[Criteria to indicate kidney biopsy in type 2 diabetic patients with proteinuria: Survey among French nephrologists].

Diabetic nephropathy is usually a presumptive diagnosis based on clinical and biological evidence. Renal biopsies are performed in diabetic patients with atypical findings evoking non-diabetic renal disease who could benefit from specific therapies. French speaking nephrologists were asked which criteria they retain to indicate renal biopsy in patients with type 2 diabetes and albuminuria>0.5g/day or equivalent through an online anonymous questionnaire. Among the suggested criteria were absence of diabetic retinopathy, hematuria, rapid decrease in GFR, short diabetes duration or rapid raise of

Ten Tips Nephrologists Wish the Palliative Care Team Knew About Caring for Patients with Kidney Disease.

Many patients with chronic kidney disease (CKD) and end-stage renal disease have unmet palliative care (PC) needs. Physical and emotional symptoms are common. Some, like uremia and fluid overload, improve with dialysis, but the increasing age of patients initiating renal replacement therapy leaves many untreatable comorbidities like dementia and frailty to negatively impact quality of life. Written by nephrologists and PC clinicians, this article will help PC providers to have a richer understanding of kidney disease-related symptom burden, disease trajectory, prognosis, and barriers to hospic

[French nephrologists’ practices in the management of anemia and iron deficiency in chronic kidney disease].

BACKGROUND: Real-world practices for managing anemia and iron deficiency in chronic kidney disease appear heterogeneous and sometimes controversial. This study aimed to describe the prescribing preferences and habits of French nephrologists in this area. METHODS: All nephrologists seeing patients at one of the 40 centers participating in the Chronic Kidney Disease – Renal Epidemiology and Information Network (CKD-REIN) cohort were invited to participate in two waves (2015–2016 and 2019–2020) of a practice survey. The self-administered questionnaires collected information on nephrologists’ cha

Expert Opinion on the Conceptual, Clinical, and Therapeutic Aspects of Fabry Nephropathy: A Nephrologists' Perspective.

BACKGROUND: Progressive nephropathy (Fabry nephropathy), as observed in both the early-onset and later-onset types of Fabry disease and in as many as 50% of males and 20% of females, represents a major driver of overall disease burden in this population. Nonetheless, recognizing and diagnosing Fabry nephropathy remains clinically demanding for nephrologists given its pivotal role in altering disease progression and improving patients' quality of life. Therapeutics presents additional complexity as evidence-based criteria for treatment and prognostic algorithms remain limited. SUMMARY: Against

Evaluation of a Training Program in Peritoneal Dialysis Catheter Insertion for Nephrologists in South America.

UNLABELLED: <p>Introduction: For patients requiring renal replacement therapy, peritoneal dialysis (PD) offers an alternative to maintain quality of life. The long-term success of PD depends on using a safe, functional, and durable peritoneal catheter (PC). This study aimed to assess the outcomes of a training program for nephrologists on PD catheter insertion in South American dialysis centers. METHODS: This longitudinal, retrospective, multicenter study was conducted in Colombia, Chile, Ecuador, and Bolivia. Patients who underwent PC insertion between January 2022 and May 2023 were included

Acute Kidney Injury Secondary to Aortic Dissection Following Transcatheter Aortic Valve Implantation: Perspectives from Nephrologists.

Aortic dissection (AD) is a rare but serious complication of transcatheter aortic valve implantation (TAVI) and should be considered in the differential diagnosis of acute kidney injury (AKI). We herein report an older man who developed AKI secondary to Stanford type B AD after transfemoral TAVI. Non-contrast computed tomography (CT) revealed inward displacement of the calcified intima, which is a key early finding. Contrast-enhanced CT confirmed AD extending to the renal artery. As endovascular intervention was not feasible, conservative management was selected. Nephrologists should recognize

From combined heart-kidney to kidney transplantation program: What nephrologists should know about dilated cardiomyopathy.

A 42-year-old hemodialysis (HD) patient was investigated in our department for symptomatic heart failure (HF) despite daily home dialysis. He had a history of living donor kidney transplantation at the age of 18 that lasted 7 years. Home dialysis was then started. At the age of 40, he developed acute heart failure symptoms. Echocardiography revealed severe dilated cardiomyopathy (DCM). Coronarography and myocardial perfusion scintigraphy showed no abnormal findings. Betablockers were administrated and RAAS inhibitor dosing was optimized. Dyspnea persisted and patient was referred to our depart

Liver support--a task for nephrologists? Extracorporeal treatment of a patient with fulminant Wilson crisis.

BACKGROUND: Patients with Wilson's disease may present with cirrhosis, acute hepatitis or fulminant hepatic failure. Without urgent orthotopic liver transplantation, a fulminant Wilson crisis has a mortality of 100%. We report on an 18-year-old female patient with fulminant hepatic failure due to Wilson crisis. METHODS: The molecular adsorbent recirculating system (MARS) was used to eliminate albumin-bound toxins and to bridge waiting until an organ became available. RESULTS: A total of 18 MARS sessions and 4 plasma exchange sessions were performed. Bilirubin levels and hepatic encephalopath

Nephrologist Affiliation With Dialysis Facilities and Patient Trajectories in End-Stage Kidney Disease.

IMPORTANCE: Across the treatment modalities for end-stage kidney disease (ESKD), kidney transplantation offers the best survival and quality-of-life outcomes, yet most patients are unable to receive a transplant, making dialysis the most common long-term treatment option. Despite its advantages, home peritoneal dialysis (PD) use remains limited in the US, with financial incentives potentially contributing to this underutilization. OBJECTIVE: To examine the association between nephrologist affiliation with dialysis facilities and facility characteristics, including profit status and PD service

Nephrologist follow-up improves outcomes after hospitalization with acute kidney injury.

INTRODUCTION: Acute kidney injury (AKI) is a common medical condition for patients during hospitalization and is associated with long-term sequelae. The incidence of AKI is increasing, whereas in-hospital mortality from the condition is decreasing. We compared outcomes following discharge in patients with AKI between those who received follow-up with nephrologists and those who only received routine follow-up (i.e., follow-up not at the nephrology department). MATERIALS AND METHODS: This retrospective cohort study identified 66,859 AKI survivors with discharge eGFR values of less than 60 mL/m

A practical guide for nephrologist peer reviewers: understanding and appraising Mendelian randomization studies.

Identifying risk factors for disease onset and progression has been a core focus in nephrology research. Mendelian Randomization (MR) has emerged as a powerful genetic epidemiological approach, utilizing genome-wide association studies (GWAS) to establish causal relationships between modifiable risk factors and kidney disease outcomes. MR uses genetic variants as instrumental variables to infer causal relationships between exposures and disease outcomes. This method leverages the natural randomization of genetic variants to balance confounders, akin to matched cohorts in observational research

The Nephrologist's Perspective in Evaluation and Management of Localized Renal Masses: Core Curriculum 2025.

The evaluation and management of renal masses, predominantly of the clear cell renal cell carcinoma subtype, have been evolving over the past several years and demanding greater attention from nephrologists. Oncologic survival of localized tumors approaches 100%, where survival is more closely tied to underlying comorbidities including chronic kidney disease (CKD). Early diagnosis and thus increasing prevalence related to incidental discovery of renal masses allows greater emphasis on nephron-sparing procedures and for residual kidney function preservation. Compared with radical nephrectomy, p

Acute kidney injury following CAR-T cell therapy: a nephrologist's perspective.

Chimeric antigen receptor T (CAR-T) cell therapy, an emerging personalized immunotherapy for various haematologic malignancies, autoimmune diseases and other conditions, involves the modification of patients' T cells to express a chimeric antigen receptor that recognizes tumour or autoimmune cell antigens, allowing CAR-T cells to destroy cancerous and other target cells selectively. Despite remarkable clinical improvements in patients, multiple adverse effects have been associated with CAR-T cell therapy. Among the most recognized adverse effects are cytokine release syndrome, immune effector
