Role of the neurologist in the evaluation and treatment of patients with trigeminal neuralgia.

The neurologist, although not usually the first healthcare provider to evaluate a patient with trigeminal neuralgia (TN), is often involved in confirming the diagnosis and managing the patient's pain with medications. The neurologist has several other important roles for patients with TN: assessing and reducing the individual's pain, patient and family education, and encouraging referral to a neurosurgeon for pain-reducing procedures when appropriate. In general, surgical procedures for TN should be considered when the patient does not attain pain relief after adequate trials of two or three m

[Transient cerebral ischemic complications. The neurologist's point of view].

Transient ischaemic attacks (TIA) are defined by the focal and sudden loss of a cerebral function or the vision of one eye, resolving without sequelae within 24 hours and related to a vascular cause, thromboembolic much more frequently than haemodynamic. TIA represent between 9% and 25% of all cerebrovascular accident (CVA) with a variable global incidence from one study to another, between 0.2 and 3.3/1,000/year. The natural history of TIA is characterized by an excess mortality and an increased risk of cerebral infarction and myocardial infarction. It is therefore essential to recognize thes

Indications for stereotactically-aided differential diagnosis: the neurologist's view.

The contribution of stereotactic biopsy (SB) in three pathological conditions; neoplastic disease, immunodeficiency states and degenerative cerebral atrophy, is reviewed. When craniotomy is contraindicated, SB is the most reliable method for demonstrating the neoplastic nature and the degree of malignancy of an intracranial space occupying lesion and may help to delineate it limits. In immunodepressed patients, SB is frequently used to differentiate tumours from opportunistic infections. In degenerative dementia, SB is susceptible of differentiating Alzheimer's disease from other degenerative

A Primer on COL4A1/COL4A2-Related Cerebral Small Vessel Disease for Neurologists.

COL4A1 and COL4A2 encode type IV collagen, a principal structural component of basement membranes, and pathogenic variants in these genes cause a multisystem small vessel vasculopathy with prominent cerebrovascular involvement. Despite increasing recognition with the expansion of genetic testing, COL4A1/2-related cerebral small vessel disease remains underrecognized in clinical practice, in part due to its striking phenotypic heterogeneity. Presentations span a broad clinical spectrum-from asymptomatic individuals or incidental imaging findings to recurrent ischemic events, intracerebral hemor

Treatment Preferences of Neurologists in Generalized Myasthenia Gravis: A Conjoint Analysis Study.

INTRODUCTION: Generalized myasthenia gravis (gMG) is a chronic autoimmune neuromuscular disorder for which treatment decision-making is increasingly complex due to the emergence of biologic targeted therapies. This study aimed to evaluate neurologists' treatment preferences in acetylcholine receptor seropositive gMG using conjoint analysis to simulate real-world clinical decision-making. METHODS: We conducted a cross-sectional, web-based study among neurologists involved in the management of gMG in collaboration with the Spanish Society of Neurology. Participants were presented with eight hyp

Association of British Neurologists (ABN) autoimmune myasthenia gravis management guidelines (2025 update).

The 2025 update of the Association of British Neurologists guidelines for the management of autoimmune myasthenia gravis (MG) emphasises several points that are distinct from the 2015 guidelines based on recent research and publications. The main differences from the previous guidelines are: (1) The recommendation to prescribe daily steroids rather than the alternate day regimen is now standard practice. (2) There is a clear emphasis on the beneficial effects of early thymectomy. (3) Randomised controlled trial evidence now supports early use of rituximab (within 1 year of generalised disease

Electroencephalographic compatibility with fitness to drive: A nationwide survey among Swiss neurologists.

OBJECTIVE: The Swiss guidelines for driving with epilepsy require that electroencephalogram (EEG) findings must be compatible with the fitness to drive (FTD) without specifying any criteria. This nationwide survey investigated how Swiss neurologists implement this requirement in clinical practice. METHODS: An online survey, including 19 EEG examples and statements about the compatibility of the EEG with FTD, was distributed to all members of the Swiss Society of Clinical Neurophysiology and all Swiss neurological clinics with residency programs. Descriptive statistics and Fleiss' Kappa for in

Dopamine Transporter Scan (DAT) in Parkinsonism - a Short Review for Non-Neurologists.

The Dopamine Transporter Scan (DaT) is a radionuclear imaging technique which was approved by the FDA to differentiate essential tremor (ET) from Parkinson's disease (PD). The scan is a crude indicator of the number of dopamine-secreting cells and is abnormal in presynaptic parkinsonian syndromes. In this article we review this and other possible clinical situations in which a DaT scan may be useful.

Primary care providers perform more neurologic visits than neurologists among Medicare beneficiaries.

RATIONALE, AIMS AND OBJECTIVES: Little is known about which medical providers, other than neurologists, are involved in the care of neurologic conditions. We aimed to describe the current distribution of outpatient neurologic care by provider type. METHODS: We conducted a restrospective, cross-sectional analysis using a 20% national sample claims database that contains information on medical care utilizations from adult Fee-for-Service Medicare beneficiaries in 2015. We identified patient visits for evaluation and management services for common neurologic conditions and by medical provider ty

[German neurology and neurologists during the Third Reich: brain research and "euthanasia"].

The connection between systematic killing of the mentally ill and disabled, euphemistically called "euthanasia" in the National Socialism ideology, and German brain research has been thoroughly investigated and in detail; however, the impact of this criminal nexus on the image and self-perception of German neurologists as well as the status of neurology as a medical discipline is still the subject of controversial debates.Between 1939 and 1945 the Kaiser Wilhelm Institute (KWI) in Berlin along with other research centres were insofar enmeshed in the "euthanasia" program as brains of killed pat

Neurocysticercosis is a neglected microbleed mimic. a cautionary note for stroke neurologists.

BACKGROUND: Calcified cysticerci share the morphology and signal properties with true cerebral microbleeds, but this condition is neglected at the time of evaluating MRIs for clinical or research purposes. We report our experience with the MRI reading obtained from the Atahualpa Project, a population-based stroke study, conducted in a village endemic for cysticercosis. METHODS: Neuroimaging studies were performed in Atahualpa residents aged ≥60 years identified during a door-to-door survey. Lesions of interest included neuroimaging markers of small vessel disease. Out of 311 residents, 248 (8

Comparison of clinical gait analysis strategies by French neurologists, physiatrists and physiotherapists.

Clinical and functional gait analysis is used widely by different professionals dealing with patients with hemiplegia. The aim of this study was to examine the gait analysis strategies of neurologists, specialists in physical and rehabilitation medicine (physiatrists) and physiotherapists. Differences in global analysis strategy and choice of indicators between different clinicians have not previously been studied precisely, and we believe that a standardized approach would enhance the training of young practitioners. The knowledge acquisition phase (specialists' expertise identification) was

Diagnosis of motor neuron disease by neurologists: a study in three countries.

Ninety four neurologists in the United Kingdom, China, and West Germany responded to two structured questionnaires. The first assessed the diagnostic weighting assigned to a number of symptoms, signs, and clinical investigations ascertained from classical descriptions and case notes of patients with motor neuron disease (MND). The second tested the likelihood and consistency of diagnosis in a series of case summaries representing the clinical data of 10 patients with clinically and pathologically documented motor neuron disease. There was a wide measure of agreement concerning the common clini

Major depression, anxiety disorder and suicidality in epilepsy: What should neurologists do?

Four to five patients with epilepsy (PWE) can suffer from Major Depressive episodes (MDE). Comorbid anxiety disorders (AD) frequently occur together with MDE. Failure to treat MDE can negatively affect several aspects of their life as well as the management of the epilepsy. Often, suicidal ideation is among its symptoms, which need to be addressed without delay to prevent suicidal attempts or a completed suicide. Unfortunately, access to health care professionals is very limited and, in many communities, non-existent. Accordingly, it falls upon the treating neurologist to begin a pharmacologic

Fetal Callosal Anomalies: A Narrative Review and Practical Recommendations for Pediatric Neurologists.

Agenesis of the corpus callosum is a common indication for fetal neurology consultation, increasingly identified through advances in fetal sonography and fetal magnetic resonance imaging. Despite improvements in diagnostic accuracy, prognostic counseling is challenging due to highly variable neurodevelopmental outcomes. Several factors contribute to neurodevelopmental outcome variability, including associated anomalies and etiologic considerations such as genetic, acquired, and environmental factors. This narrative review discusses existing literature on prenatal findings, postnatal outcomes,
