECTIVE: We hypothesized, first, that impairment of autoregulation of renal blood flow (RBF), glomerular filtration rate (GFR), and of the pressure-natriuresis in spontaneously hypertensive rats (SHR) is present in early phase of heart failure and precedes its decompensation. Second, that both the total renal denervation (T-RDN) and/or selective renal afferent denervation (A-RDN) would improve renal autoregulation and pressure-natriuresis in SHR with heart failure. METHODS: Heart failure was elicited by volume overload achieved using aorto-caval fistula (ACF). T-RDN and A-RDN were performed

Urinary sodium-guided diuretic therapy in acute decompensated heart failure: A grade assessed systematic review and meta-analysis with trial sequential analysis.

BACKGROUND: Diuretic resistance remains a major challenge in acute decompensated heart failure (ADHF). Urinary sodium (UNa) concentration provides an early marker of loop diuretic responsiveness, yet its clinical utility remains uncertain. METHODS: We conducted a systematic review and meta-analysis (PROSPERO ID: CRD420251175306). PubMed, Embase, Scopus, and CENTRAL were searched from database inception to October 1, 2025, for studies evaluating UNa-guided diuretic titration in hospitalized ADHF patients. Primary outcomes were 24- and 48 h diuresis and natriuresis; secondary outcomes included

Renal negative pressure treatment as a novel therapy for heart failure-induced renal dysfunction.

Congestion is the primary pathophysiological lesion in most heart failure (HF) hospitalizations. Renal congestion increases renal tubular pressure, reducing glomerular filtration rate (GFR) and diuresis. Because each nephron is a fluid-filled column, renal negative pressure therapy (rNPT) applied to the urinary collecting system should reduce tubular pressure, potentially improving kidney function. We evaluated the renal response to rNPT in congestive HF. Ten anesthetized ∼80-kg pigs underwent instrumentation with bilateral renal pelvic JuxtaFlow catheters. GFR was determined by iothalamate cl

Endothelin mediates sex-differences in acclimation to high salt diet in rats.

INTRODUCTION: Current understanding of sodium (Na+) handling is based on studies done primarily in males. Contrary to the gradual increase in high salt (HS) induced natriuresis over 3-5 days in males, female Sprague Dawley (SD) rats have a robust natriuresis after 1 day of HS. Renal endothelin-1 (ET-1) signaling, through ET receptor A and B, is an important natriuretic pathway and was implicated in our previous dietary salt acclimation studies, however, the contribution of ET receptors to sex-differences in acclimation to dietary Na+ challenges has yet to be clarified. We hypothesized that ET

Renal mechanisms of angiotensin II-induced hypertension.

The kidneys play a central role in the long-term control of arterial pressure by regulating sodium balance and extracellular fluid volume. The renin-angiotensin system is important in the regulation of the arterial pressure through its chronic effects on the pressure natriuresis relationship. Under physiologic conditions, angiotensin II (Ang II) is important in causing the long-term relationship between arterial pressure and sodium excretion to be very steep, so that minimal changes in arterial pressure are necessary to maintain sodium balance in response to variations in sodium intake. An ina

Diuretic effects of KW-3902 (8-(noradamantan-3-yl)-1,3-dipropylxanthine), a novel adenosine A1 receptor antagonist, in conscious dogs.

The diuretic effects of KW-3902 (8-(noradamantan-3-yl)-1,3-dipropylxanthine), a novel adenosine A1 receptor antagonist, were determined and compared with those of trichlormethiazide (TCM) and furosemide in saline-loaded conscious dogs. KW-3902, at doses higher than 0.1 mg/kg (p.o.), produced dose-dependent increases of urine volume and sodium excretion and these effects were statistically significant at doses of 1-100 mg/kg. The increase in potassium excretion was lower than that of sodium, and the ratio of sodium to potassium excretion (Na/K) tended to be elevated. TCM (0.3 mg/kg) and furosem

Is there a role for renin profiling in selecting chronic heart failure patients for ACE inhibitor treatment?

BACKGROUND: It remains uncertain whether angiotensin converting enzyme (ACE) inhibitors benefit all heart failure patients or just those with renin-angiotensin-aldosterone system (RAAS) activation. OBJECTIVE: To determine whether the response to an ACE inhibitor, assessed by urine sodium excretion, was different in patients with low renin versus those with high renin. DESIGN: Plasma renin activity (PRA) was measured in 38 patients with stable chronic heart failure (21 male, 17 female; mean (SD) age 71 (6) years, range 59-82 years) on chronic diuretic treatment alone. Unique Role of Proximal Tubule Dipeptidyl Peptidase 4 on Blood Pressure, Renal Sodium Handling, and Na+/H+ Exchanger Isoform 3 Phosphorylation.

BACKGROUND: Dipeptidyl peptidase 4 (DPP4) is a transmembrane serine exopeptidase highly expressed in the proximal tubule (PT). While its enzymatic role is well characterized, its non-enzymatic functions remain unclear. DPP4 physically associates with the Na+/H+ exchanger isoform 3 (NHE3), and DPP4 inhibitors promote natriuresis; however, the mechanisms by which DPP4 regulates NHE3 and its role in blood pressure (BP) regulation remain controversial. We hypothesized that PT DPP4 promotes sodium reabsorption and attenuates pressure-natriuresis by preventing NHE3 phosphorylation at serine 552 (pS5

[Effect of diuretics on the humoral system of kidney prostaglandins in hypertension patients].

Sixty-four patients with stage IIB essential hypertension were examined for the urinary excretion of prostaglandins E2 and F2 alpha, natriuresis and diuresis in the control period and then on the 3rd, and the 12th-14th day of the treatment with diuretics (furosemid, hypotiazid, oxodolin, brinaldix). All diuretics were found to alter the urinary PG excretion, with the degree of their effect depending on the type of the drug, the route and duration of its administration. Furosemid, to a greater degree than other diuretics, increased the urinary excretion of PGE2 thus bringing down the PGF2 alpha
