[The present and future of neurophysiological examination].

This paper discusses the following routine examinations and new neurophysiological examinations that will likely be adopted in future. 1) EEG: a) The source derivation method and intracranial EEG recording method are useful for the detection of the exact focus in epileptic seizure. b) Polysomnography is applied to sleep disorders (ex. apnea syndrome). c) EEG topograph mapping and brain electric source analysis (BESA) are not only applied to neurological disorders, but also used for the detection of electrical equivalent dipole source localization. 2) Evoked potential (EP): ABR, SSEP and P-VEP

Neurophysiologic bases of speech discrimination.

The mismatch negativity (MMN) is an automatic cortical evoked potential that signifies the brain's detection of acoustic change. In other words, the MMN reflects the neurophysiologic processes that underlie auditory discrimination. As such, the MMN provides an objective tool for evaluating central auditory mechanisms involved in speech perception. We are using the MMN to study the central auditory processes that encode acoustic changes important for speech perception in 1) normal-hearing adults and children, 2) individuals with impaired auditory systems (including persons with learning disabil

Neurophysiological consequences of nitroxide antioxidants.

Nitroxides are antioxidant compounds that have been shown to provide radioprotection in vivo and in vitro. Radioprotection in vivo is limited by toxicity, which appears to be neurologic in nature. To further evaluate the toxicity of these compounds, three representative nitroxides, Tempol, Tempamine, and Tempo, were examined in slices of guinea pig hippocampus. Each nitroxide increased the population spike and caused potentiation of excitatory postsynaptic potential--spike coupling. Repetitive activity and epileptiform activity were observed at the highest concentrations of Tempo and Tempamine

Propriospinal myoclonus: a neurophysiologic analysis.

We are reporting a neurophysiologic analysis of two patients presenting with thoracoabdominal spontaneous muscle jerks. Polymyographic recordings showed myoclonic bursts with onset in the upper rectus abdominis or lower intercostal muscles followed by rostral propagation to the upper intercostal and caudal propagation to the abdominal muscles by slowly conducting pathways. Jerk-locked back-averaging did not show time-locked cortical or premovement potentials. Peroneal somatosensory evoked response, C-reflex, and intercostal nerve conduction were normal. These findings suggest a generator for t

[Neurophysiologic study of the phrenic nerve].

Phrenic nerve conduction was studied in 19 volunteers using two different percutaneous methods of stimulation, mono and bipolar. A response was obtained in all nerves studied. The results are comparable using mono or bipolar stimulation method. No significant difference in latency, amplitude, or intensity stimulation applied was found between left and right sides. There was no correlation of age or height with the latency. Latencies above 10 ms or differences between right and left above 1.6 ms are abnormal values. We studied two patients with neurological disorders, one of them with Guillain-

Neurophysiologic assessment of the anal sphincters.

One hundred twenty consecutive patients with either fecal incontinence (60 patients), chronic constipation (41 patients), or idiopathic intractable pelvic pain (19 patients) were prospectively assessed. Patients underwent concentric needle electromyography (EMG), bilateral pudendal nerve terminal motor latency evaluation, anorectal manometry, and cinedefecography. The most common EMG finding in patients with fecal incontinence was decreased recruitment of motor units with squeezing and polyphasic motor unit potentials; these are consistent with an injury pattern. The most common EMG finding in

Neurophysiologic assessment of endolymphatic hydrops.

An electrophysiological method for assessing endolymphatic hydrops of the cochlea in an animal model of Meniere's disease is described. Eighth nerve gross action potentials (AP) were evoked by tone bursts presented at discrete phases of a simultaneously delivered 50-Hz pure tone. Hydropic cochleas showed significantly less modulation of AP amplitude and latency than normal cochleas. This method may be useful in objectively evaluating Meniere's disease with electrocochleography.

Neurophysiological studies on conjoined twins.

EEG, VEP and BAEP studies were performed on conjoined (craniopagus parieto-occipitalis) twins, born prematurely without any common neural tissue, but with shared venous channels and contiguous posterior poles. At thirty-three wks (CA) the EEG of one of the twins showed higher amplitudes and less quiescence, suggesting a more mature cerebrum but the same twin also showed longer latencies in the BAEP, suggesting a less mature brainstem. Sleep spindles at four and a half months frequently occurred at the same time in both of the twins, at times synchronous and at other times with a latency of ons

Neurophysiological studies in GM1, gangliosidosis.

Neurophysiological studies (EEG, ERG, VEP) have been carried out on 8 children with proven GM1 gangliosidosis (3 of Type I and 5 of Type II). All the EEGs were abnormal showing an increasing amount of irregular slow activity as the disease progressed. Around 2 to 3 years of age, Type II patients often showed a fluctuating 4-5 c/s rhythmic activity especially prominent in the temporal regions. Paroxysmal activity was not a conspicuous feature in any of the patients. The ERG was normal in all cases but the VEP was variably altered. The EEG/ERG/VEP findings in GM1 gangliosidosis differ from those

Neurophysiological study of newborns with hypocalcemia.

The EEG, sleep cycle, visual and auditory evoked potentials (VEP and AEP) were studied in 38 newborns who presented with seizures and had serum calcium levels below 7 mg/dl. Nineteen infants displayed increased theta and alpha, and decreased delta waves with variable but mild degrees of background EEG depression during sleep, which may reflect neuronal hyperexcitability induced by hypocalcemia. Hypocalcemic infants spent less time in quite sleep than control infants, but the time spent in active sleep did not change significantly. Seventy-seven percent of seizures occurred in active sleep. 17%

A proposed neurophysiological basis of alexithymia.

The possibility that alexithymia respresents a symptom of subtle brain dysfunction is discussed. Several lines of research have shown that emotional thought, behavior and, in particular, the use of affect-laden speech are influenced by impaired function of the right hemisphere or a disconnection between the two hemispheres. A patient with agenesis of the corpus callosum is presented demonstrating severe alexithymia.

Laryngeal spasm: a neurophysiologic redefinition.

Within the limits of standard neurophysiologic techniques, we have attempted to redefine laryngeal spasm as distinct from the glottic closure reflex. This distinction is based upon the observation that laryngeal spasm is solely mediated by the superior laryngeal nerve. Stimulation of other afferent nerves, capable of eliciting the glottic closure reflex, produces little adductor after-discharge activity that is characteristic of laryngeal spasm. In this regard, modification of output function from the adductor motoneuron aggregate by means of temporal and spatial summation of sensory input dat

The evolution of neurophysiological features in holoprosencephaly.

The evolution of EEG, visual and auditory evoked responses (VER and AER) and sleep is described in three cases of semilobar holoprosencephaly. During the neonatal period, the waking EEG was characterized by almost continuous high amplitude rhythmic alpha-theta activity in case 1 and 2, which became discontinuous during quiet sleep. Moderate amplitude rhythmic alpha-theta waves were seen in case 3. This rhythmic alpha-theta activity gradually disappeared with increasing age, being replaced by non-specific slow dysrhythmia. In case 3, the subsequent EEGs were characterized by focal spikes at 4 m

Advancing the Neurophysiological Understanding of Delirium.

Delirium is a common problem associated with substantial morbidity and increased mortality. However, the brain dysfunction that leads some individuals to develop delirium in response to stressors is unclear. In this article, we briefly review the neurophysiologic literature characterizing the changes in brain function that occur in delirium, and in other cognitive disorders such as Alzheimer's disease. Based on this literature, we propose a conceptual model for delirium. We propose that delirium results from a breakdown of brain function in individuals with impairments in brain connectivity an
