Adherence to antiretroviral therapy in HIV-infected pediatric patients improves with home-based intensive nursing intervention.

Adherence to combination antiretroviral therapy (ART) has been shown to be a determining factor in controlling viral replication, maintaining immunologic function and long-term survival in HIV-positive individuals. Little information is available on strategies to improve adherence in pediatric HIV-infected patients. We conducted a randomized, nonblinded, pilot study to determine if a home-based nursing intervention would improve medication adherence. The study was offered to all eligible HIV-positive patients receiving care at Connecticut Children's Medical Center's (CCMC) Pediatric and Youth

Early Outcomes of Nurse-Led Outreach for Lost to Follow-Up Patients after Pediatric Heart Surgery.

AIM: The primary aim of this paper is to review the efficacy of a clinical outreach program aimed at reducing the number of CHD survivors who are lost to follow-up (LTF). The secondary aim was to identify common reasons for the delays in timely follow-up care. METHODS AND RESULTS: A retrospective review was initially conducted. A total of 1555 records of patients who had congenital heart surgery over a 33-year period (1989 to 2022) were reviewed for follow-up status. Patients who had not been seen by a cardiology provider for at least 18 months at the time of review were deemed LTF and were e

Legacy at the Bedside: A Nurse-Led Evidence-Based Framework for Memory-Making in the Pediatric Intensive Care Unit.

Children who die in the pediatric intensive care unit (PICU) and their families often face sudden deterioration, limited time, invasive technology, and emotionally overwhelming decisions. Memory-making can help families preserve connection, support parenting and sibling participation, and create tangible or digital keepsakes, yet bedside teams may feel uncertain about when and how to offer these practices without adding distress. This evidence-based practice article synthesizes recent pediatric palliative care, bereavement, and critical care literature and translates it into a nurse-led bedsid

Assessment of a Stepwise Intervention to Improve Nurse-administered Penicillin Allergy Screening and De-labeling in Pediatric Inpatients.

INTRODUCTION: Penicillins are first-line treatments for many childhood infections, but providers avoid them when patients report a penicillin allergy, although approximately 94% of these patients may tolerate penicillin. Patients with penicillin allergy labels often receive second-line antibiotics, which increases the risk of treatment failure and adverse events. To address this, programs to implement penicillin allergy de-labeling have increased; however, strategies to accomplish de-labeling most efficiently through guideline dissemination have not been well-studied. METHODS: A multidiscipli

Clinical Competence for Youth Suicide: Use of Simulation in Pediatric and Psychiatric-Mental Health Nurse Practitioner Programs.

Approximately one in five adolescents in the United States has a mental health concern and suicide is the second leading cause of death in this population. It is vital to prepare nurse practitioner (NP) students with the required skills to assess and manage youth with suicidal ideation. Unfortunately, it is challenging for NP students to accumulate these critical competencies due to limited preceptors and the infrequent presentation of suicidality in certain clinical sites. Furthermore, the serious nature of a child in mental health crisis often requires students to take a passive role while d

Differences in pain assessment and decisions regarding the administration of analgesics between novices, intermediates and experts in pediatric nursing.

This article describes a study examining the influence of expertise on nurses' pain assessments and decisions regarding pharmacological interventions in children. In an experimental design, novices (n = 271), intermediates (n = 222), and experts (n = 202) in pediatric nursing, various cases were presented. Each case consisted of a combination of a vignette and a video. Subjects were asked (1) to assess the child's pain intensity, (2) to specify their confidence in the assessment, and (3) to state whether or not they would administer a non-narcotic analgesic. The results indicated that expertis
