Perioperative Complications in Percutaneous Nephrolithotomy: Predictive Risk Factors and Hemodynamic Alterations.

Background Percutaneous nephrolithotomy (PCNL) is one of the most frequently used methods of treating large and/or complex kidney stones. Despite the growing interest in this area, there are relatively few papers that report studies of the changes in hemodynamics in the perioperative period and the potential factors that might influence the patient's condition. This aspect, however, has not been well described in the current literature, although fluctuations in blood pressure, temperature, and electrolyte balance during surgery may also lead to development of complications. Objectives In this

Totally tubeless, tubeless, and tubed percutaneous nephrolithotomy for treating kidney stones.

BACKGROUND: Percutaneous nephrolithotomy (PNL) is the standard of care for removing large kidney stones (> 2 cm). Once the procedure is complete, different exiting strategies exist to manage the percutaneous tract opening, including placement of an external nephrostomy tube, placement of an internal ureteral stent, or no external or internal tube. The decision to place or not place a tube is handled differently among clinicians and may affect patient outcomes. OBJECTIVES: To assess the effects of tubeless PNL (with ureteral stenting), totally tubeless PNL (without ureteral stenting or nephros

Factors Predicting Outcomes of Supine Percutaneous Nephrolithotomy: Large Single-Centre Experience.

Objective: Percutaneous nephrolithotomy (PCNL) is the treatment of choice for large renal calculi. The prone position has been considered the preferred position to obtain renal access. However, the supine position has recently gained popularity, which confers several potential advantages. The current study analyses the prognostic factors for successful supine PCNL procedures in a larger tertiary centre. Subjects: Prospective data were collected from all patients undergoing PCNL in the Galdako modified Valdivia position at our institution between February-2007 and September-2020. Surgical outco

Intracardiac Migration of the Ureteral Double-J Stent during Percutaneous Nephrolithotomy.

The complications of percutaneous nephrolithotomy (PNL) include hemorrhage, damage to adjuvant organs, and other medical issues, although intracardiac migration of ureteral double-J stent has never been found during PNL and delaying the diagnosis might cause mortality. We report the case of a 60-year-old male who was admitted to receive one-stage PNL for right renal stones. During operation, an unexpected atrial fibrillation with a drop in blood pressure was suddenly encountered and the chest X-ray subsequently showed that the ureteral double-J had penetrated deep into the heart. Emergent endo

Management of Splenic Injury During Percutaneous Nephrolithotomy: Report of Two Cases.

Percutaneous nephrolithotomy (PCNL) has become the standard of care for the removal of kidney stones >2 cm. Major complications, although rare, are between 1% and 7%. Splenic injury during PCNL is rare and can often be managed conservatively, but has the potential to be devastating, necessitating the importance of early diagnosis. Our team describes two cases of splenic injury during PCNL with emphasis on diagnosis and management. Although both cases were managed conservatively through close monitoring and prolonged nephrostomy tube presence, one case had a concurrent pneumothorax. Both cases

Conservative Management of Liver Perforation During Percutaneous Nephrolithotomy: Case Couplet Presentation.

Percutaneous nephrolithotomy (PCNL) remains the recommended intervention for large kidney stones, major complications, although rare, are between 1% and 7%. Literature regarding liver injury during PCNL is sparse, and many incidences occur unnoticed. In general, most liver injuries can be treated conservatively when compared with other organ injury sustained during PCNL. Despite this, there is still significant potential for intraperitoneal bleeding as well as possible hemodynamic instability that may result secondary to the inadvertent access. Our team describes two cases of liver injury duri

Metabolic Syndrome Increases Risk of Postoperative Myocardial Infarction Following Percutaneous Nephrolithotomy.

INTRODUCTION: Percutaneous nephrolithotomy (PCNL) is the gold standard treatment for upper tract stone burdens greater than 2 cm. Metabolic syndrome (MetS) is a constellation of conditions (diabetes mellitus, hypertension, dyslipidemia, and obesity) and is a risk factor for nephrolithiasis. Our objective was to investigate adverse cardiovascular outcomes of PCNL in patients with comorbid MetS diagnoses. MATERIALS AND METHODS: Data from the Healthcare Cost and Utilization Project State Inpatient Database for Florida and California were used to identify PCNL patients (ICD9: 55.03, 55.04) betwee

Clinically insignificant residual fragments after percutaneous nephrolithotomy: medium-term follow-up.

BACKGROUND AND PURPOSE: Clinically insignificant residual fragments (CIRFs), defined as asymptomatic, noninfectious, ≤4  mm fragments, are sometimes observed after percutaneous nephrolithotomy (PCNL). Because the natural history of these fragments is unclear, we investigated the medium-term outcome of these fragments. PATIENTS AND METHODS: During a 3-year period, 430 patients underwent PCNL. Overall stone-free rate was 74.5%, and CIRFs were encountered in 22% of cases 3 months after surgery. A total of 38 patients who had CIRFs immediately after PCNL with at least 24 months of follow-up were

Effect of medical management on recurrent stone formation following percutaneous nephrolithotomy.

PURPOSE: Percutaneous nephrolithotomy is a commonly used procedure for treatment of large or complex renal calculi. In some instances postoperative residual stone fragments are an unavoidable result. Yet to our knowledge no study has examined the impact of medical management on stone formation in patients with or without residual fragments following percutaneous nephrolithotomy. Thus, we have conducted the first investigation of aggressive medical management following percutaneous nephrolithotomy and its impact on stone formation rates in patients with and without residual fragments. Laparoscopic assisted percutaneous nephrolithotomy (PCNL) in ectopic kidneys: two different techniques.

Two patients presented with calculi in ectopic kidneys. Complete clearance of calculi was achieved by laparoscopic assisted percutaneous nephrolithotomy (PCNL), by different but related techniques. In one patient with prior history of open pyelolithotomy, the anterior surface of the kidney was exposed by mobilizing the overlying sigmoid colon laparoscopically and the percutaneous tract was established into the desired calyx under combined laparoscopic and fluoroscopic control. In the second patient, the tract was established between the major mesenteric vessels without any mobilization of the

Air travel and thromboembolic complications after percutaneous nephrolithotomy for staghorn stone.

Thromboembolic complications after percutaneous surgery for staghorn stone are rare. We report a case of silent deep vein thrombosis (DVT) after long-distance air travel that was complicated by both recurrent pulmonary emboli and paradoxical arterial embolus despite full-dose systemic heparin anticoagulation. Management options for thromboembolic complications in the context of percutaneous renal surgery are discussed, and risk factors predisposing to silent deep vein thrombosis after long-distance air travel are outlined.

Comparative Analysis of Standard Percutaneous Nephrolithotomy (PCNL) and Mini-Percutaneous Nephrolithotomy (Mini-PCNL) for Renal Stones Larger Than 2 cm at a Rural Hospital: A Study Protocol.

Background Percutaneous nephrolithotomy (PCNL) is considered a standard treatment option for large-size renal stones but is associated with drawbacks such as bleeding and prolonged recovery. Mini-PCNL offers a less invasive alternative, but its efficacy compared to standard PCNL for renal stones larger than 2 cm remains under debate. This study aims to compare the efficacy and safety of standard PCNL versus mini-PCNL for renal stone sizes more than 2 cm. Methods This single-centre prospective interventional study will be conducted at Acharya Vinoba Bhave Rural Hospital (AVBRH). The study will

Air Pyelogram for Percutaneous Nephrolithotomy in Patients with Iodinated Contrast-induced Anaphylaxis.

Percutaneous nephrolithotomy (PCNL) is the preferred treatment for large renal calculi. Fluoroscopic access conventionally requires iodinated contrast, which may precipitate anaphylaxis in sensitized individuals. We report two cases of contrast-induced anaphylaxis, in which air pyelography was successfully used to delineate the pelvicalyceal system and obtain safe renal access during PCNL.

Pushing the Envelope: Ambulatory Percutaneous Nephrolithotomy for Patients with Complex Renal Calculi.

Introduction: Ambulatory percutaneous nephrolithotomy (PCNL) has grown in popularity for patients with smaller stone burdens and few comorbidities. Its feasibility in patients with complex renal calculi is less clear. In this study, we evaluate the safety of same-day discharge after PCNL for patients with complex renal stones when compared with those admitted after surgery. Methods: We retrospectively identified adult patients who underwent PCNL for complex stones from April 2019 to January 2021 at our institution. Our practice of routinely admitting patients was changed in October 2020 largel

[THE OPERATIVE AND POSTOPERATIVE RESULTS OF PERCUTANEOUS NEPHROLITHOTOMY FOR THE ELDERLY IN COMPARISON TO A YOUNGER POPULATION].

INTRODUCTION: Kidney stone disease is very common in the western world and combined with the aging of the population, it brings about the need to treat older patients with numerous comorbidities. Percutaneous nephrolithotomy (PCNL) is the standard of care for patients with stones 2 cm and above but is more invasive. Thus, with the elderly patient comes the dilemma: whether to choose the percutaneous path or to proceed with ureteroscopy even at the cost of several procedures in order to complete the task at hand. Recent developments also added the choice of smaller working channels and the opti
