[Dermatological aspects of AIDS in western Africa. Apropos of 140 cases].

This clinical survey, carried out during a 3 years period in the Dermatological Department of Bouake Hospital (Ivory Coast) analyses the skin and mucous membranes troubles caused by AIDS among adults. It is the first of that kind in Western Africa. 140 patients were concerned, showing carious dermatological troubles, 25 of them were counted. Few tropical skin diseases have their clinical picture altered by immunodeficiency. However, the Buruli ulcer may be described, which has in this condition a particularly development. We have underlined the particularities of some ubiquitous diseases, eith

[Iatrogenic palatine necrosis by embolization of sphenopalatine arteries during management of a rebel epistaxis].

INTRODUCTION: The treatment of epistaxis sometime requires an embolization. This may result in ischemic palate necrosis, oronasal communication and dental losses. The repair of these lesions is complex. OBSERVATION: A 53-year-old patient, suffering from high blood pressure and hypercholesterolemia, benefited from bilateral embolization of the sphenopalatine arteries in order to treat a persistent epistaxis. He developed a middle cerebral artery stroke in the aftermath. During the treatment of the neurological sequelae, a necrosis of the 2/3 of the posterior aspect of the hard palate was disco

Oral health in Africa: a neglected public health priority.

Oral health in Africa is a significantly neglected public health priority, impacting approximately 480 million people (43.7% of the WHO African region population). Despite its critical link to general well-being and other Non-Communicable Diseases (NCDs), the continent faces a high burden of conditions like untreated dental caries, severe periodontal disease, and life-threatening conditions such as Noma. Key challenges include a historic lack of prioritisation in health policy and resource allocation, a critically under-resourced oral health workforce (ratio of 0.44 professionals per 10,000 pe

Nasal reconstruction in children: a review of 29 patients.

Acquired large nasal defects are much more common in adulthood than in childhood because of the frequency of skin tumors after a certain age. However, from their experience in treating a number of children with sequelae of noma and burns, the authors have collected a series of 17 total and 12 partial nasal reconstructions in children aged 1 to 15 years. After reviewing the various methods used for recreating the lining, the support, and the skin cover in the whole series, three cases are reported in detail. A 1-year-old patient received a tempororetroauricular flap after total amputation of th

ATP-sensitive potassium channels: A double-edged sword in neurodegenerative diseases.

ATP-sensitive potassium channels (KATP channels), a group of vital channels that link the electrical activity of the cell membrane with cell metabolism, were discovered on the ventricular myocytes of guinea pigs by Noma using the patch-clamp technique in 1983. Subsequently, KATP channels have been found to be expressed in pancreatic β cells, cardiomyocytes, skeletal muscle cells, and nerve cells in the substantia nigra (SN), hippocampus, cortex, and basal ganglia. KATP channel openers (KCOs) diazoxide, nicorandil, minoxidil, and the KATP channel inhibitor glibenclamide have been shown to have

Facial bone reconstruction with prefabricated vascularized calvarium flaps in children and young adults: Advantages and long-term results.

INTRODUCTION: Reconstruction of facial bone defects in children is challenging. The use of well-vascularized bone is mandatory to obtain stable lasting results. This study reports our experience of facial bone reconstruction using prefabricated vascularized calvarium flaps. METHODS: Retrospective case series of 50 patients who underwent 52 maxillary, malar, and mandibular reconstructions between 1988 and 2014 using prefabricated vascularized calvarium flaps. Forty-nine patients suffered from noma sequels; one patient had craniofacial cleft Tessier 3-11. Surgery consisted of a two-step procedu

Reconstruction of complex oro-facial defects using the myocutaneous sub-mental artery flap.

Oro-facial defects require reconstruction that provides suitable colour match and texture. Moreover inner and outer cheek lining and bulk are key considerations. In cases of severe oro-facial infections concomitant mandibular abnormality, for example trismus, can mandate the need for tissue to obturate mandibular defects. We assessed the use of the myocutaneous sub-mental artery flap (MSA) in non-oncological patients with such defects. Twenty two consecutive patients were included in this case series. All patients were survivors of Cancrum Oris (NOMA). Demographic details, nutritional status a

Oral fungal and bacterial infections in HIV-infected individuals: an overview in Africa.

Oral opportunistic infections developing secondary to human immunodeficiency virus (HIV) infection have been reported from the early days of the epidemic and have been classified by both the EC-Clearinghouse and the World Health Organisation (WHO). Among the fungal infections, oral candidiasis, presenting in African HIV-infected patients has been sporadically documented. We review the literature with respect to candidal carriage, oral candidiasis prevalence and the predictive value of oral candidiasis for a diagnosis of underlying HIV disease in African HIV-infected patients. The use of oral c

Nutrition and oral health in Africa.

Poverty, low birth weight, low life expectation at birth, widespread malnutrition, numerous endemic infections, little or no access to safe water, poor oral hygiene, deplorable environmental sanitation and political instability among other problems, characterise the lives of many Africans, particularly in sub-Saharan Africa. In African countries undergoing rapid urbanisation, health problems associated with undernutrition and overnutrition coexist, and these result from lifestyle changes which promote physical inactivity, increased consumption of fats and refined carbohydrates, as well as abus

Accurate and fully automated diameter measurements of Circle of Willis arteries on MRA imaging.

The Circle of Willis (CW), visualized via Magnetic Resonance Angiography (MRA), is crucial for assessing cerebral circulation. Accurate artery identification is essential not only for detecting stenosis and pathological changes but also for understanding vascular adaptations in healthy aging. Manual CW assessment is time-consuming, necessitating automated alternatives. This study evaluates intracranial artery diameter estimations from the Express IntraCranial Arteries Breakdown (eICAB) toolbox against manual measurements. eICAB was tested on 631 participants from the Northern Manhattan Study (

Sketches of surgical cases drawn in 1884-87 at the East London Hospital for Children, Shadwell.

Most of the surgical cases were under the care of the Surgeon Robert William Parker, who was on the staff of the East London Hospital for Children, Shadwell. He was born in 1842 and qualified in 1869, and after working at the London Hospital he served with distinction as a volunteer with the Anglo-American Ambulance in the Franco-Prussian War 1870-1871. Then he became Resident Medical Officer at the Children's Hospital, 49 Great Ormond Street, and later in 1876 joined the staff of the Shadwell Hospital and was an active member of several London medical societies at which he presented surgical

Measles in Africa: a deadly disease. Some personal comments.

Measles dominate childhood illnesses in tropical Africa with a variety and severity of complications virtually unknown in the West. There is no apparent continental variation in measles virus but environmental factors vary widely. The frequent association of severe measles with malnutrition and recent studies indicating impaired humoral, cellular, and mucocutaneous immunologic function in the malnourished may explain this perplexing disease. Prevention of early malnutrition combined with lasting and economic early immunization with heat-stable vaccine, currently unavailable, are needed to halt

The association of common infectious exposures with cognitive performance in community-dwelling older adults.

INTRODUCTION: The Northern Manhattan Study (NOMAS) previously identified that a combined infectious disease exposure index correlates with impaired cognitive performance in older adults. METHODS: We extended these findings by examining the association of serological titers of five common infectious diseases (herpes simplex virus [HSV] 1 and 2, cytomegalovirus [CMV], Chlamydia pneumoniae, and Helicobacter Pylori) with domain-specific cognitive performance and incident cognitive impairment and dementia in 593 community-dwelling older adults. We performed confounder-adjusted mixed linear regress

The origins of oral medicine in the Hippocratic collected works.

The collected works of Hippocrates describe for the first time in a systematic way a large number of oral diseases, such as ulcers, inflammations, abscesses and tumours from the epiglottis, mouth, tongue, palate, uvula and the sublingual area. Several of these case reports are remarkable for the accurate observation of clinical symptoms and signs, the aetiology, the pathogenesis and their therapeutic approach in relation to prognosis. The Hippocratic authors report cases of aphthae as part of a polysystemic disease, described many centuries later by Behçet and Adamantiades, while they associat
